Summary: The study found that surgical experts have high consensus on the suitable size of hiatal hernia for MSA implantation and the tension on hiatal repair, but lack consensus on other aspects of mediastinal dissection and MSA implantation. There is a need for further standardization of the surgical procedure to improve teaching effectiveness and ensure efficacy and safety outcomes when the device is used in the community.

Summary: There is no clinical credence to the manufacturer's recommendation for the use of fluoroscopy and limitation to 15 mm when dilating a patient for dysphagia after MSA implantation. Use of a larger size dilator was not associated with perforation or device erosion, but also did not reduce the need for repeat dilation.

Summary: Roemheld syndrome, also known as gastrocardiac syndrome, is a condition that involves gastrointestinal and cardiovascular symptoms. The pathophysiology of this syndrome is still unknown. A case study of a patient with Roemheld syndrome and hiatal hernia was presented, and their symptoms were successfully treated with robotic assisted hernia repair, esophagogastroduodenoscopy (EGD), and LINX magnetic sphincter augmentation.

Summary: The study demonstrated that there were no significant differences in the outcomes of MSA treatment among normal weight, overweight, and obese patients. All patient groups experienced significant improvements after MSA, including reduced acid suppressive medication use and improved GERD symptoms.

Summary: This retrospective study analyzed the data of patients who underwent magnetic sphincter augmentation (MSA) procedure and reviewed the cases of MSA device removal. The majority of removals were due to dysphagia, heartburn, and epigastric pain, and erosion occurred in 2.5% of patients. Changes in sizing technique and crura repair were associated with a decrease in the incidence of device removal.

Summary: This article reviews the published literature to define the ideal patient selection for Magnetic Sphincter Augmentation (MSA) in Anti-reflux Surgery (ARS). The authors found a lack of data to support the use of MSA in patients with advanced GERD, obesity, or a history of bariatric surgery. They concluded that younger patients with mild/moderate GERD and normal motility on manometry have better outcomes after MSA. Patients with allergies to certain materials should avoid MSA. The ideal patient selection for MSA may expand as more data emerges.

Summary: The study on magnetic sphincter augmentation revealed that smaller device sizes are more likely to be removed or eroded. The 7-year cumulative risk of removal was 4.81% and erosion was 0.28%. The primary reasons for removal were dysphagia/odynophagia, persistent GERD, and unknown/other causes.

Summary: Laparoscopic placement of the LINX Magnetic Sphincter Augmentation (MSA) device, either with or without formal hiatal hernia repair, is an effective alternative to fundoplication in appropriate patients. Formal hiatal hernia repair during MSA implantation provides better control of reflux with reduced dysphagia and postoperative hernia risk compared to no hiatal hernia repair. Systematic crural repair should be performed during MSA implantation regardless of the presence or size of hiatal hernia.

Summary: Preoperative smoking, postoperative smoking, or changes in smoking status were not associated with postoperative weight loss among bariatric patients. Smoking cessation was linked to increased patient satisfaction and improvement in quality of life compared to continued smokers. Both smoking cessation and postoperative weight loss independently predicted higher patient satisfaction.

Summary: This study aimed to investigate the rate of NSAID use and associated complications in patients who underwent sleeve gastrectomy (SG). The findings revealed that a significant proportion of patients continued to use NSAIDs after SG, with some using them frequently. These results highlight the potential risks of NSAID use in SG patients, suggesting a need for caution among healthcare providers.

Summary: This study evaluated the preoperative weight loss strategy for obese patients undergoing abdominal wall hernia repair at our institution, and assessed the outcomes of patients who required emergent repair during the deferral period. The results showed that preoperative weight loss was unsuccessful in most obese patients, and patients who required emergent repair had worse outcomes. This suggests that our institution's BMI threshold needs to be reconsidered.

Summary: The study found that preoperative weight loss is not predictive of postoperative weight loss success after bariatric surgery, although greater preoperative weight loss may be associated with a slight decrease in length of hospital stay. However, preoperative weight loss does not affect operative time, complication rates, ICU admissions, or intraoperative complications.

Summary: The need for structured training in robotic technology for thoracic surgery is increasing, but there is currently no standard curriculum for residents. Multiple training programs are in development, incorporating simulation technology and gradual increase in responsibilities within operations.
