Summary: A novel flow cytometric microsphere-based immunoassay (FloCMIA) using recombinant human proinsulin (PI) as an alternative to the Radioligand Binding Assay (RBA) was developed in this study. The method showed fair ability to distinguish between samples from diabetic patients and normal controls, with a prevalence of 50% for PAA in the diabetic patient population. The assay demonstrated good performance, wide dynamic range, and the potential for multiplex immunoassays for the detection of autoantibodies in Type 1 Diabetes Mellitus and other autoimmune diseases.

Summary: This study aimed to evaluate the severity of depression and fatigue in elderly patients with poorly controlled type 2 diabetes. The study found that female patients, those aged 21 years and above, those using premixed insulin, and those living alone had higher scores in terms of depression and fatigue. Additionally, patients with vitamin D levels below 20ng/mL experienced higher levels of fatigue.

Summary: The study found a positive association between serum CRP levels and depressive symptoms in patients with diabetes, while no significant association was found in patients without diabetes. Additionally, higher serum CRP levels were associated with an increased risk of depressive symptoms in subjects with diabetes mellitus.

Summary: This study aimed to evaluate the overall health status, health-related abilities, and problems of elderly patients with diabetes and multimorbidity compared to those with diabetes only. The results showed that elderly patients with diabetes and multimorbidity experienced more health-related abilities and problems, particularly in the domains of depression and diabetes distress. Furthermore, the level of health literacy and self-management in elderly, multimorbid patients with diabetes was significantly higher than in those with diabetes only.

Summary: Three distinct patterns of maternal C-peptide trajectory were observed in pregnant women with type 1 diabetes, with the appearance of C-peptide at 34 weeks associated with midtrimester hyperglycemia, elevated cord blood C-peptide, and high rates of neonatal complications such as large for gestational age, neonatal hypoglycemia, and neonatal intensive care admission. This finding suggests the transfer of C-peptide from fetal to maternal serum and is inconsistent with the hypothesis of pregnancy-related beta-cell regeneration.

Summary: The study found that age played an important role in the effectiveness of agomelatine in treating depressed T2DM patients. While there were no significant differences in treatment outcomes between agomelatine and paroxetine/fluoxetine in younger patients, agomelatine showed better results in older patients, indicating that it might be more suitable for elderly depressed T2DM patients.

Summary: This study investigated the self-care behavior, depression, and self-efficacy among elderly Chinese type 2 diabetes mellitus (T2DM) patients. The results showed that only 22.5% of patients reported good self-care behavior, and depression partly mediated the association between self-efficacy and self-care behavior. Self-efficacy was negatively associated with depression and depression was negatively associated with self-care behavior. Therefore, interventions focusing on self-efficacy may improve diabetes self-care behavior among elderly T2DM patients.

Summary: It is uncertain whether albuminuria can identify elderly patients with diabetes at high risk of ESKD or mortality. The study found that both microalbuminuria and macroalbuminuria are risk factors for incident ESKD and mortality in elderly patients with diabetes.

Summary: A study in Denmark from 1997 to 2017 found that the incidence rates of fractures were higher in patients with type 1 and type 2 diabetes compared to those without diabetes, except for foot fractures. The incidence of hip fractures decreased by 35.2%, 47.0%, and 23.4% in patients with type 1 diabetes, type 2 diabetes, and without diabetes, respectively, over the last 5 years. However, vertebral fractures increased by 14.8%, 18.5%, and 38.9% in the same groups. After age adjustment, patients with type 1 diabetes still had a higher risk of fractures compared to those without diabetes, while patients with type 2 diabetes had a risk similar to those without diabetes.

Summary: The study highlighted a high prevalence of cognitive impairment, depression, and anxiety among elderly patients with type 2 diabetes mellitus. Indian ethnicity and completion of secondary or tertiary education were significant factors associated with cognitive impairment. Early detection and treatment of diabetes, as well as addressing associated risk factors, are crucial in preventing cognitive decline.

Summary: Children with new-onset type 1 diabetes undergo progressive beta-cell endoplasmic reticulum stress and abnormal proinsulin processing, as evidenced by increasing proinsulin to C-peptide ratios. Moreover, the ratio reflects a more aggressive beta-cell attack with younger age, as well as decreased glycemic control.

Summary: The study utilized the CogEvo computerized cognitive assessment and training tool to distinguish patients with mild Alzheimer's disease and mild cognitive impairment from cognitively normal older people. The results indicated that CogEvo showed good correlation and was able to differentiate between the three groups effectively. Further research is needed to assess the effectiveness of CogEvo as a combined assessment and training tool for early-stage cognitive impairment.

Summary: This study investigated the causes of caregiver burden in patients with dementia using the Dementia Assessment Sheet for Community-based Integrated Care System-21 items (DASC-21) and Zarit Caregiver Burden Interview (ZBI). The results showed that behavioral and psychological symptoms of dementia (BPSD), cognition, and instrumental activities of daily living (IADL) have affected caregiver burden.

Summary: This study investigated the association between metabolic syndrome (Met-S) and subclinical atherosclerosis in elderly diabetic patients, using the carotid artery plaque score (PS) as an indicator. A total of 187 subjects were divided into middle-aged and older groups. The results showed a significant association between Met-S and subclinical atherosclerosis in the middle-aged group, but not in the older group.

Summary: Insulin is synthesized in beta-cells from preproinsulin, which is then processed into proinsulin and further separated into C-peptide and insulin. Hyperproinsulinemia, suspected to be a pancreatic beta-cell defect exacerbated by hyperglycemia, was successfully managed with a combination of alogliptin, pioglitazone, and luseogliflozin in a patient with type 2 diabetes mellitus. Monitoring proinsulin levels may be a useful strategy in managing treatment-resistant cases of diabetes mellitus.
