Cerebral venous thrombosis: state of the art diagnosis and management

This review article aims to discuss the pathophysiology, clinical presentation, and neuroimaging of cerebral venous thrombosis (CVT). Different approaches for diagnosis of CVT, including CT/CTV, MRI/MRV, and US will be discussed and the reader will become acquainted with imaging findings as well as limitations of each modality. Lastly, this exhibit will review the standard of care for CVT treatment and emerging endovascular options. A literature search using PubMed and the MEDLINE subengine was completed using the terms cerebral venous thrombosis, stroke, and imaging. Studies reporting on the workup, imaging characteristics, clinical history, and management of patients with CVT were included. The presentation of CVT is often non-specific and requires a high index of clinical suspicion. Signs of CVT on NECT can be divided into indirect signs (edema, parenchymal hemorrhage, subarachnoid hemorrhage, and rarely subdural hematomas) and less commonly direct signs (visualization of dense thrombus within a vein or within the cerebral venous sinuses). Confirmation is performed with CTV, directly demonstrating the thrombus as a filling defect, or MRI/MRV, which also provides superior characterization of parenchymal abnormalities. General pitfalls and anatomic variants will also be discussed. Lastly, endovascular management options including thrombolysis and mechanical thrombectomy are discussed. CVT is a relatively uncommon phenomenon and frequently overlooked at initial presentation. Familiarity with imaging features and diagnostic work-up of CVT will help in providing timely diagnosis and therapy which can significantly improve outcome and diminish the risk of acute and long-term complications, optimizing patient care.

Summary: The study investigated the impact of microvascular tissue reperfusion and its influencing factors in acute ischemic stroke patients. The findings revealed that microvascular tissue reperfusion was strongly associated with good functional outcomes after successful reperfusion, and higher mTICI grade and lower perioperative glucose level may predict microvascular tissue reperfusion.

Summary: This study aimed to investigate the impact of early venous filling (EVF) after mechanical thrombectomy (MT). The results showed that the EVF group had a higher rate of intracranial hemorrhage, symptomatic intracranial hemorrhage, and malignant cerebral edema. Additionally, the cortical veins subgroup of EVF had a higher mortality rate.

Summary: Headache and pulsatile tinnitus were the patient's symptoms. Treatment of the occlusive arteriovenous fistula with interventional radiology was successful. This case demonstrates that minimally invasive surgery is an effective alternative when conventional methods cannot be performed.

Summary: This study investigated the potential threshold values in the functional assessment of intracranial atherosclerotic stenosis (ICAS) by studying the relation between pressure-derived indexes and perfusion parameters. The results showed that cut-off values of translesional pressure gradients (Pd/Pa=0.81 or Pa-Pd=8 mm Hg) were preliminary established in a highly selected subgroup with ICAS, which may facilitate clinical decision-making in the management of ICAS.

Clinical and subclinical microemboli following neuroangiography in children

Summary: This retrospective study aimed to assess the frequency, imaging appearances, and risk factors of brain microemboli following pediatric neuroangiography using early diffusion-weighted MRI imaging. The study found that the occurrence of cerebral microemboli is uncommon in children after uncomplicated neuroangiography. However, the incidence approaches that reported in adults when there is underlying vasculopathy and the transradial technique is used. Additionally, there is a significant association between the number of angiographic runs and microemboli.

Summary: This study compares the rates of symptomatic strokes between direct and indirect revascularization surgeries for moyamoya and finds that both modalities have comparable rates of overall strokes. Therefore, the choice of revascularization modality can be made based on the patient's risk assessment.
