Altered expression of HER-2 and the mismatch repair genes MLH1 and MSH2 predicts the outcome of T1 high-grade bladder cancer

The identification of factors predicting the outcome of stage T1 high-grade bladder cancer (BC) is a major clinical issue. We performed immunohistochemistry to assess the role of human epidermal growth factor receptor-2 (HER-2) and microsatellite instability (MSI) factors MutL homologue 1 (MLH1) and MutS homologue 2 (MSH2) in predicting recurrence and progression of T1 high-grade BCs having undergone transurethral resection of bladder tumor (TURBT) alone or TURBT + intravesical instillations of bacillus Calmette-Guerin (BCG). HER-2 overexpression was a significant predictor of disease-free survival (DFS) in the overall as well as in the two patients' population; as for progression-free survival (PFS), it was significant in the overall but not in the two patients' population. MLH1 was an independent predictor of PFS only in patients treated with BCG and MSH2 failed to predict DFS and PFS in all populations. Most importantly, the higher the number of altered markers the lowers the DFS and PFS. In multivariate Cox proportional-hazards regression analysis, the number of altered molecular markers and BCG treatment were significant predictors (p = 0.0004 and 0.0283, respectively) of DFS, whereas the number of altered molecular markers was the only significant predictor (p = 0.0054) of PFS. Altered expression of the proto-oncogene HER-2 and the two molecular markers of genetic instability MLH1 and MSH2 predicted T1 high-grade BC outcome with the higher the number of altered markers the lower the DFS and PFS. These findings provide grounds for further testing them in predicting the outcome of this challenging disease.

Summary: This study examines the factors influencing the costs of cardiovascular disease in Italy's Apulia region, with findings showing an increase in health costs in 2016, lower spending for women, and a significant increase in health expenditure with age. Day-hospital has the lowest cost, surgery increases costs, with acute myocardial infarction being the most expensive pathology compared to atrial fibrillation. The inclusion of real data in estimating the economic burden of CVD from a regional health service perspective is significant for health management and planning.

Early treatment with dexamethasone intravitreal implants in diabetic macular edema: Naive versus refractory patients

Summary: In this study, dexamethasone 0.7 mg implants (DEX-I) were found to improve visual acuity and reduce macular edema mainly in treatment-naive patients with diabetic macular edema (DME). Treatment-refractory patients also showed significant reductions in central macular thickness (CMT) after DEX-I injections, but no changes were observed in best-corrected visual acuity (BCVA). These findings suggest that DEX-I may be a promising first-line treatment option for DME.

High-Dimensional Clustering via Random Projections

Summary: This method addresses unsupervised classification for high-dimensional data using a Random Projection Ensemble approach. By generating low-dimensional independent random projections and conducting model-based clustering on each, the approach identifies top projections with the best grouping structures. The final partition is obtained by aggregating clusters found in these projections. Results suggest potential for high-dimensional clustering.

Symptomatic SARS-CoV-2 Infection with Ageusia after Two mRNA Vaccine Doses

Summary: Despite being fully vaccinated, some individuals may still experience symptomatic SARS-CoV-2 infection. A case report of a young dental practitioner showed symptomatic infection 55 days after receiving the Pfizer vaccine, with ageusia as a notable symptom. The use of personal protective equipment helped prevent the spread of infection to patients and colleagues. While vaccines have proven high efficacy, it is important to adopt new guidelines and markers to identify individuals who may not achieve full immunization.

Estimating the Proportion of Asymptomatic COVID-19 Cases in an Italian Region with Intermediate Incidence during the First Pandemic Wave: An Observational Retrospective Study

Summary: This retrospective epidemiological study analyzed asymptomatic COVID-19 cases in the Apulia region, Italy, during the first wave of the pandemic. The study found that the proportion of asymptomatic infections was 19.9%, which decreased with increasing age, the presence of underlying comorbidities, and in males. The study also revealed that the median duration of asymptomatic SARS-CoV-2 RNA positivity was 19 days, and 74% of cases resolved within 14 days after the first positive PCR test.

Summary: This study evaluated the knowledge, attitudes, and opinions of the general population in Italy regarding COVID-19 and influenza immunization. The findings showed that 17.5% of respondents were unlikely to accept a future COVID-19 vaccine, mainly due to a lack of trust, fear of side effects, or a lack of perceived susceptibility to the disease. While over 50% of participants were willing to receive flu vaccinations, only 28.2% had received it in the previous five seasons. Higher knowledge about COVID-19 and previous flu vaccination were significantly associated with the intention to be vaccinated against COVID-19 and influenza.

Most vaccinees retained vaccine-specific T-cell memory six months after vaccination.

Summary: In Europe, the implementation of National Immunisation Technical Advisory Groups (NITAGs) has not been optimal, despite their establishment in most countries. A survey conducted within the European Joint Action on Vaccination (EU-JAV) collected information on decision-making processes regarding the introduction of new vaccines or changes to recommendations. Disease burden and availability of financial resources were identified as the most relevant criteria. Collaboration between NITAGs could lead to better workload optimization and resource utilization.

Assessment of Stress Salivary Markers, Perceived Stress, and Shift Work in a Cohort of Fishermen: A Preliminary Work

Summary: Due to work-related stress, work has become a daily challenge for many occupations. The scientific community is now studying the disorder in various professions to evaluate and assess it. This paper proposes a comprehensive assessment approach by studying three biological salivary markers. The research on fishermen in the Apulia region of Italy reveals that sleep disturbances and work conflicts significantly affect the levels and rhythms of salivary markers. The results provide potential opportunities for using this approach in health surveillance to prevent mental and/or physical disorders and suggest further research directions.
