Summary: A study in Denmark from 1997 to 2017 found that the incidence rates of fractures were higher in patients with type 1 and type 2 diabetes compared to those without diabetes, except for foot fractures. The incidence of hip fractures decreased by 35.2%, 47.0%, and 23.4% in patients with type 1 diabetes, type 2 diabetes, and without diabetes, respectively, over the last 5 years. However, vertebral fractures increased by 14.8%, 18.5%, and 38.9% in the same groups. After age adjustment, patients with type 1 diabetes still had a higher risk of fractures compared to those without diabetes, while patients with type 2 diabetes had a risk similar to those without diabetes.

Summary: This study aimed to investigate the incidence, risk factors, and predictors of cardiovascular mortality for aortic stenosis in type 2 diabetes patients. The results showed that aortic stenosis was associated with an increased risk of cardiovascular mortality and hospitalization for heart failure, with age, female gender, chronic kidney disease, hyperlipidemia, and peripheral arterial disease being significant risk factors for aortic stenosis.

Summary: This retrospective cohort study aimed to estimate the incidence of corneal endothelial transplantation (CET) and identify risk factors among patients with noninfectious ocular inflammation. The results showed that CET occurred infrequently in patients with ocular inflammation, but was associated with factors such as tube shunt surgery, hypotony, and band keratopathy.

Summary: This study aimed to compare the prevalence of hypogonadism between male patients with latent autoimmune diabetes (LADA) and type 2 diabetes (T2DM) and investigate the risk factors for hypogonadism in these patients. The rate of hypogonadism in the LADA group was comparable to those of the T2DM group after adjusting for possible confounders. BMI, triglycerides, and estradiol were independently associated with the presence of HH in male diabetic patients.

Summary: Type 2 diabetes is a known risk factor for dementia, with higher or unstable HbA(1c) levels and the presence of diabetic complications increasing the risk of dementia among patients with diabetes. Effective management of glycemia may play a significant role in preserving cognitive health in older adults with diabetes.

Summary: This study aimed to investigate the relationship between different combinations of risk factors and complications in diabetic patients. The results showed that a combined diagnosis of arterial hypertension and dyslipidemia was strongly associated with diabetic complications across all risk clusters. The presence of additional diagnoses such as smoking, depression, obesity, or liver disease further increased the risk of complications.

Summary: Limited evidence suggests a link between plasma levels of multiple metals and incident cancer risk in patients with type 2 diabetes mellitus. Copper and lead were significantly associated with increased cancer risk, while zinc and chromium were associated with decreased cancer risk. Monitoring metal levels in diabetic patients is crucial for cancer prevention.

Summary: In multimorbid older patients with type 2 diabetes mellitus (T2DM), more than one third were overtreated, highlighting the high prevalence of this problem. Careful balancing of benefits and risks in the choice of glucose-lowering medication (GLM) may improve patient care, especially in the context of comorbidities such as severe renal impairment, and frequent non-GP healthcare contacts.

Summary: This study examined the association between smoking behavior change and the risk of cardiovascular disease incidence and mortality in patients with type 2 diabetes mellitus (T2DM). The results showed that quitting smoking was associated with a significantly decreased risk of myocardial infarction and ischemic stroke, whereas reducing smoking was not significantly associated with these risks.

Summary: This study found that there was no increased risk of fracture in males with type 2 diabetes following diagnosis, and a slight reduced risk in females. Individuals aged 85 years and older with type 2 diabetes had a significantly lower risk of incident fracture. In the most deprived areas, both males and females with type 2 diabetes had reduced risk of fracture.

Summary: This study aimed to identify the risk factors associated with hypothyroidism and assess the relationship between hypothyroidism and hypertension in patients with diabetes. The study found a higher prevalence of diabetes mellitus among patients with hypertension, and subclinical hypothyroid subjects need to be frequently screened for hypertension. These findings highlight the importance of an integrated approach to managing hypertension and diabetes in high-risk populations.

Summary: The study aimed to determine if enzyme-inducing antiseizure drugs (ASDs) affect the risk of developing chronic obstructive pulmonary disease (COPD) or lung cancer in smokers. Results showed a slightly decreased risk of COPD and lung cancer among smokers taking enzyme-inducing ASDs, with this association being attenuated in heavy smokers.

Summary: The study assessed the completeness and validity of bariatric surgery codes in the UK CPRD GOLD compared with HES data. Different surgery code patterns were found between bariatric surgery and other gastrointestinal surgery. The sensitivity of CPRD GOLD bariatric surgery coding improved with stricter algorithms, while PPVs remained similar. Validity measures of CPRD GOLD bariatric surgery codes were consistent across obese patients and more restrictive populations.

Summary: Our study found no association between glycemic control, measured by HbA1c levels, and the risk of VTE. However, when the most recent HbA1c value recorded within 90 days before the index date, women with HbA1c levels > 7.0% had a 36-55% increased relative risk of VTE compared to women with HbA1c levels > 6.5-7.0%. There was no observed effect of glycemic control on the risk of VTE in men.

Summary: This study explores the association between beta-blocker use and new-onset depression. The findings suggest that current short-term use of any beta-blocker is associated with an increased risk of developing depression, while long-term use is not. The elevated risk among short-term users is mainly observed in propranolol users with a neuropsychiatric disorder, indicating that the underlying indication may be a contributing factor to the onset of depression.

Summary: This study evaluated the association between non-steroidal anti-inflammatory drugs (NSAIDs) and myocardial infarction (MI). The results showed that current use of diclofenac was associated with an increased risk of MI, especially among new concomitant users of proton-pump inhibitors (PPIs). The risk of MI in ibuprofen users was concentrated in new PPI users, while there was no material increased risk in naproxen users or past users of most NSAIDs in the absence of PPIs. These findings suggest the possibility of reverse causality bias.

Summary: This study compared the incidence rates of adverse events in patients receiving apremilast with those receiving other systemic treatments for psoriasis or psoriatic arthritis. The results showed that there were no new safety signals identified in the apremilast-exposed cohort. The incidence rates of adverse events were comparable between the apremilast and non-apremilast cohorts, indicating that the long-term safety of apremilast in real-world settings is similar to that reported in clinical trials.
