Summary: The aim of this retrospective study was to analyze the treatment of spinopelvic dissociations (SPDs) in a single center and identify ways to improve surgical and non-surgical options. The study included twenty patients who underwent surgical treatment for SPDs between 2013 and 2021. Different surgical techniques were used, and follow-up evaluations were conducted to assess outcomes. The findings suggest that decompression surgery and fracture reduction can lead to clinical improvement in post-traumatic neurological deficits, while sexual disorders may be less responsive to treatment. Open stabilization techniques, such as double iliac screw fixation, may be beneficial for restoring sagittal balance in cases of severe deformities.

This study evaluated injury characteristics, changes in quality of life, and functional outcomes in patients with spinopelvic dissociation over a 1-year period. Results showed a significant reduction in functional outcomes after 1 year, with only one-third of patients achieving pre-injury functional levels.

Summary: Our case report describes a successful management of a non-compliant patient with comorbidities such as obesity and DVT. The patient underwent fast-tracked in situ spinopelvic fixation for a pulled-out screw in a minimally displaced fracture, resulting in early mobilization and full weight-bearing outcomes. This report demonstrates the viability of accepting pulled-out screws, given considerations for the patient's health, fracture geometry, quick follow-up in situ spinopelvic fixation, early mobilization, and lower risk for complications.

Summary: A 32-year-old male suffered from trauma caused by a heavy object (tree) falling on his back. He presented with a complete perianal tear and loss of power in L3-S1, resulting in 1/5 complete loss of sensation below the level of L2. Imaging revealed spinopelvic dissociation and cauda equina syndrome. Spinopelvic fixation and fusion with rigid fixation were performed, followed by extensive physiotherapy which resulted in the patient regaining normal function. This paper concludes that timely and effective surgical intervention promotes neurological recovery following decompression.

Secondary prophylaxis focuses on non-antimicrobial options, such as fecal microbiota transplantation and newer agents like RBX2660 and bezlotoxumab. Further studies are needed to determine the optimal therapies for CDI prophylaxis.

This study aimed to review the etiology, pathophysiology, diagnosis, and treatment options for PA, as well as analyze the characteristics and clinical course of patients. The findings can guide the development of a personalized treatment strategy for patients with PA.
