Risk Factors and Clinical Impact of Delayed Cerebral Ischemia after Aneurysmal Subarachnoid Hemorrhage: Analysis from the China National Stroke Registry

Background: Delayed cerebral ischemia (DCI) after aneurysmal subarachnoid hemorrhage (aSAH) is a significant cause of morbidity and mortality. While it is important to detect early signs of DCI, patients with high risk of DCI are difficult to identify, and the potential risk factors are uncertain. This study aimed to identify independent risk factors of DCI and clarify their clinical impact on outcome and the disease course. Methods: For this nationwide, multicenter, prospective clinical study involving consecutive patients with aSAH recruited from the China National Stroke Registry, demographic, clinical, radiological, and laboratorial data during hospitalisation, in-hospital complications, functional outcomes, and mortality at 3, 6, and 12 months were recorded. Using univariate and multivariate logistic regression to determine risk factors associated with the development of DCI, the contribution of DCI to short- and long-term outcomes was evaluated. Results: Of all 504 patients with aSAH, 155 developed DCI. Multivariate analysis revealed that being female, a history of diabetes mellitus, a Hunt and Hess grade of 4-5, and a World Federation of Neurosurgical Societies grade of IV-V were independent risk factors of DCI. DCI was associated with high in-hospital complications, and with a high utilization rate of supporting interventions. DCI also contributed to poorer functional outcome and higher mortality at discharge and after 3, 6, and 12 months. Conclusions: Female sex, a history of diabetes mellitus, and poor clinical grade are independent early risk factors for the development of DCI and can contribute to a better identification of patients at a high risk for DCI. Summary: This study reviewed and analyzed the evidence on the associations between pretreatment collateral status and outcomes after endovascular treatment (EVT) for acute basilar artery occlusion (BAO). The results showed that good pretreatment collateral status was associated with favorable 90-day functional outcome, successful recanalization, and reduced 90-day mortality.

The combination of heart rate variability and ABCD2 score portends adverse outcomes after minor stroke or transient ischemic attack

Summary: This study investigated the relationships between heart rate variability (HRV) combined with the ABCD2 score and adverse outcomes after acute minor stroke (MS) or transient ischemic attack (TIA). The combination of HRV and ABCD2 score efficiently stratified the risk of 1-year recurrent stroke after MS/TIA, and lower SDNN was independently related to recurrent stroke in patients with MS/TIA.

Circular RNA circ_0008896 contributes to oxidized low-density lipoprotein-induced aortic endothelial cell injury via targeting miR-188-3p/NOD2 axis

Summary: This study aimed to investigate the role and mechanism of circ_0008896 in Atherosclerosis (AS) using oxidized low-density lipoprotein (ox-LDL)-induced human aortic endothelial cell (HAECs). The results showed that circ_0008896 was upregulated in AS patients and ox-LDL-stimulated HAECs. Functionally, circ_0008896 knockdown reversed ox-LDL-induced inflammatory response, oxidative stress, apoptosis, and growth arrest in HAECs. Mechanistically, circ_0008896 functioned as a sponge for miR-188-3p to relieve the repression of miR-188-3p on its target NOD2.

Summary: This study investigated the association between CYP2B6 polymorphisms and the efficacy of clopidogrel in patients with minor stroke or transient ischemic attack. The results showed no significant difference in the efficacy of aspirin plus clopidogrel compared with aspirin alone, regardless of the presence of CYP2B6 polymorphisms. Therefore, both carriers and noncarriers of CYP2B6 polymorphisms may benefit from aspirin plus clopidogrel treatment for secondary stroke prevention.

Summary: The imaging modality used for selecting patients for endovascular thrombectomy (EVT) in the late window varies among clinicians with different specialty training. In an international survey, interventionists were more likely to proceed directly to EVT and prefer less advanced imaging techniques compared to non-interventionists. When faced with uncertainty, non-interventionists were more likely to follow clinical guidelines, while interventionists relied more on their assessment of evidence.

Lipid management in ischaemic stroke or transient ischaemic attack in China: result from China National Stroke Registry III

Summary: This study aimed to assess the management of LDL-C and goal achievement in patients with ischemic stroke or TIA, as well as the association between baseline LDL-C level, LLT, and stroke recurrence. A post hoc analysis of the CNSR-III showed a mild increase in LDL-C goal achievement rate among stroke and TIA patients in mainland China. Lower baseline LDL-C level was significantly associated with a decreased short- and long-term risk of ischemic stroke.

Summary: The study aimed to investigate the potential vascular risk factors for deep medullary veins (DMVs) and their associations with cerebral small vessel disease (CSVD). The results showed that DMVs were associated with age, sex, body mass index, blood pressure, and other vascular factors. They were also related to the presence and total burden of CSVD, as well as specific imaging markers. However, these associations disappeared after adjusting for potential confounders.

Automatic segmentation of the female pelvic floor muscles on MRI for pelvic floor function assessment
