Methods for segmentation of rheumatoid arthritis bone erosions in high-resolution peripheral quantitative computed tomography (HR-pQCT)

Objective: The comparison between different techniques to quantify the 3-dimensional size of inflammatory bone erosions in rheumatoid arthritis(RA) patients. Methods: Anti-cyclic citrullinated peptide antibody(ACPA) positive RA patients received high-resolution peripheral quantitative computed tomography (HR-pQCT) scans of the metacarpophalangeal joints (MCP). Erosions were measured by three different segmentation techniques: (1) manual method with calculation by half-ellipsoid formula, (2) semi-automated modified Evaluation Script for Erosions (mESE), and (3) semi-automated Medical Image Analysis Framework (MIAF) software. Bland & Altman plots were used to describe agreement between methods. Furthermore, shape of erosions was classified as regular or irregular and then compared to the sphericity obtained by MIAF. Results: A total of 76 erosions from 65 RA patients (46 females/19 males), median age 57 years, median disease duration 6.1 years and median disease activity score 28 of 2.8 units were analyzed. While mESE and MIAF showed good agreement in the measurement of erosion size, the manual method with calculation by half-ellipsoid formula underestimated erosions size, particularly with larger erosions. Accurate segmentation is particularly important in larger erosions, which are irregularly shaped. In all three segmentation techniques irregular erosions were larger in size than regular erosions (MIAF: 19.7 vs. 3.4 mm(3); mESE: 15.5 vs. 2.3 mm(3); manual = 7.2 vs. 1.52 mm(3); all p < 0.001). In accordance, sphericity of erosions measured by MIAF significantly decreased with their size (p < 0.001). Conclusion: MIAF and mESE allow segmentation of inflammatory bone erosions in RA patients with excellent inter reader reliability. They allow calculating erosion volume independent of erosion shape and therefore provide an attractive tool to quantify structural damage in individual joints of RA patients. (C) 2018 Elsevier Inc. Summary: This article introduces the design, methods, development and deployment of the international registry promoted by the AutoInflammatory Disease Alliance (AIDA) Network for defining and assessing patients with immune-mediated scleritis. The registry collects real-world data from patients worldwide and allows standardized data collection while ensuring data security and anonymity. It also communicates with similar registries to promote research collaboration. The registry has already involved 99 centers from 20 countries and has shown significant progress in terms of participant numbers and data collection.

Summary: This study evaluated the effect of discontinuing methotrexate (MTX) for 2 weeks after each dose of the Sinovac-CoronaVac vaccine on immunogenicity and safety in patients with rheumatoid arthritis (RA). The results showed that the group that discontinued MTX had a higher rate of seroconversion and higher antibody titers compared to the group that continued MTX. However, there was an increased risk of flare after the second MTX withdrawal.

Summary: This study aimed to determine the immunogenicity of the third dose of CoronaVac vaccine in patients with autoimmune rheumatic diseases (ARD) and identify factors associated with impaired response. The results showed a significant increase in antibody positivity rates after the third dose of the vaccine in ARD patients. However, certain medications may reduce immune response. These findings are important for guiding vaccination strategies in patients with ARD.

Association between osteoarthritis and atherosclerosis: A systematic review and meta-analysis

Summary: In this study, quantitative MRI techniques were used to assess the relationship between tissue changes in intervertebral discs and clinical outcomes following kyphoplasty for vertebral fractures. The results showed that lower T2 values in adjacent discs were associated with poorer physical outcomes, indicating potential for using quantitative T2 mapping as a tool for evaluating tissue degeneration and its impact on patient quality of life post-kyphoplasty.

Dispensing anti-osteoporotic drugs changed during the COVID-19 pandemic

Summary: The study found that during the COVID-19 pandemic, the average monthly prescriptions for oral bisphosphonates and SERMs decreased, while those for intravenous bisphosphonates and teriparatide increased. Although prescriptions for DMAB decreased during the first lockdown, there was a rebound in the following months.

An AI-Powered Clinical Decision Support System to Predict Flares in Rheumatoid Arthritis: A Pilot Study

Summary: This study aimed to evaluate the accuracy, effectiveness, usability, and acceptance of the Rheuma Care Manager (RCM), a clinical decision support system (CDSS) with an artificial intelligence-powered flare risk prediction tool for managing rheumatoid arthritis (RA). The RCM showed high sensitivity, specificity, and an excellent area under the receiver operating characteristic curve for flare risk prediction. The use of the RCM increased decision confidence, reduced deviations between physicians, and resulted in more treatment reductions. The usability and acceptance of the RCM were rated positively.

Summary: This study aims to compare the incidence of subclinical atherosclerosis and cardiovascular disease (CVD) in Chronic Inflammatory Disease (CID) participants and non-diseased individuals. Using a case-cohort design and testing specific biomarkers for CID, subclinical atherosclerosis, and CVD morbimortality, the study found differences between the high-risk group and the comparison group in terms of age, gender, income, anthropometric measures, and related diseases and inflammation markers levels.

Systematic calibration reduces sources of variability for the preliminary OMERACT juvenile idiopathic arthritis MRI- sacroiliac joint score (OMERACT JAMRIS-SIJ)

Summary: This study aimed to investigate whether systematic calibration improves the scoring proficiency of JAMRIS-SIJ and whether contrast-enhancement enhances its performance. The results showed that calibrated readers achieved greater reliability in scoring specific inflammatory and structural lesions. Sensitivity and reliability for scoring inflammatory lesions were higher on fluid-sensitive sequences compared to contrast-enhanced sequences. Therefore, systematic calibration should be implemented before using JAMRIS-SIJ in clinical trials, and it is unlikely that contrast-enhanced MRI will improve the performance of this method.

Summary: This study evaluated whether the initial use of glucocorticoid (GC) bridging in RA patients leads to a higher probability of long-term GC and bDMARD use. The results showed that patients who initially started GC had a higher risk of later GC use, but the risk of bDMARD use was not significantly increased.

Cardiovascular safety of zoledronic acid in the treatment of primary osteoporosis: A meta-analysis and systematic review

Summary: This study assessed the cardiovascular safety of zoledronic acid in the treatment of primary osteoporosis. The results showed that in women with primary osteoporosis, zoledronic acid may increase the risk of atrial fibrillation and arrhythmias, but the cardiovascular risk in men with osteoporosis is uncertain.
