Summary: Small island developing states have limited availability of cardiac surgery, but there is a pressing clinical need. Independent cardiac centers exist with acceptable outcomes and lower costs, highlighting the need to strengthen regionalization models to deliver cardiac surgical care in SIDS.

Summary: Using composite indices, this paper argues that Turkey has always been different from the EU in terms of liberal democracy and civil liberties, and recent democratic regression has widened the gap further. This, along with the large economy and population, presents a challenge to the EU's integration capacity and institutional functioning. Full membership has never been a realistic goal, and both the EU and Turkey can benefit from a strong and pragmatic partnership.

Summary: This article demonstrates that the optimal concentration for using sodium citrate as an anticoagulant catheter lock is 7%. It also highlights that the addition of methylene blue and parabens significantly enhances the antibacterial properties. Ash Access sponsored a clinical trial, which showed positive results and led to the regaining of market rights for the catheter lock.

Summary: There are both detractors and supporters of merging neuroscience with education. It is important to carefully integrate cognitive neuroscience into educational practice in a sensible and ethical manner. Neuroeducators play a crucial role in assessing evidence, determining who can use new knowledge, and dealing with unexpected consequences. This special issue aims to support the development of training programs that integrate curriculum design, classroom instruction, and developmental cognitive neurosciences.

Summary: In this study, the researchers investigated the effectiveness of infection prevention interventions within the ICU setting. They found that selective digestive decontamination (SDD) was highly effective in preventing ICU infections. However, the results differed significantly when comparing randomized controlled trials with cluster randomized trials, suggesting a possible spillover effect of SDD.

Summary: Aging is a significant risk factor for cancer, and the metabolic changes that occur during aging create a favorable environment for tumorigenesis while suppressing immune surveillance. Targeting the metabolic reprogramming that happens during aging may provide new opportunities for cancer prevention and improving the efficacy of cancer treatments.

Summary: This study evaluated the effect of P2Y12 inhibition on critically ill patients hospitalized for COVID-19. The randomized clinical trial showed that the use of P2Y12 inhibitors did not improve the number of days alive and free of cardiovascular or respiratory organ support. Therefore, routine use of P2Y12 inhibitors is not recommended in critically ill patients hospitalized for COVID-19.

Summary: Hepatitis is a complex inflammatory condition in the liver, involving interactions between liver cells, inflammatory mediators, immune cells, and platelets. Platelets, responsible for clotting, play a role in various mechanisms underlying hepatitis, contributing to inflammatory outcomes. This study introduces platelets into an existing model of hepatitis dynamics and demonstrates their potential to stimulate chaotic dynamics and chronic outcomes. The findings highlight the importance of platelets in understanding and developing therapeutic interventions for hepatitis and other inflammation-related medical conditions.

Summary: Infants with intestinal failure associated liver disease (IFALD) often face high mortality on the transplant waiting list due to the scarcity of size-matched donor organs. Bridging liver transplantation has been used to address this issue, and we report on two cases where children with irreversible intestinal failure underwent this procedure. While both patients initially recovered well, one died from sepsis after 6 months and the other died a few days post-transplant. This raises ethical concerns about the appropriate utilization of scarce donor resources in the current era of organ scarcity.

Summary: The effects of 9-D1t-PhytoP on platelet activation and vascular cells were tested, along with possible interactions with platelets and the endothelial EP3 receptor. The results showed that 9-D1t-PhytoP has prothrombotic and wound-healing properties, and mimics the migration stimulus of PGE(2) in endothelial cells. Computational analysis revealed its potential as an EP3 agonist by forming a stable complex with the EP3 receptor.

Summary: Substituted judgment is widely accepted in decision-making for incapacitated patients, except for those who never previously possessed the ability to contemplate medical decisions. This paper argues that a three-condition test should be used for never-capacitated patients and that the medical values of certain religious or cultural communities may be helpful in vindicating their interests.
