Anticoagulation Control in Older Atrial Fibrillation Patients Receiving Vitamin K Antagonist Therapy for Stroke Prevention

Hanis Zulkifly, Gregory Y. H. Lip, Deirdre A. Lane

Summary: This study examined the quality of anticoagulation control, adverse clinical outcomes, and bleeding risk in atrial fibrillation patients aged 80 years and above. The results showed suboptimal anticoagulation control in all patients and increased bleeding risk in the older age group. However, there were no differences in thromboembolic events and all-cause mortality in this age group.

Assessment and mitigation of bleeding risk in atrial fibrillation and venous thromboembolism: A Position Paper from the ESC Working Group on Thrombosis, in collaboration with the European Heart Rhythm Association, the Association for Acute CardioVascular Care and the Asia-Pacific Heart Rhythm Society

Diana A. Gorog, Ying X. Gue, Tze-Fan Chao, Laurent Fauchier, Jose Luis Ferreiro, Kurt Huber, Stavros Konstantinidis, Deirdre A. Lane, Francisco Marin, Jonas Oldgren, Tatjana Potpara, Vanessa Roldan, Andrea Rubboli, Dirk Sibbing, Hung-Fat Tse, Gemma Vilahur, Gregory Y. H. Lip

Summary: This article comprehensively reviews the published evidence on bleeding risk assessments in patients with atrial fibrillation (AF) and venous thromboembolism (VTE) and proposes consensus statements to summarize 'best practice' in antithrombotic therapy for these patients. The article discusses the epidemiology and size of the bleeding risk problem in AF and VTE, reviews established bleeding risk factors, and summarizes bleeding definitions. It also reviews the importance of patient values and preferences and balancing the risk of bleeding against thromboembolism.

Integrated care for optimizing the management of stroke and associated heart disease: a position paper of the European Society of Cardiology Council on Stroke

Gregory Y. H. Lip, Deirdre A. Lane, Radoslaw Lenarczyk, Giuseppe Boriani, Wolfram Doehner, Laura A. Benjamin, Marc Fisher, Deborah Lowe, Ralph L. Sacco, Renate Schnabel, Caroline Watkins, George Ntaios, Tatjana Potpara

Summary: Managing patients with stroke involves a multidisciplinary approach and coordination among various healthcare professionals, caregivers, and next-of-kin. The European Society of Cardiology Council on Stroke has proposed a consensus on Integrated care management to optimize the treatment of stroke and related heart disease, focusing on evidence-based practices and practical approaches for everyday clinical practice.

Impact of malignancy on outcomes in European patients with atrial fibrillation: A report from the ESC-EHRA EURObservational research programme in atrial fibrillation general long-term registry

Summary: This study evaluated the outcomes of European atrial fibrillation (AF) patients with active or prior malignancy. The results showed that active malignancy was independently associated with all-cause death, MACE, and bleeding events. However, the use of oral anticoagulants was not significantly associated with higher risks of all-cause death or bleeding in patients with active malignancies.

Stroke in Older Adults Living in Care Homes: Results From a National Data Linkage Study in Wales

Stephanie L. Harrison, Gregory Y. H. Lip, Ashley Akbari, Fatemeh Torabi, Leona A. Ritchie, Asangaedem Akpan, Julian Halcox, Sarah Rodgers, Joe Hollinghurst, Daniel Harris, Deirdre A. Lane

Summary: This study found that older individuals with a history of stroke in the 12 months prior to moving to a care home are at a higher risk of experiencing another stroke and mortality following a stroke. It was also observed that these individuals are often not prescribed medications for secondary stroke prevention.

Risk of Cerebrovascular Events in Intracerebral Hemorrhage Survivors With Atrial Fibrillation: A Nationwide Cohort Study

Summary: The study found that patients with ICH and AF have a high risk of cerebrovascular ischemic events and a very high risk of all-cause death one year after the incident. Further clinical trials are needed to determine the optimal stroke prevention treatment for ICH survivors with AF.

Contemporary management of atrial fibrillation in primary and secondary care in the UK: the prospective long-term AF-GEN-UK Registry

Alena Shantsila, Gregory Y. H. Lip, Deirdre A. Lane

Summary: This study established a registry to examine the management of UK patients with atrial fibrillation (AF) by different healthcare professionals. The study found that guideline-adherent oral anticoagulation significantly reduced the risk of death and stroke at 1 year, but only a quarter of patients received rhythm-control management.

Interactive Remote Patient Monitoring Devices for Managing Chronic Health Conditions: Systematic Review and Meta-analysis

Donato Giuseppe Leo, Benjamin J. R. Buckley, Mahin Chowdhury, Stephanie L. Harrison, Masoud Isanejad, Gregory Y. H. Lip, David J. Wright, Deirdre A. Lane

Summary: Telemonitoring is an effective approach to improve medical care for patients with long-term conditions, and patients have a high level of acceptance towards this technology. Meta-analyses revealed that telemonitoring can reduce mortality, improve blood pressure and glycated hemoglobin, but it does not have a significant impact on patients' quality of life, and there may be an increased risk of hospitalization.

Living with a left ventricular assist device: Capturing recipients experiences using group concept mapping software

Anita Slade, Margaret O'Hara, David Quinn, Laura Marley, Stephen Griffith, Melanie Calvert, M. Sayeed A. Haque, Hoong Sern Lim, Philippa Doherty, Deirdre Lane

Summary: This study used GCM software to remotely capture and structure the experiences of LVAD recipients, identifying important domains and items. The positive impact of the LVAD received the highest ratings, with other important domains including physical and cognitive limitations, LVAD restrictions, partner/family support, and equipment and clothing.

Prevalence and outcomes of atrial fibrillation in older people living in care homes in Wales: a routine data linkage study 2003-2018

Summary: The prevalence of atrial fibrillation (AF) is relatively high among older care home residents, and it is closely associated with cardiovascular disease and mortality risk. This suggests the need for appropriate treatment and management of AF in older adults, even in the presence of other risk factors.

Summary: Education for patients with atrial fibrillation in Denmark varied in terms of coverage, organization, and content. Initially, almost half of the hospitals did not provide patient education, but at 1-year follow-up, 69% of hospitals started offering patient education. Patient education differed in terms of delivery, frequency, and duration.
