Summary: When radiotherapy is used in the treatment of head and neck cancers, the brain may also receive radiation, resulting in potential neurocognitive changes and affecting quality of life. However, there has been limited research on this topic. Most studies show that neurocognitive outcomes are worse in patients who received radiotherapy compared to the control group, particularly in memory and language domains. Magnetic Resonance Imaging may be useful in detecting early structural and functional changes that can indicate future neurocognitive changes. Neurocognitive impairment is associated with poorer quality of life. More high-quality and large-scale studies are needed to draw definitive conclusions.

Summary: Oral-functioning impairment can negatively affect the quality of life of head and neck cancer patients after radiotherapy. This scoping review proposes a definition for oral functioning in these patients and identifies questionnaires for assessing patient-reported oral functioning. The common denominators for oral functioning were found to be chewing, speaking, and swallowing. Based on the included studies, the VHNSS 2.0 questionnaire is recommended for assessing oral functioning in these patients.

Summary: The study found that attending a pre-treatment psychoeducational intervention was associated with higher survival rates and lower recurrence rates in patients receiving radiotherapy for head and neck cancers. Multivariable analysis showed that non-attendees had unfavorable survival and recurrence outcomes. Furthermore, attendees had a lower frequency of treatment-related issues compared to non-attendees.

Summary: This qualitative study explores the experiences of head and neck cancer (HNC) patients in communicating with health professionals. The loss of speech capacity is found to be distressing and traumatic for survivors of HNC. Tensions arise in the communication between patients and health professionals, including abrupt, hurried communication versus slower, more mindful communication. Disparities in health literacy levels and a lack of empathy are also observed.

Summary: This study assessed the early impact of radiotherapy on health-related quality of life (HRQoL) outcomes, psychological distress, nutritional status, and overall performance of patients with head and neck cancer (HNC) in Poland. The results revealed negative effects of radiotherapy on functioning and symptom scales, as well as changes in nutritional status and depressive symptom severity. These findings highlight the importance of considering the impact of radiotherapy on HNC patients' well-being and the need for supportive interventions.

Summary: This study examines the resource requirements and impact on quality of life of parotid sparing adaptive radiotherapy (PSART). The study found that the time spent on contouring and treatment planning nearly doubled when implementing PSART. Although patients receiving PSART had poorer quality of life scores at 3 months post-treatment, xerostomia-related quality of life recovered to near baseline scores by 9 months.

Summary: This article reviews the techniques of organ at risk (OAR) sparing in MR-guided adaptive radiotherapy and discusses future developments to improve treatment outcomes. Increasing the spatial conformity of dose distributions and using biological information for personalized treatment decisions are important steps in reducing normal tissue toxicities. MR-linac devices provide the potential for advancements in radiotherapy treatment by incorporating daily MR imaging during treatment delivery. The incorporation of imaging and clinical biomarkers into adaptive techniques shows promise in improving the therapeutic ratio.

Summary: This study reports the benefits of autologous fat injection for correcting deformities and improving functionality in 40 patients treated for head and neck cancer. Esthetic improvement was achieved in 77.5% of patients and functional improvement in 89.2%. The study confirms the effectiveness of autologous fat grafting as a treatment for sequelae of head and neck cancer treatment.

Summary: This study demonstrates the use of Data Envelopment Analysis as a real-time decision support tool to assess the quality of radiotherapy plans for head and neck cancer patients. By comparing new plans to a library of previous clinically approved plans, the method provides benchmarking to evaluate the relative quality and improvement potential of each plan. The integration of DEA-based quality feedback into the RT planning process shows significant contributions to planning quality.

Summary: The study found that midtreatment blood volume increases derived from DCE-MRI serve as an early predictor of dysphagia, with BV increases in the PCM correlating with various parameters of swallowing difficulties. Further investigation of these promising imaging markers to assess individual patient sensitivity to treatment and subsequent risk of toxicities is crucial for improving the personalization of RT planning.

Summary: The study compared the clinical and imaging responses in bioselected patients with larynx cancer to develop predictive models for surgeon-assessed response, laryngectomy-free survival, and overall survival. The results showed that imaging offers a non-invasive opportunity to evaluate response to induction chemotherapy and further evaluation is needed to optimize approaches to bioselection. Additionally, clinical trials utilizing imaging to predict outcomes, including laryngectomy-free survival, are warranted.
