Clopidogrel significantly lowers the development of atherosclerosis in ApoE-deficient mice in vivo

The anti-platelet drug clopidogrel has been shown to modulate adhesion molecule and cytokine expression, both playing an important role in the pathogenesis of atherosclerosis. The aim of this study was to investigate the impact of clopidogrel on the development and progression of atherosclerosis. ApoE(-/-) mice fed an atherogenic diet (cholesterol: 1 %) for 6 months received a daily dose of clopidogrel (1 mg/kg) by i.p. injection. Anti-platelet treatment was started immediately in one experimental group, and in another group clopidogrel was started 2 month after beginning of the atherogenic diet. Blood was analysed at days 30, 60 and 120 to monitor the lipid profile. After 6 months the aortic arch and brachiocephalic artery were analysed by Sudan IV staining for plaque size and by morphometry for luminal occlusion. Serum levels of various adhesion molecules were investigated by ELISA and the cellular infiltrate was analysed by immunofluorescence. After daily treatment with 1 mg/kg clopidogrel mice showed a significant reduction of atherosclerotic lesions in the thoracic aorta and within cross sections of the aortic arch [plaque formation 55.2 % (clopidogrel/start) vs. 76.5 % (untreated control) n = 8, P < 0.05]. After treatment with clopidogrel P-/E-selectin levels and cytokine levels of MCP-1 and PDGF beta were significantly reduced as compared to controls. The cellular infiltrate showed significantly reduced macrophage and T-cell infiltration in clopidogrel-treated animals. These results show that clopidogrel can effectively delay the development and progression of 'de-novo' atherosclerosis. However, once atherosclerotic lesions were already present, anti-platelet treatment alone did not result in reverse remodelling of these lesions.

TAVI may improve peri-procedural outcomes, although TAVI patients required more pacemaker implantation and showed more aortic regurgitation, while SAVR patients required more blood transfusions and had longer hospital stays.

Direct Impact of Human Platelets on the Development of Transplant Arteriosclerosis

Summary: The F8 technique with the PME21 achieved a similar EOA as the 1 size larger PME23 using NE after AE. SI and CS did not result in a larger EOA compared with the NE technique. PME21 using SI had a significantly larger leakage volume than using NE and there was no significant difference between other techniques.

Long-Term Cultivation of Human Atrial Myocardium

Summary: This study presents a method for long-term cultivation of beating human atrial myocardium. The authors obtained tissues from the right-atrial appendage of patients with sinus rhythm and cultivated trabeculae for up to 3 weeks. They analyzed the functional parameters, gene expression, and tissue microstructure, and confirmed the viability and electromechanical function of the cultivated myocardium. This method allows for studying the physiology and pathophysiology of human atrial myocardium.

Outcomes of aortic valve repair: early results from the German Aortic Valve Registry

Summary: AV repair/AV-sparing root surgery is an effective strategy for treating young patients with aortic regurgitation and/or aortic root aneurysm. This study analyzed real-world data from the German Aortic Valve Registry and found that the surgical procedure resulted in satisfactory clinical outcomes, including high survival rates and freedom from cardiac adverse events at one year. Additionally, the specific surgical technique used did not significantly impact the outcomes.

Summary: The success of LVAD as a treatment for terminal left-side heart failure is limited by complications such as pump thrombus. Treatment options include medication augmentation, thrombolysis, and pump exchange. Heart transplantation is not a practical option due to a scarcity of organ donors. Ex vivo evaluation of the Ozaki procedure in comparison with the native aortic valve and prosthetic valves

Summary: In this ex vivo study, the hydrodynamic performance and cusp kinematics of the Ozaki neocuspidized aortic valve were compared with native aortic and prosthetic valves. The results showed that the Ozaki valve had excellent hydrodynamic performance and similar cusp motion characteristics to the native aortic valve, suggesting potential beneficial clinical outcomes.

Double valve replacement in a case of Hedinger syndrome

Summary: Hedinger Syndrome, also known as carcinoid heart disease, is a rare cardiac complication of neuroendocrine tumors that affects the tricuspid and pulmonary valves. This case study describes a patient with a neuroendocrine tumor with liver metastasis who underwent surgical valve replacement for symptomatic tricuspid valve regurgitation and pulmonary valve stenosis. The surgery was successfully performed before the development of severe right ventricular failure or pulmonary hypertension in this case of carcinoid heart disease. An interdisciplinary approach and regular follow-up are recommended for such cases.
