Summary: Depression is common in patients with ischemic heart disease, and depressed patients are more likely to develop atherosclerosis and experience major cardiac events compared with the general population. The underlying pathophysiological mechanisms of these two diseases are highly interwoven and include an increased release of stress hormones, dysregulation of the autonomic nervous system, alterations of pathways related to primary and secondary hemostasis, endothelial dysfunction, and higher level of residual inflammation. Furthermore, depression negatively impacts compliance with medication regimens. Early recognition and treatment of depression provide the opportunity to improve outcomes of patients with ischemic heart disease.

Among 37 included studies, 22 adopted the Fried criteria for defining frailty and were included in meta-analyses, showing that frailty is quite common in older persons with IHD. Insufficient data were found to assess the longitudinal association between frailty and IHD. Further research is needed to explore the potential common biological ground between frailty and IHD.

Summary: Mental disorders starting in childhood can have severe consequences throughout an individual's lifespan. While effective short-term treatments exist for common mental disorders in young people, little is known about their long-term effects. This editorial perspective examines the long-term effectiveness and safety of treatments for attention deficit hyperactivity disorder, behavior disorders, and anxiety and depressive disorders in children aged 6 to 12 years, and discusses methodological difficulties and risk-benefit ratio of these treatments.

Summary: This study aimed to determine the gait parameters that determine the optimal gait modification strategy for individual patients with knee osteoarthritis. Through 3D motion analysis of 47 patients, it was found that 68.1% of the patients had the optimal strategy of trunk lean, and there were no significant differences in baseline characteristics, kinematics, and kinetics during comfortable walking. However, the regression model of this study only explained 12.3% of the variance, suggesting that clinical application may not be feasible, and direct assessment of kinetics seems to be the most optimal strategy for selecting the most optimal gait modification strategy.

Summary: This study investigated the effects of running speed and step frequency alterations on patellofemoral joint stress (PFJS) and patellar tendon force (PTF) parameters. The findings showed that with increasing running speed, peak PFJS and PTF increased, while PFJS and PTF impulse per km decreased. Increasing step frequency resulted in decreased peak PFJS and PTF as well as PFJS and PTF impulse per km. The effectiveness of step frequency alteration was dependent on running speed.

Most fatigue-induced landing alterations present at the time of return-to-sport normalize after resumption of sports activities. However, a larger knee abduction moment in the injured leg after resumption of sports activities can still be observed.

Summary: In their response, Dekkers et al. argue that treatment is the best option for children with mental disorders due to 'sound evidence' of its effectiveness in both short and long-term. While we agree that there is evidence for short-term effectiveness and certain treatments have shown some long-term effects, such as behavioral parent training for behavioral disorders, we strongly disagree that there is solid evidence for long-term effectiveness overall.

Summary: This systematic review examined the effectiveness of exercise and physical activity programs on fatigue and sleep in people with arthritis. 36 studies involving 2281 participants were included. The evidence was inconclusive for the benefits of exercise on fatigue and sleep in people with osteoarthritis and psoriatic arthritis, while there was slight evidence of short-term benefit in people with ankylosing spondylitis and rheumatoid arthritis. However, the evidence for medium- and long-term benefits and any sleep parameters was uncertain.

Summary: Osteoarthritis (OA) is a common condition that causes disability worldwide, with clinical pain as its main symptom. Insomnia symptoms are closely associated with clinical OA-related pain in a significant percentage of OA patients. This review focuses on the mechanisms linking insomnia symptoms to OA-related pain and the efficacy of non-pharmacological treatments on both symptoms. The evidence suggests that depressive symptoms, pain catastrophizing, and pain self-efficacy partially explain the association between insomnia symptoms and pain in OA patients. Treatments including an insomnia intervention are more effective in improving insomnia symptoms, but not in reducing clinical OA-related pain. However, positive effects on insomnia symptoms from treatment may lead to long-term pain reduction. Future studies investigating the neurobiological and psychosocial mechanisms underlying this association will help develop effective treatments targeting both insomnia symptoms and OA-related pain.
