Management of idiopathic recurrent pericarditis in adults and in children: a role for IL-1 receptor antagonism

Recurrent pericarditis is one of the most frequent pericardial diseases, affecting up to 30% of the patients who have experienced acute pericarditis. While the diagnosis of acute pericarditis is sometime straight forward, its etiology and therapeutic management are still a challenge for physicians. In developed countries, the idiopathic form is the most frequent, and the search for an infectious etiology is almost invariably negative. Nevertheless, since standard treatment with nonsteroidal anti-inflammatory drugs and colchicine is not always able to neutralize pericardial inflammation in recurrent pericarditis, anakinra, an IL-1 receptor antagonist, has been proposed as a possible therapeutic alternative for refractory forms. IL-1 is a cytokine that exerts a pivotal role in innate immunity and in the pathogenesis of some autoimmune diseases, such as rheumatoid arthritis, and in autoinflammatory disorders, as familial Mediterranean fever and cryopyrin-associated periodic syndromes. The successful management of patients with acute idiopathic recurrent pericarditis (IRP) needs a teamwork approach, where cardiologists, rheumatologists, clinical immunologists and internists are involved. In this review, we will discuss the clinical and therapeutical challenges of IRP both in adults and children from a clinical practice standpoint. We will also briefly illustrate the main pathogenic mechanisms of IRP to provide internists and cardiologists with the rationale for approaching the use of anakinra in selected clinical cases.

Pediatric Scleritis: An Update

Summary: Posterior idiopathic scleritis is the most common type of scleritis observed in childhood, but anterior and necrotizing inflammatory scleritis can also occur. While less common than in adults, systemic disorders can be associated with scleral inflammation in children and should be promptly recognized and treated to prevent complications. A multidisciplinary diagnostic work-up is necessary to rule out infectious and autoimmune causes and tailor treatment to the specific patient.

Vascular Behcet syndrome: from pathogenesis to treatment

Prevalence and clinical associations of ultrasound-confirmed enthesitis in systemic lupus erythematosus

Summary: A retrospective analysis found that enthesitis, defined as inflammation at the sites of tendon and ligament attachments to bone, is fairly common in patients with systemic lupus erythematosus (SLE). Patients with enthesitis had a distinct disease phenotype with less renal involvement, more arthritis, and a low response to anti-CD20 therapy.

Summary: This study describes the presence of interstitial lung disease (ILD) in antineutrophil cytoplasmic antibodies-associated vasculitides (AAV). It found that non-specific interstitial pneumonia (NSIP) was the most common ILD pattern, observed in c-ANCA positive patients. The study also highlights the early complication of ILD in AAV and emphasizes the importance of early diagnosis by both pulmonologists and rheumatologists.

Color Doppler Eye Ultrasonography in giant cell arteritis: differential diagnosis between arteritic and non-arteritic sudden blindness

Summary: The aim of this study was to assess the role of Color Doppler Eye Ultrasound (CDEUS) in the differential diagnosis between arteritic and non-arteritic blindness. The results showed that CDEUS displayed good reliability in distinguishing between arteritic and non-arteritic central retinal artery occlusion (CRAO), but no significant difference was found in arteritic and non-arteritic anterior ischemic optic neuropathy (AION). Therefore, CDEUS is not a reliable procedure in the diagnostic work-up of giant cell arteritis (GCA) and should be restricted to the exclusion of thrombo-embolic occlusions within the territory of central retinal artery.
