Sexual Dysfunction, Cardiovascular Risk and Effects of Pharmacotherapy

Background: Sexual dysfunction affects millions of people with an increasing prevalence, worldwide. The pathophysiology of the disease shares several similarities with cardiovascular disease (CVD), including atherosclerosis, endothelial dysfunction, structural vascular damage and subclinical inflammation. Erectile dysfunction (ED) and female sexual dysfunction are common among patients with CVD and risk factors such as hypertension, diabetes, obesity and metabolic syndrome. Given the common pathogenesis of the diseases, ED is an independent prognostic factor of future ED events. Patients with overt ED or risk factors are usually treated with several drugs for the management of these conditions. Several of these drugs have been evaluated for their effect on sexual activity. Results and Conclusion: Among the antihypertensive drugs, diuretics and beta-blockers seem to exert a detrimental impact on sexual function, with nebivolol being the only beta-blocker with favorable properties through an increase in nitric oxide bioavailability. In contrast, renin-angiotensin system inhibitors and calcium-channel blockers have a neutral effect on sexual activity. Hypoglycemic drugs have been less evaluated in the ED setting, with metformin, pioglitazone and liraglutide presenting favorable results. Statins on the other hand have not provided consistent results with observational studies suggesting a detrimental role in sexual activity and a few randomized studies indicating a neutral or even beneficial effect on erectile function.

Summary: This study used LSCI combined with PORH to evaluate skin microvascular function in PA patients, and found that SMF was significantly impaired in PAs compared to EHs and NTs, with a positive correlation between plasma aldosterone and occlusion flux in PAs. Further research is needed to explore the clinical implications of these findings in terms of pathophysiology and cardiovascular risk prediction.

Impact of Primary Aldosteronism in Resistant Hypertension

Summary: The review summarized the latest data on the association between primary aldosteronism and resistant hypertension, emphasizing the importance of screening all patients with resistant hypertension for primary aldosteronism. Epidemiological data suggests that up to 20% of patients with resistant hypertension may have primary aldosteronism. Patients with this condition are at increased risk of renal disease, diabetes mellitus, atrial fibrillation, and obstructive sleep apnea, and they also have extended target organ damage and higher cardiovascular morbidity and mortality. Specific treatments, such as adrenalectomy and mineralocorticoid receptor antagonists, can significantly reduce blood pressure, reverse target organ damage, and mitigate cardiovascular risk. Therefore, evaluating all patients with resistant hypertension for primary aldosteronism is crucial for improving their quality of life and cardiovascular profile.

Summary: This study aimed to investigate the association between asymmetric dimethylarginine (ADMA) and macrovascular endothelial function in patients with systemic sclerosis (SSc). The results suggest that ADMA may be associated with accelerated atherosclerosis in early stages of SSc, as evidenced by a significant positive correlation between ADMA levels and carotid intima-media thickness (cIMT) in individuals with early SSc.

Summary: This study aimed to assess the association between diabetic kidney disease (DKD) and sexual dysfunction (SD), including erectile dysfunction (ED) and female sexual dysfunction (FSD), in type 2 diabetes mellitus (T2DM) patients. The results showed that SD was common in older T2DM patients, with DKD affecting almost half of them. The eGFR was significantly associated with SD, ED, and FSD.
