Telestroke Adoption Among Community Hospitals in North Carolina: A Cross-Sectional Study

Objective: This study identifies community and hospital characteristics associated with adoption of telestroke among acute care hospitals in North Carolina (NC). Methods: Our sample included 107 hospitals located in NC. Our analytic dataset included variables from the American Hospital Association (AHA) annual survey, AHA Health IT supplement, Healthcare Cost Report Information System, and Centers for Disease Control and Prevention's WONDER online database. We supplemented our secondary sources with data on telestroke adoption and market-level variables developed for NC. We used the Consolidated Framework for Implementation Research and previous telehealth studies to guide selection of variables. We conducted a multivariate logistic regression to determine associations with telestroke adoption. Results: Proportion of discharges that are Medicare (odds ratio [OR] = 1.93, P < .04) and total operating margin (OR = 2.89, P = .00) were positively associated with telestroke adoption. Critical access hospital status was positively associated with telestroke adoption, although not at P < .05 (OR = 5.61, P = .07). Distance to the nearest hospital with a telestroke program (OR = .91, P = .01) and volume of emergency department visits (OR = .98, P < .05) were both negatively associated with telestroke adoption. Conclusions: Our study is novel in its focus on telestroke adoption and use of variables not included in previous telehealth analyses. Our findings suggest some hospitals have neither the financial resources nor the ability to pool resources for acquiring needed technology, and differences in adoption may result in geographic inequities in access to telestroke services.

Analysis of shared cognitive tasks in the application of non-invasive ventilation to patients with COPD exacerbation

Summary: Interprofessional teamwork is crucial in utilizing noninvasive ventilation (NIV) for patients with exacerbated COPD. A cognitive task analysis was conducted to identify the shared cognitive tasks in interprofessional teams. The study found that conducting thorough assessments, formulating care plans, and continuously monitoring patient status were the shared cognitive tasks. Access to up-to-date information and expertise, as well as shared understanding of care plans, played important roles in the execution of NIV treatment.

Telehealth Use by Pregnancy Stage Among Commercially Insured Patients in the United States, 2016-2019

Summary: This study examines the utilization of telehealth in maternal health services among commercially insured patients during the antenatal, delivery, and postpartum phases. The results show that 3.5% of visits were conducted through telehealth, with higher usage in the antenatal and postpartum periods. Additionally, the proportion of telehealth services increases in areas with a higher share of Black and Latinx residents.

Evaluating artificial intelligence software for delineating hemorrhage extent on CT brain imaging in stroke AI delineation of ICH on CT

However, it was more likely to over- or under-estimate the extent when ICH was more extensive, intraventricular, or extra-axial.

Safety and efficacy of endovascular thrombectomy for primary and secondary MeVO

Summary: This study aimed to evaluate the efficacy and safety of endovascular treatment (EVT) in primary and EVT-induced secondary medium-vessel occlusions (MeVO). The results showed that primary MeVO had poorer outcomes in terms of complications, EVT efficacy, and the proportion of patients with a modified Rankin Scale (mRS) of 0-2 at 3 months compared to exclusive large-vessel occlusion (LVO). However, EVT-induced secondary MeVO showed similar safety and efficacy outcomes as exclusive LVO and had better outcomes in terms of mRS 0-2 compared to primary MeVO.

One year clinical outcome of dual anti-platelet therapy with the Novel Ticagrelor plus Aspirin versus Clopidogrel plus Aspirin for Endovascular Intervention of patients with Intracranial Aneurysm: A meta-analysis

Summary: Compared with clopidogrel, DAPT with ticagrelor plus aspirin was associated with significantly lower thromboembolic events without an increase in hemorrhagic events in the treatment of intracranial aneurysms, according to the one-year clinical outcome analysis. However, larger upcoming trials are needed to confirm the effectiveness and safety of ticagrelor-associated DAPT.

An examination of geographic access to outpatient stroke rehabilitation services in Tennessee, a stroke belt state

Summary: This study assessed the geographic accessibility of stroke rehabilitation facilities in Tennessee. The study found that rural counties had significantly longer travel distances and times compared to urban counties. Counties with high and medium stroke prevalence also had higher travel times and distances compared to low stroke prevalence counties. Therefore, addressing transportation barriers and implementing tele-rehabilitation are crucial for improving access to stroke aftercare in these areas.

Role of serum alkaline phosphatase as a potential biomarker of severity and prognosis in intracerebral hemorrhage

Summary: The study aimed to investigate the relationship between serum alkaline phosphatase (ALP) levels and the severity and prognosis of intracerebral hemorrhage (ICH). The results showed that serum ALP levels in ICH patients were significantly elevated and correlated with hematoma volume and Glasgow coma scale (GCS) score. High serum ALP levels may serve as a potential predictor for the severity and unfavorable prognosis of ICH.
