Stranger Months: How SARS-CoV-2, Fear of Contagion, and Lockdown Measures Impacted Attendance and Clinical Activity During February and March 2020 at an Urban Emergency Department in Milan

Stefano Franchini, Marzia Spessot, Giovanni Landoni, Cecilia Piani, Chiara Cappelletti, Federica Mariani, Simona Mauri, Maria Vittoria Taglietti, Manuela Fortunato, Federico Furlan, Barbara Guglielmi, Eleonora Setti, Davide Di Napoli, Giovanni Borghi, Federico Pascucci, George Ujlaki-Formenti, Riccardo Sannicandro, Matteo Moro, Sergio Colombo, Lorenzo Dagna, Antonella Castagna, Moreno Tresoldi, Patrizia Rovere-Querini, Alberto Ambrosio, Fabio Ciceri, Alberto Zangrillo, Michele Carlucci, Roberto Faccincani

Summary: The unprecedented wave of patients with acute respiratory failure due to COVID-19 hit emergency departments in Lombardy, causing a significant drop in ED attendance and an increase in patients needing oxygen, noninvasive ventilation, ICU transfers, and in-hospital mortality. The fear of contagion led to missed or delayed diagnoses for conditions other than COVID-19.

Summary: The study aimed to describe the incidence and predictive factors of secondary infections in COVID-19 patients. Risk factors identified for secondary infections included low lymphocyte count, baseline PaO2/FiO2 decrease, and ICU admission within the first 48 hours.

Incidence of deep venous thrombosis in COVID-19 hospitalized patients during the first peak of the Italian outbreak

Summary: This study assessed the incidence of DVT in COVID-19 patients and found that despite receiving prophylactic treatment, the incidence of DVT in hospitalized patients was 14.5%. The study also showed that a high D-dimer level at admission was associated with DVT, and that ICU patients and age were the main predictors of in-hospital mortality.

Summary: This study found that HIV-1 drug resistance has remained stable since 2011, although HIV-MDR still exists at a lower rate, emphasizing the need for continuous monitoring and accurate management of ART-experienced patients with HIV.

Viral resistance burden and APOBEC editing correlate with virological response in heavily treatment-experienced people living with multi-drug resistant HIV

Summary: The study found that heavily treatment-experienced individuals with multidrug-resistant HIV had high levels of drug resistance mutational load but less pronounced APOBEC editing. Patients who failed treatment showed higher levels of DNA resistance and mutational load compared to those who responded.

Summary: The study evaluated the impact of alpha-fetoprotein (AFP) levels on mortality in HIV-1-infected patients with hepatocellular carcinoma (HCC). The results suggest that AFP levels greater than 28.8 ng/mL, HBV coinfection, and treatment allocation are predictive markers for death in this patient population.

Summary: In a setting with high antibiotic resistance, IDC and RDTs are associated with earlier prescription of appropriate therapy in GNBSIs, without impact on mortality.

Evaluation of HIV-DNA and residual viremia levels through week 96 in HIV-infected individuals who continue a two-drug or switch to a three-drug integrase strand transfer inhibitor-based regimen

Rossana Scutari, Laura Galli, Claudia Alteri, Andrea Poli, Lorenzo Piermatteo, Alba Bigoloni, Carlo Federico Perno, Adriano Lazzarin, Francesca Ceccherini-Silberstein, Antonella Castagna, Maria Mercedes Santoro, Nicola Gianotti

Summary: This study investigated HIV-DNA and residual viremia levels over 96 weeks in virologically-suppressed HIV-1-infected individuals. The results showed a small reduction in HIV-DNA and RV levels at W96 in individuals who switched to the E/C/F/TAF arm compared to those who remained on the DTG + 1 RTI arm. However, there were no significant differences between the two arms in the changes in HIV-DNA and HIV-RNA over time.
