Summary: Therapeutic strategies focused on pulp preservation are important in managing deep caries and exposed pulp in vital teeth. While vital pulp treatments (VPTs) have been used for over a century, recent advancements in regenerative endodontics and biologically based therapies have expanded the indications for VPT to include mature cariously affected teeth and teeth with irreversible pulpitis. However, gaps in scientific knowledge and limited educational dissemination among dentists pose challenges in accurately diagnosing the inflammatory state of the pulp and providing consistent treatment options. This review discusses the current status of VPT, identifies the major problems hindering its clinical acceptance, and offers potential solutions for making VPT a more consistently prescribed evidenced-based treatment in dental practice.

Summary: The promotion of minimally invasive treatments focused on maintaining pulp vitality is a priority in endodontics. This review discusses the outcomes of partial and full pulpotomy procedures and analyzes various factors that influence the results. Current evidence suggests that age, gender, tooth type, and intraoperative pulpal hemorrhage do not significantly impact the outcome, while factors such as caries depth, pulp inflammation, capping materials, and restoration integrity may affect the success. Further experimental research is needed to definitively determine the success of pulpotomy procedures.

Summary: This study aimed to investigate whether a full pulpotomy (FP) performed by a general practice dentist (GPD) in a public dental care clinic (PDCC) is suitable for patients who cannot afford root canal treatment (RCT) and to assess its success rate and patient satisfaction. The results of the study showed excellent success rate and patient satisfaction with FP performed by a GPD in a PDCC, making it a viable routine treatment option for symptomatic irreversible pulpitis.

Summary: The objective of this study was to evaluate the short-term evolution of pain and the treatment success of full pulpotomy as permanent treatment for irreversible pulpitis. The results showed that there was no significant difference between full pulpotomy and root canal treatment in terms of short-term pain evolution, clinical success, and global success. However, there was a significant difference in radiographic success. This study adds to the existing literature supporting the use of pulpotomy as an acceptable and conservative treatment option for a larger population of dentists.

The results showed that full pulpotomy could be an alternative treatment to RCT with similar success rates, lower pain levels, and shorter pain relief time. Additionally, patients were more satisfied with pulpotomy in terms of time, intraoperative pain, pleasantness, and cost.

Summary: This study investigated the factors that affect clinicians' decisions in direct pulp capping (DPC) cases, the preferred method of caries removal, and the preferred capping material. The results showed that carious-exposed pulp was the most important factor in clinical decisions, while the number of exposures had the least impact. Overall, complete caries removal was preferred over selective caries removal, and calcium silicate-based materials were preferred over calcium hydroxide-based materials.
