Summary: At the start of the 20th century, cesarean section (CS) was uncommon in obstetrics. By the end of the century, CS rates had increased dramatically worldwide. The increase can be attributed to a rise in women delivered by repeat CS and a decrease in vaginal birth after CS rates due to concerns of uterine rupture. This paper reviewed international VBAC policies and trends, highlighting the low risk of intrapartum rupture and the need for better resources and practices for TOLAC.

Summary: This study aimed to assess the reproducibility of a standardized method for measuring Cesarean section scars and niches using transvaginal ultrasound. The results showed that both 2D and 3D ultrasound imaging can be used to assess the position and characteristics of the scars and niches with high reproducibility. The distance from the scar to the internal cervical os was the most reproducible measurement parameter.

Summary: In high-income settings, it is generally agreed upon that pregnant women with a previous cesarean section should have the choice between planning another cesarean section or opting for vaginal birth after cesarean, as long as there are no contraindications. Clinical guidelines recommend providing counseling on the associated risks and benefits to help women make an informed decision. Recent research suggests that planned vaginal birth after cesarean is associated with a higher risk of certain complications but also has benefits such as shorter hospital stays and increased likelihood of breastfeeding.

Summary: This study evaluated the reproductive outcomes of patients with previous cesarean section undergoing single-embryo transfer (SET). The results showed that patients with previous CS and VD had similar CPR and LBR, but CS patients had a significantly higher preterm birth rate in multiple births compared to VD. In IVF/ICSI and FET cycles, patients under 35 years old treated with SCT had lower CPR and LBR, and patients over 35 years old had even lower rates.

Summary: Infertility, regardless of fertility treatment, is linked to higher risks of adverse pregnancy outcomes, such as cesarean section (CS) and preterm birth (PTB). These complications have significant physical, mental, emotional, social, and financial costs. Single embryo transfer policies in publicly funded IVF programs have shown positive health outcomes and cost savings. Low-dose aspirin prophylaxis may be considered for reducing PTB in infertility patients. Lifestyle modification and social policies addressing the needs for fertility treatment may also have a beneficial impact on both CS and PTB rates.

Short and long IPIs are associated with higher risk of PTB after a previous PTB, suggesting a potentially modifiable risk factor for women with a history of PTB.

Summary: This study aimed to investigate factors predicting cesarean section in patients with previous cesarean section using ultrasonography at 37 weeks gestation. Results showed that history of vaginal birth in previous cesarean section patients can predict reaching full-term, while weight gain, vaginal birth history, and estimated fetal weight are independent factors affecting cesarean section time after 37 weeks of pregnancy.

Summary: This study examined the hepatitis C virus cascades of care among immigrants and long-term residents in Ontario, Canada, showing that immigrants had higher rates of nucleic-acid testing, treatment initiation, and viral clearance compared to long-term residents. The findings provide a population-based benchmark for future studies and evaluation of treatment programs and surveillance activities before the introduction of newer medications.
