Summary: This review examines the relationship between adenomyosis and infertility, finding that adenomyosis patients have lower clinical pregnancy rates, lower live birth rates, higher miscarriage rates, and higher odds of adverse obstetric outcomes. Treatment appears to improve fertility outcomes.

Summary: This review synthesized systematic reviews and meta-analyses published in the last 5 years to explore the association between endometriosis and adenomyosis with reproductive and obstetric outcomes. The findings suggest that women with endometriosis and adenomyosis have reduced pregnancy and live birth rates, increased miscarriage rates, and higher risks of various adverse obstetric outcomes. Women with deep infiltrating endometriosis and severe adenomyosis should be considered at high obstetric risk and referred to tertiary care centers for safe management during pregnancy and delivery. It is uncertain whether the same applies to pregnant women with minimal endometriosis and adenomyosis forms. Emerging evidence suggests that some adverse outcomes observed in women with endometriosis are associated with coexisting adenomyosis.

Summary: Adenomyosis is associated with reduced odds of clinical pregnancy and increased risk of miscarriage. It also has strong correlations with pre-eclampsia, small for gestational age infants, and preterm deliveries. Deep infiltrating endometriosis and adenomyosis may lead to particularly adverse obstetric outcomes, especially in cases of natural conception.

Summary: The management of deep endometriosis requires careful preoperative assessment to provide accurate information about the extent of the disease and patient's objectives. Imaging techniques such as ultrasound and MRI, along with clinical evaluation, play a crucial role in detecting lesions not visible during initial laparoscopy. In this case study, a young woman underwent laparoscopic excision of endometriosis which was found to be safe and effective in improving fertility and alleviating pain symptoms.

Summary: This study investigates the pregnancy outcomes of women with infertility with both AM and EM after laparoscopic surgery. It found that age, endometrioma size, and uterus size were significantly associated with successful delivery. The study also highlights the potential adverse effects of endometriosis and adenomyosis on pregnancy outcomes, which may be related to the size of the uterus and the endometriosis fertility index (EFI).

Summary: This study evaluated the effects of different ovarian stimulation protocols on IVF/ICSI outcomes in infertile women with adenomyosis. The results showed that in fresh ET cycles, the short protocol had significantly lower implantation rate and clinical pregnancy rate compared to ultra-long and long protocols. In FET cycles, the origin of embryos did not affect pregnancy outcomes.

Summary: This study evaluated the improvement of symptoms and pregnancy outcomes in infertile patients with various types of adenomyosis treated with high-intensity focused ultrasound (HIFU). The results showed that HIFU significantly improved dysmenorrhea and heavy menstruation in all types of adenomyosis patients, with no significant impact on anti-Mullerian hormone levels. Among the participants, 38.7% successfully became pregnant after HIFU treatment, with the highest pregnancy rate observed in patients with internal adenomyosis.

Summary: The junctional zone is a significant structure for maintaining uterine fertility, and its changes are closely linked to infertility and adenomyosis (ADS). ADS is a crucial factor causing female infertility. The challenge for clinicians is to treat ADS-related infertility and preserve reproductive function, especially for young patients who desire fertility preservation and pregnancy.

Summary: This study used the MUSA criteria to evaluate the impact of adenomyosis on the live birth rate after donor egg embryo transfer. The results indicated that while the ultrasonographic features of adenomyosis may not affect reproductive outcomes, the location and size of focal lesions may be important and require further investigation.
