Low-cost, self-made CAD/CAM-guiding system for mandibular reconstruction

Facial symmetry, as well as function, remains the big challenge for surgeons who attempt mandibular reconstruction. Nowadays several studies recommend the use of computer aided surgery (CAS) and CAD/CAM technology to guide mandibular segmental osteotomies and reconstruction using free fibula flap. Although these systems have radically changed the way of doing mandibular reconstructive surgery, they are expensive and require extended periods of time for prototypation. This may be an important limitation in case of malignant neoplasms which require short-term treatment. The aim of our study is to investigate the reliability and efficiency of a protocol to obtain cutting guides produced in a homemade way. This study includes four consecutive patients who underwent a segmental mandibulectomy and fibula osteo-cutaneous free flap reconstruction for oral squamous cell carcinoma between January and September 2016. The CAD/CAM system algorithm proposed was based on the use of free open source software for digital planning and 3D layer plastic deposition printer. A cost of about 3 Euro for each case was estimated. An average mean distance between 3D preoperative and postoperative mesh points of 1.631 mm and a standard deviation of 5.496 mm has been demonstrated by 3D volume overlay analysis. Overlapping results with much shorter prototyping time was required with the in-house procedure described as compared to the available commercial system. In conclusion, we expect that this technique will reduce operative time and cost however further study and large series are needed to confirm our results and better define the applicability in everyday surgical practice. Similarly, academic institutions should produce quality content to guide TMJD patients towards a correct diagnostic-therapeutic process.

Specific local nasal immunotherapy: Single center experience on 324 patients

Summary: This retrospective study evaluated the efficacy and safety of specific local nasal immunotherapy (LNIT) in patients with allergic rhinitis. The results showed significant clinical efficacy of LNIT in patients allergic to mites and pollens, but no difference in mucociliary clearance time. In conclusion, specific LNIT is a valid alternative to subcutaneous and sublingual administration, providing effective and safe treatment with fewer systemic reactions.

Minimally Invasive Harvesting Technique for Costal Cartilage Graft: Donor Site, Morbidity and Aesthetic Outcomes

Summary: This study describes a new technique for harvesting cartilage grafts in reconstructive surgery with a small incision size of less than 1.5 cm. The technique was successfully applied to 36 patients without major complications such as infections or chest wall deformities. The Vancouver Scar Scale was used to evaluate postoperative scarring, and the results showed minimal scarring at both 1 week and 6 months after the procedure. This minimally invasive method provides a valid and effective approach for cartilage grafts.

Summary: This study aimed to improve the effectiveness of pre-surgical diagnosis by using a machine learning tool to analyze inflammatory biomarkers and radiomic metrics extracted from MRI images to differentiate between benign and malignant salivary gland tumors. The results showed that both inflammatory biomarkers and radiomic features were capable of supporting a differential diagnosis between different types of tumors.

To address the symptoms, we opted for an external dacryocystorhinostomy to remove scar tissue and the remaining portion of the lacrimal sac, combined with endoscopic sinus surgery (ESS).

The Use of Tactile Sensors in Oral and Maxillofacial Surgery: An Overview

Summary: This overview aimed to characterize the type, development, and use of haptic technologies for maxillofacial surgical purposes. Relevant manuscripts were searched electronically until 1 November 2022, and a total of 31 articles were included. The application of existing technologies is mainly focused on digital simulation purposes, and the integration into Computer Assisted Surgery (CAS) is far from being widely actuated. Virtual reality showed interesting results and has the potential to revolutionize teaching and learning.

Impact of Sarcopenia on Survival of Patients With Malignant Salivary Glands Tumors

Summary: The aim of this study was to evaluate the prognostic performance of preoperative sarcopenia in stratifying patients with malignant salivary gland tumors at high risk of disease progression. The study found that sarcopenia was statistically related to age, tumor size, lymph node metastases, and tumor stage. Kaplan-Meier survival curves showed that 47.3% of sarcopenic patients died before their final follow-up.
