Strategies to optimize MEDLINE and EMBASE search strategies for anesthesiology systematic reviews. An experimental study

BACKGROUND: A high-quality electronic search is essential for ensuring accuracy and comprehensiveness among the records retrieved when conducting systematic reviews. Therefore, we aimed to identify the most efficient method for searching in both MEDLINE (through PubMed) and EMBASE, covering search terms with variant spellings, direct and indirect orders, and associations with MeSH and EMTREE terms (or lack thereof). DESIGN AND SETTING: Experimental study. UNESP, Brazil. METHODS: We selected and analyzed 37 search strategies that had specifically been developed for the field of anesthesiology. These search strategies were adapted in order to cover all potentially relevant search terms, with regard to variant spellings and direct and indirect orders, in the most efficient manner. RESULTS: When the strategies included variant spellings and direct and indirect orders, these adapted versions of the search strategies selected retrieved the same number of search results in MEDLINE (mean of 61.3%) and a higher number in EMBASE (mean of 63.9%) in the sample analyzed. The numbers of results retrieved through the searches analyzed here were not identical with and without associated use of MeSH and EMTREE terms. However, association of these terms from both controlled vocabularies retrieved a larger number of records than did the use of either one of them. CONCLUSIONS: In view of these results, we recommend that the search terms used should include both preferred and non-preferred terms (i.e. variant spellings and direct/indirect order of the same term) and associated MeSH and EMTREE terms, in order to develop highly-sensitive search strategies for systematic reviews.

Early mobilization in acute stroke phase: a systematic review

Summary: This study aimed to investigate the effectiveness and safety of early mobilization in the acute stroke phase. The results showed that early mobilization can improve functional capacity after stroke, and the optimal time to start early mobilization should be determined based on hemodynamic stability and safety criteria.

Minimal fresh gas flow sevoflurane anesthesia and postoperative acute kidney injury in on-pump cardiac surgery: a randomized comparative trial

Effects of non-invasive brain stimulation (NIBS) on vestibulopathy disorders: a systematic review

Summary: This review analyzed the effects of non-invasive brain stimulation (NIBS) on patients with chronic vestibular dysfunction (VD). The results showed that transcranial direct current stimulation (tDCS) combined with vestibular rehabilitation therapy (VRT) improved dizziness and balance in VD patients, with high certainty of evidence and low risk of bias.

Antimicrobial action of four herbal plants over mixed-species biofilms of Candida albicans with four different microorganisms

Summary: This study evaluated the antimicrobial effect of four herbal plant extracts on mixed-species biofilms composed of Candida albicans and other pathogenic bacteria. The results showed that these extracts had inhibitory effects on the mixed-species biofilms, and some extracts even completely eliminated the biofilms. Therefore, the glycolic extracts of P. paniculata, H. virginiana, S. barbatiman, and G. sylvestre may be considered as potential alternatives for endodontic therapy.

Evaluation of genetic instability, oxidative stress, and metabolism-related gene polymorphisms in workers exposed to waste anesthetic gases

Summary: Professionals in operating rooms are exposed to waste anesthetic gases daily, which may lead to genetic damage and oxidative stress. This study found that exposed professionals had higher rates of genetic instability and oxidative stress, especially with longer exposure durations. Certain gene polymorphisms may influence the extent of oxidative stress. Minimizing gas exposure is necessary to reduce the impact on healthcare workers.

Modulation of gene expression and inflammation but not DNA damage after sevoflurane anesthesia

Summary: This study investigated the expression of hOGG1, TP53, and IL-6 genes in leukocytes of surgical patients before, during, and after sevoflurane anesthesia. The results showed that TP53 and hOGG1 expression was decreased after anesthesia, while IL-6 expression and DNA damage were not affected. Systemically, mild neutrophilia and increased IL-6 levels were observed after anesthesia. These findings suggest that sevoflurane anesthesia can downregulate gene expression and induce an inflammatory response, but does not cause DNA damage in patients undergoing minor elective surgery without comorbidities.

Antifungal Action of Herbal Plants' Glycolic Extracts against Candida Species

Summary: This study evaluated the isolated and synergistic antifungal effect of glycolic extracts from Rosa centifolia L., Curcuma longa L., Rosmarinus officinalis L., and Punica granatum L. against Candida spp. The findings showed that these plant extracts have antifungal and antibiofilm action against Candida spp. at different concentrations and times of action.

Global anaesthesia-related cardiac arrest rates in children: a systematic review and meta-analysis

Summary: The study found that postoperative cardiac arrest rates in children were negatively correlated with the country's HDI and were not correlated with time. From 2001 to 2022, ARCARs were higher in low-HDI countries compared to high-HDI countries. Children under 1 year old in both high and low HDI countries had higher ARCARs compared to those older than 1 year old during the same period.

Evolution of Myocardial Hypertrophy Associated With Pregnancy in Hypertensive Women Six Months Postpartum

Summary: Systemic arterial hypertension (SAH) is a major risk factor for cardiovascular disease. Pregnant women with hypertension experience changes in hemodynamics, which may lead to greater myocardial hypertrophy. This study aims to evaluate left ventricular hypertrophy (LVH) in hypertensive women during the third trimester of pregnancy and 6 months postpartum and identify clinical variables associated with an elevated risk of LVH. The findings show that all women had LVH at the end of gestation, and 79% maintained hypertrophy at 6 months postpartum. An increase in systolic blood pressure (SBP) at 6 months postpartum was associated with the maintenance of hypertrophy.

Sexual dimorphism in the murine model of extraparenchymal neurocysticercosis

Summary: In this study, we investigated the sexual dimorphism in a rat model of extraparenchymal neurocysticercosis caused by T. crassiceps. The results showed that female rats exhibited stronger inflammatory responses and more intense hydrocephalus compared to males. These findings reveal the sexual dimorphism in T. crassiceps extraparenchymal neurocysticercosis.

Neurofibromatosis type 1, fibromuscular dysplasia, and ischemic stroke: an association lost in time? A case report

Summary: We report a rare case of Neurofibromatosis Type 1 (NF1) causing ischemic stroke (IS) in a young patient, which was caused by fibromuscular dysplasia. Angiographic study showed occlusion in the right internal carotid artery (ICA) just after its origin and the left ICA just before the intracranial portion, and brain magnetic resonance imaging revealed an area of brain infarction in the right frontoparietal region. Despite the concomitant neuroimaging findings, the rare association makes it difficult to establish the contribution of each disease to the outcome, the optimal treatment to implement, or the prognosis.

Summary: Evaluation of the possible toxic effects of occupational exposure to anesthetics is limited. This cohort study assessed oxidative stress, DNA damage, and gene expression in physicians occupationally exposed to modern anesthetics during their residency period. The results showed induction of oxidative stress, gene expression modulation, and genotoxicity in physicians during their specialization period.
