Summary: Tricuspid regurgitation is a significant finding in patients who have undergone repair for tetralogy of Fallot, and it can have primary or secondary causes. Moderate or severe tricuspid regurgitation is associated with a higher occurrence of atrial tachyarrhythmias in these patients. The optimal timing for tricuspid valve intervention during pulmonary valve replacement in repaired tetralogy of Fallot patients remains uncertain.

Summary: The prevalence and determinants of tricuspid regurgitation (TR) in patients with repaired Tetralogy of Fallot (rTOF) remain incompletely understood. This study found that despite substantial right ventricular (RV) dilatation in rTOF patients, the prevalence of TR is surprisingly low and has weak correlation with factors such as right atrial volumes and tricuspid annular diameter. These findings question the commonly held notions regarding the pathophysiology of functional TR in these patients.

Summary: This study found that adverse events in patients with repaired tetralogy of Fallot (rToF) are closely associated with pathological remodelling patterns. These patterns include a more circular left ventricle, dilated LV base, increased RV apical sphericity, and decreased RV basal sphericity.

Summary: The study demonstrated that in patients with repaired tetralogy of Fallot, BVGFI <37, right ventricular end-systolic volume index >85 mL/m(2), atrial tachycardia, and age at repair >2.5 years were independently associated with the composite outcome. Classification and regression tree analysis revealed that BVGFI was the most discriminatory CMR parameter associated with a high risk for adverse outcomes.

Summary: This study evaluated the midterm outcomes and predictors of adverse clinical outcomes (ACO) in patients with repaired tetralogy of Fallot (rTOF) who underwent surgical or transcatheter pulmonary valve replacement (PVR). The results showed a significant reduction of right ventricular (RV) volume early after PVR, followed by further improvement of biventricular function by midterm follow-up. The midterm freedom from ACO was favorable.

Summary: 4D flow MRI was used to analyze circulation in the main pulmonary artery of repaired TOF patients, revealing a correlation between longitudinal circulation and RV dysfunction but not with PRF. Vortex formation was observed in 75% of patients, indicating a potential relationship between circulation and RV function in this population.

Summary: This study characterizes the 5-year outcomes of severe tricuspid regurgitation (TR) and identifies distinct clinical risk profiles. Patients with newly identified severe TR have a high 5-year risk for death and death or heart failure hospitalization. Partitioning patients using supervised survival tree models can help identify relevant subgroups in clinical trials and risk/benefit analysis for TR therapies.

Summary: The aim of this study was to determine the prognostic value of severe atrial functional tricuspid regurgitation (AF-TR) in patients with atrial fibrillation (AF) and its determinants. The study found that severe AF-TR is an independent predictor of all-cause mortality in AF patients, while mild/moderate AF-TR apparently has no impact on prognosis.

Summary: The study retrospectively reviewed the clinical records of TOF patients who underwent PVR between 2001 and 2012, finding that concomitant TVP can effectively improve TR and RVEDVi, but may not prevent the recurrence of TR when PV failure occurs. Concomitant TVP can effectively preserve tricuspid valve function until PV failure.

Summary: Interest in TR has grown due to progressively worse outcomes associated with increasing TR severity. The aim of the Tricuspid Valve Academic Research Consortium document is to standardize definitions and endpoints for trials addressing knowledge gaps in TR management. Another document will focus on defining trial endpoints and discussing trial design options.

Summary: The study found that children with asymptomatic carriage of rhinovirus before cardiac surgery did not have an increased risk of prolonged PICU stay. Rhinovirus-positive patients had shorter hospital stays and required less ventilatory support. Overall, virus-positive patients had shorter PICU and hospital stays, while virus-negative patients were more likely to exhibit symptoms of respiratory infection.

Summary: The study analyzed the survival and incidence of Fontan completion in patients with single-ventricle atrioventricular septal defect. The results showed that patients with moderate-severe or severe atrioventricular valve regurgitation had a worse survival, and repairing the atrioventricular valve could improve the survival rate. This study provides important information for the diagnosis and treatment of these patients.

Summary: This retrospective cohort study in a tertiary children's hospital in the Netherlands found that 24% of children developed severe acute kidney injury after arterial switch operation. Severe AKI was associated with increased morbidity, longer PICU stay, and higher mortality rates compared to children without severe AKI. Sepsis was identified as a risk factor for developing severe AKI.

Summary: This study evaluated the short-term effects of enalapril on Fontan patients with moderate-good systolic ventricular function, showing that enalapril treatment reduced SBP and levels of N-terminal pro-B-type natriuretic peptide but did not improve exercise capacity, ventricular function, or arterial stiffness. Summary: The study found that only half of the participants actually visited the information portal, despite being invited by their cardiologists. Among those who did visit, their disease-specific knowledge and mental health were significantly better. This highlights the importance of effective implementation of online evidence-based patient information portals in clinical practice.
