Summary: This study assessed the impact of Fontan-associated liver disease (FALD) on post-transplant outcomes and compared single heart transplant (HT) with combined heart-liver transplant (CHLT) in adult Fontan patients. The results showed that the severity of FALD was associated with post-transplant mortality. Compared with HT alone, CHLT had better survival rates in patients with severe liver disease.

Summary: This study assessed the survival of lung transplant recipients from donors with abnormal kidney function and compared the lung recovery rates between donors with and without abnormal kidney function. The results showed that there was no significant difference in lung recipient survival. However, the lung recovery rate from donors with abnormal kidney function was lower compared to donors with normal kidney function.

Summary: The study reviewed the outcomes of neonatal heart transplants at Children's Hospital Colorado over nearly 30 years. It found an early survival rate of 95.2%, with 1- and 5-year survival rates of 85.7% and 75%, respectively. Long-term follow-up at 10 and 20 years showed survival rates of 72.2% and 50%.

Summary: This study retrospectively analyzed the impact of donor-recipient organ size matching on survival and graft rejection outcomes in combined heart-lung transplantation. The results showed that matching based on predicted heart mass ratio (pHM ratio) can reduce the risk of mortality associated with organ size mismatch.

Summary: This review summarizes the recent literature on the application of deep learning (DL) models in glaucoma detection and surveillance using posterior segment optical coherence tomography (OCT) imaging. DL models achieve high diagnostic accuracy in detecting glaucomatous optic neuropathy (GON) by using OCT derived parameters such as retinal nerve fiber layer (RNFL) scans, macular scans, and optic nerve head (ONH) scans, as well as a combination of these parameters. DL models offer time-efficient, objective, and potential options in the management of glaucoma.

Summary: This study compared the waitlist and post-transplant outcomes of liver transplantation alone (LTA) and simultaneous liver kidney (SLK) listings. It found that SLK patients had lower mortality on the waitlist and higher survival rates after transplantation. Therefore, the kidney function of patients should be taken into consideration when choosing the transplantation method.

Summary: This study examines the outcomes of lung transplant for patients with COVID-19-related acute respiratory distress syndrome or pulmonary fibrosis. The results show that the outcomes of lung transplant for COVID-19 patients are similar to those with other pretransplant causes of respiratory failure.

Summary: This study of heart/kidney transplant using HCV+ donors found that while there may be a potential increased risk of early postoperative renal dysfunction, the results suggest that HCV+ donors are an important source of transplantable organs due to similar survival rates and decreased waitlist time.

Summary: Post-cross clamp late allocation (LA) liver grafts are at risk of being discarded due to logistical complexity. This study found that outcomes of LA liver grafts are similar to those allocated through standard allocation (SA), despite longer cold ischemia time. Improving allocation policies for LA offers and sharing best practices between transplant centers and organ procurement organizations can help reduce unnecessary discards.

Summary: The study found that in the current era, ES-ASD or ES-VSD patients represented a decreasing proportion of all lung or heart-lung transplants compared to other indications. The odds of transplantation for ES-ASD and ES-VSD patients were significantly lower compared to other candidates for lung transplantation. In the early era, ES-ASD patients who underwent heart-lung transplants had equivalent survival rates to those who underwent lung transplants, while ES-VSD patients had superior survival rates.

Summary: This study examines the impact of place-based factors on clinical outcomes in lung transplant recipients. The findings suggest that higher social vulnerability is associated with a greater risk of lung rejection, while higher local life expectancy is associated with a lower likelihood of readmission. There is no association between residence and posttransplant survival.

Summary: The social determinants of health significantly impact outcomes, especially for children undergoing liver transplant, where those from disadvantaged neighborhoods experience higher morbidity and mortality risks. However, there is still a lack of understanding on how to address these social adversities effectively. More data on specific social risk factors and interventions are needed to improve outcomes for these children.

Summary: Intrahepatic cholangiocarcinoma is a rare malignancy of the biliary epithelium with a silent presentation that often leads to late diagnosis. While advances in medical management have been made, surgical resection still offers a survival advantage, and neoadjuvant and adjuvant treatments are broadening patient treatment options.

Summary: Intrahepatic cholangiocarcinoma (iCCA) has traditionally been considered a contraindication for liver transplantation (LT), but recent studies have shown positive outcomes for LT after neoadjuvant therapy. A center developed a protocol for neoadjuvant therapy and LT for locally advanced, unresectable iCCA patients in 2010. Patients who underwent LT had a high overall survival rate at 1, 3, and 5 years, with a higher than expected proportion of patients with genetic alterations in fibroblast growth factor receptor (FGFR) and DNA damage repair pathways. Further research is needed to determine criteria for LT in iCCA and factors predicting survival.

Summary: This study found that the differentiation of hepatocellular carcinoma and liver fibrosis synergistically determine the efficacy of resection and transplantation. Therefore, the current criteria for liver transplantation should incorporate tumor grade or liver fibrosis for optimal organ allocation.
