Summary: We report a rare case of successful live birth after assisted oocyte activation of failed fertilized oocytes. A woman with multi-factor infertility underwent in vitro fertilization (IVF) using intracytoplasmic sperm injection (ICSI) of frozen/thawed testicular sperm aspiration (TESA) sample. The oocytes showed failed fertilization, but the cycle was rescued using an assisted oocyte activation (AOA) protocol. One genetically normal embryo resulting from the AOA was transferred and led to a normal pregnancy and live birth.

The study also shows that Artificial Oocyte Activation (AOA) after Intracytoplasmic Sperm Injection (ICSI) successfully rescued oocyte activation failure and resulted in a live birth.

Summary: During mammalian fertilization, intracellular calcium oscillations are needed for oocyte activation. Sperm lacking PLC zeta can still induce calcium rises in oocytes after IVF but not ICSI. Additional activation treatments such as SrCl2 and PLC zeta mRNA can rescue the developmental ability of Plcz1(-/-) sperm-derived oocytes after ICSI, leading to successful term development and healthy offspring.

Summary: This study investigates the mechanisms underlying the effects of Actl7a gene knockout on sperm acrosome formation, male infertility, and sperm-zona pellucida binding ability during fertilization. The findings suggest that Actl7a deficiency leads to malformed sperm acrosomes, male infertility, and impaired sperm-ZP binding ability. Furthermore, abnormal localization and expression of the PLCZ1 protein in misshapen sperm are identified, resulting in reduced calcium oscillations in oocytes.

Summary: This retrospective cohort study compared the fertilization rates in ICSI cycles treated with artificial oocyte activation (AOA) using 10 mu mol/l ionomycin or commercial A23187. The results showed that the fertilization rates were significantly higher in the ionomycin group compared to the A23187 group for all subgroups. Therefore, AOA with 10 mu mol/l ionomycin may be more effective in improving oocyte activation, especially in cases of sperm-related defects.

Summary: Spermatozoa can be recovered in half of patients with non-obstructive azoospermia (NOA) through testicular sperm extraction (TESE) or microTESE. Intracytoplasmic sperm injection (ICSI) is then used with the recovered spermatozoa. For patients who cannot retrieve spermatozoa, round spermatid injection (ROSI) is a potential option, although the success rates are currently low. One possible reason is oocyte activation deficiency (OAD), which can be improved through optimal assisted oocyte activation (AOA) stimulation.
