Summary: The current U.S. lung transplant mortality risk models do not consider the disease progression of patients over time. This study investigated the impact of accrued waitlist time on mortality in lung transplant candidates and recipients beyond clinical deterioration and proposed a new framework for conceptualizing mortality risk in end-stage lung disease.

Summary: This study aimed to investigate the impact of 'prophylactic' veno-arterial extracorporeal membrane oxygenation (VA ECMO) and tacrolimus on lung transplant recipients. The results showed that 'prophylactic' VA ECMO had comparable 72-hour primary graft dysfunction (PGD) values to the 'off-pump' group, while tacrolimus was associated with a lower risk of 30-day acute cellular rejection.

Summary: The impact of surgical learning curve on short-term clinical outcomes of patients after bilateral lung transplantation (LTx) was examined. It was found that the surgeon's experience was negatively associated with the total operating time and total anastomosis time. The study also revealed that a surgical multidisciplinary team can safely perform double LTx after 20 procedures, with more favorable short-term outcomes in the late group.

Summary: The study found that smoking history did not significantly affect transplant outcomes for recipients, but was associated with a longer post-operative course. Donor and recipient characteristics were similar, and there were no significant differences in morphological lesions of donor lungs.

Summary: This study analyzed the impact of obesity on early morbidity and mortality risk in patients undergoing liver transplantation. The results revealed significantly increased rates of vascular and biliary complications and primary graft nonfunction in obese patients. Furthermore, obese patients had a higher risk for worse 1-year graft survival compared to nonobese controls.

Summary: The association of changes in skeletal muscle mass and quality during the waiting time with outcomes of lung transplantation (LT) remains unclear. Maintaining or increasing skeletal muscle mass during the waiting time would be beneficial to improve the short-term outcomes of LT.

Summary: The aim of our study was to compare the short-term outcomes between obese and non-obese patients who underwent off-pump coronary artery bypass surgery. The results showed no significant difference in in-hospital mortality between the two groups. However, obese patients had a lower wound infection rate while non-obese patients had a lower dialysis rate. These findings suggest that off-pump coronary artery bypass surgery is a safe procedure for obese patients.

Summary: Split liver transplantation (SLT) can expand the donor pool and address the issue of organ shortage. The study found that the short-term outcomes of pediatric SLT were not affected by donor age, but the survival rate was lower when the donor age was between 45-55 years.

Summary: This study investigated the prognostic significance of N-terminal pro-B-type natriuretic peptide (NT-proBNP) in lung transplant candidates. The results showed that NT-proBNP levels were associated with age, lung function, and pulmonary artery pressure, and higher levels were associated with increased mortality among the candidates.

Summary: This study investigated the relationship between multiple frailty models before lung transplantation and short-term outcomes after surgery. It found that cumulative deficit models of frailty were associated with outcomes within the first 90 days, while phenotypic frailty was associated with 1-year survival.

Summary: The study assessed the prognostic role of preoperative BMI on survival in NSCLC patients undergoing lung resection, finding that obesity may have a protective effect for patients with lung cancer. Obese patients had improved survival compared to normal weight and underweight patients, with BMI being a strong and independent predictor of survival in NSCLC patients undergoing surgery.

Summary: In patients awaiting lung transplantation, awake ECMO strategy leads to better post-operative outcomes than non-awake ECMO strategy, with shorter ICU stay, longer ventilator free days, higher gait ability, and improved lung function at 6 months and 1 year. Additionally, awake ECMO group has lower mortality rates at 6 months and 1 year compared to non-awake group.

Summary: This study aimed to identify factors associated with survival beyond a decade after lung transplant. The results showed that approximately one-quarter of lung transplant recipients survived 10 years post-transplant. There were different factors associated with 1-year and 10-year survival, and the Lung Allocation Score did not impact long-term survival. Further research is needed to improve patient selection and management strategies to increase long-term survival.

Summary: This study conducted a longitudinal study of 281 children with CF and found that children who developed CFRD had lower height z-scores from a young age, while HbA1c levels started to rise closer to CFRD ascertainment. Height z-scores and HbA1c levels were associated with the risk of developing CFRD.

Summary: Cystic fibrosis (CF) is a genetic disease caused by mutations in the CFTR gene, resulting in dysfunctional ion channels. The management of CF has shifted from treating downstream disease manifestations to addressing the underlying gene defect. The emergence of CFTR modulators has revolutionized CF demographics, prognosis, and therapies, including nutritional management. This review explores the current state of knowledge on nutritional management in individuals with CF in the era of CFTR modulation.
