Summary: In comparison with open radical cystectomy, intracorporeal robot-assisted radical cystectomy was associated with lower estimated blood loss, lower blood transfusion rates, and lower major postoperative complication rates. However, it had a longer operative time and showed no significant difference in terms of other outcomes.

Summary: The cost-effectiveness of robot-assisted radical cystectomy with intracorporeal urinary diversion (iRARC) was assessed compared to open radical cystectomy (ORC) for bladder cancer patients. While iRARC reduced short-term morbidity and associated costs, the cost-effectiveness ratio exceeded thresholds used by many publicly funded health systems, although the procedure had a high probability of being cost-effective for certain patient subgroups.

Open Radical Cystectomy versus Robot-Assisted Radical Cystectomy with Intracorporeal Urinary Diversion: Early Outcomes of a Single-Center Randomized Controlled Trial

Summary: This study compared perioperative and 6-month outcomes between open radical cystectomy (ORC) and robot-assisted radical cystectomy (RARC) with totally intracorporeal urinary diversion (UD). Results showed that RARC with intracorporeal UD had a significantly lower overall perioperative transfusion rate compared to ORC. However, perioperative complications, hospital stay, and 6-month health-related quality of life were similar between the two groups. Long-term follow-up is needed to assess oncologic and functional outcomes.

Summary: The study compared long-term renal function outcomes between open radical cystectomy and robot-assisted radical cystectomy with extracorporeal or intracorporeal urinary diversion. The results showed that 64% of patients experienced renal function decline, which was influenced by tumor stage, pre-operative eGFR, and development of ureteral strictures.

Summary: This study compared the achievement of USC pentafecta and trifecta between open radical cystectomy (ORC) and robot-assisted RC (RARC) with totally intracorporeal urinary diversion (iUD) using a randomised controlled trial. The results showed no significant difference in USC pentafecta and trifecta achievement between the two groups. Additionally, the study found that achieving pentafecta and trifecta had a significant impact on global health status/health-related quality of life (HRQoL).

Summary: The study aimed to analyze the risk of uretero-enteric anastomotic stricture in patients undergoing robot-assisted radical cystectomy (RARC) or open cystectomy (ORC). The results showed that RARC had a lower risk of stricture, and there was no evidence that perioperative complications were associated with the development of a stricture.

Summary: This study retrospectively evaluated the outcomes of robot-assisted partial cystectomy (RAPC) compared with open partial cystectomy (OPC) in the treatment of localized urachal cancer (UrC). The results showed that both RAPC and OPC are feasible surgical modalities with comparable survival. However, the rate of port-site recurrences (PSR) after RAPC was higher, suggesting caution in recommending RAPC.

Robot-assisted Versus Open Radical Cystectomy in Bladder Cancer: An Economic Evaluation Alongside a Multicentre Comparative Effectiveness Study

Summary: This study aimed to assess the cost-effectiveness of robot-assisted radical cystectomy (RARC) compared to open radical cystectomy (ORC) in bladder cancer treatment. The study included 348 bladder cancer patients (ORC, n = 168; RARC, n = 180) from 19 Dutch hospitals. The results showed that RARC had no difference in terms of QALYs compared to ORC, but was more expensive. Therefore, RARC does not seem to provide value for money compared to ORC.

Summary: This study demonstrates the feasibility of blinding the surgical technique in radical cystectomy. The results indicate that patients and doctors remained blinded to the allocated treatment, but nurses did not. There were differences in blood loss and operative time between the ORC and iRARC groups, while complication rates, length of stay, and analgesic use showed no significant variation.

Summary: The Italian Radical Cystectomy Registry (RIC) is an observational prospective study that aims to understand the clinical variables and patient characteristics associated with short- and long-term outcomes among bladder cancer patients undergoing radical cystectomy. It compares the effectiveness of three different surgical techniques - open, robotic, and laparoscopic. The study found that patients undergoing robotic procedures were younger and more likely to have undergone preoperative neoadjuvant chemotherapy and BCG instillation. Hypertension was the most common comorbidity among all patients. Patients undergoing open and laparoscopic RC had a higher Charlson Comorbidities Index compared to robotic RC. Laparoscopic patients had a lower G-stage classification and open patients had a higher ASA score.

Quality of life and secondary outcomes for open versus robot-assisted radical cystectomy: a double-blinded, randomised feasibility trial

Summary: This study aimed to compare the impact of robot-assisted laparoscopy with intracorporeal urinary diversion (iRARC) and open radical cystectomy (ORC) on patients' quality of life (QoL) before and after the surgery. The results showed that the surgical technique does not appear to affect QoL significantly, and patients reported stable or improved QoL within the first 90 days postoperatively, except for sexual functioning.

Summary: This study aimed to compare short-term outcomes between robot-assisted radical cystectomy (RARC) and open radical cystectomy (ORC) for urinary bladder cancer in a large population. The results showed that the RARC group had a higher rate of readmission compared to the ORC group, but the former had lower risks of Clavien 3-5 complications and reoperations, and had more days alive and out of hospital.

Summary: In comparing robot-assisted radical cystectomy (RARC) with open radical cystectomy (ORC) at a single tertiary referral centre while controlling for stage selection bias and surgical experience, RARC demonstrated significantly better outcomes, especially with intracorporeal urinary diversion. RARC had fewer major complications, lower blood loss and transfusion rates, and decreased operating time with increasing surgeon's experience.
