Summary: Programmes for early intervention in psychosis have proven to be effective in both clinical and cost aspects. This research aims to describe how participants have engaged and responded to the innovative model of care in Ireland. Thematic analyses of semi-structured interviews revealed that the speed of referral/assessment, multidisciplinary approach, evidence-based interventions, and assertive follow-up were key facilitators of implementation. However, staff recruitment and budget challenges were identified as primary barriers.

Summary: This study reports on delusions during the initiation of indicated treatment for first-episode psychosis (FEP). Delusions were present in the vast majority of individuals experiencing onset of FEP (94%), with persecutory delusions being the most common theme. Anxiety was associated with delusional severity, and persecutory delusions were more severe with older age at onset of FEP.

Summary: This study investigated the short-term service disengagement in Italian first-episode psychosis patients receiving early intervention services, and assessed the predictors of disengagement in the first year of treatment. The results showed that approximately 16% of patients disengaged from the program within the first year, and poor baseline treatment non-adherence, living with parents, and the presence of brief psychotic disorder or schizophreniform disorder at entry were strong predictors of service disengagement.

Prior understanding of these challenges and enablers is crucial before attempting implementation to predict and increase the likelihood of success, stability, and longevity of services.

Summary: This study reviewed the literature on sensory phenomena and neurodevelopmental antecedents in obsessive compulsive disorder (OCD) to propose a bottom-up perspective for understanding OCD. The findings indicate that sensory phenomena and neurodevelopmental features are associated with OCD. The study suggests a neurodevelopmental-phenomenological model that can explain the dimensional affinity between OCD and schizophrenia spectrum disorders.

Summary: Research shows that adverse childhood experiences (ACE) can impact educational achievements in individuals at clinical high risk of psychosis (CHR). Specifically, CHR individuals with ACE had lower education levels, employment rates, and estimated IQ compared to healthy controls. Early intervention programs for psychosis should integrate interventions to support the educational and vocational recovery of young CHR individuals, and public health and social interventions to prevent or reduce the impact of ACE are recommended.

Summary: This study investigated the association between environmental exposure score (ES-SCZ) and cognitive impairments in patients with schizophrenia spectrum disorders (SSD), their unaffected siblings, and healthy controls. The results showed that ES-SCZ was not associated with cognitive domains in SSD patients, but it was negatively associated with cognition in siblings and healthy controls, and positively associated with facial affect recognition in siblings.

Summary: This study aimed to test hypotheses on the correlates of conspiracy beliefs (CBs) and vaccination, including socio-demographic factors, personality dispositions, somatic health, stressful experiences, and psychological distress. The results showed that CBs were associated with disintegration, low openness, lower education, extraversion, living in smaller settlements, and employment, while vaccination was associated with older age, CBs, and living in larger settlements.

Understanding these factors is crucial for timely detection and treatment of depression-anxiety spectrum disorders.
