Impact of Donor Type in Patients with AML Given Allogeneic Hematopoietic Cell Transplantation After Low-Dose TBI-Based Regimen

Purpose: We assessed the impact of donor type in acute myeloid leukemia (AML) patients transplanted with 2 Gy total body irradiation (TBI)-based nonmyeloablative conditioning regimen. Patients and Methods: Data from 1,715 adult patients, with AML in CR1 or CR2 were included in this retrospective survey. Results: Donors consisted either of HLA-matched sibling donors (MSD, n = 701), 10/10 HLA-matched unrelated donors (MUD, n = 611), HLA-haplo-identical donors (haplo, n = 112) or single or double umbilical cord bloods (CBT, n = 291). Chronic graft-versus-host disease (GVHD) was less frequent in CBT (28%) and in haplo (30%) patients than in MSD (50%) and MUD (51%) recipients (P < 0.001). Two-year incidence of relapse was 32%, 30%, 34% and 34% in MSD, MUD, CBT and haplo patients, respectively (P = 0.7). Two-year overall (OS) and GVHD-free relapse free survival (GRFS) were 59% and 29% in MSD patients, 56% and 39% in CBT recipients, 53% and 23% in MUD recipients, and 43% and 37% in haplo patients, respectively. In multivariate analyses, MUD patients had lower GRFS than MSD patients beyond day 100 (HR 1.3, P = 0.001) while CBT was associated with a better GRFS than MSD beyond day 100 (HR 0.6, P = 0.002). Conclusions: In this large cohort of AML patients transplanted following low-dose TBI-based conditioning the relapse incidence was not affected by donor type suggesting that the intensity of GVL effects might be comparable with these four transplant approaches. Further, CBT was associated with better GRFS beyond day 100 than MSD while the opposite was observed for MUD. (C) 2018 AACR.

Summary: This study evaluated the safety and efficacy of a novel recombinant human interleukin-22 (IL-22) dimer in combination with systemic corticosteroids for the treatment of newly diagnosed gastrointestinal acute graft-versus-host disease (GVHD). Summary: HAPLO-SCT2 is a viable option for acute leukaemia relapse after HAPLO-SCT1, with comparable engraftment, toxicity, risk factors, and long-term outcome to allo-SCT2 in a matched donor setting. A retrospective registry analysis of 82 adult patients showed a 2-year overall survival/leukaemia-free survival of 34.3%/25.4% and a 2-year non-relapse mortality/relapse incidence of 17.6%/57%. The most frequent cause of death was leukaemia.

Summary: Allogeneic hematopoietic stem cell transplantation is the best option for treating higher-risk myelodysplastic syndrome. Patients with monosomal karyotype and/or complex karyotype abnormalities have lower survival rates after transplantation. This study compared outcomes in higher-risk MDS patients who received transplantation from HLA-matched sibling donors, HLA-matched unrelated donors, and HLA haploidentical donors. The results showed that the overall survival was better in the HLA-matched unrelated donor group.

Summary: Pre-transplant detectable measurable residual disease (MRD) is still associated with high risk of relapse and poor outcomes in acute myeloid leukemia (AML). The impact of disease burden on prediction of relapse and survival in patients receiving allogeneic hematopoietic cell transplantation (allo-HCT) in first remission (CR1) was evaluated. The study found that patients in CR1 and MRD positive at time of transplant could still be salvaged by allo-HCT, with significantly better outcomes than patients transplanted with active disease.

Cardiac toxicities in multiple myeloma: an updated and a deeper look into the effect of different medications and novel therapies

Summary: With the improvement in cancer treatment, including multiple myeloma, the risk of cardiovascular disease in patients has significantly increased, especially in elderly patients and those with additional risk factors. The risk factors can be patient-, disease-, and/or therapy-related, and they impact survival negatively. About 7.5% of multiple myeloma patients experience cardiovascular events, with varied risks depending on patient characteristics and treatment used. Cardiac toxicities have been associated with immunomodulatory drugs, proteasome inhibitors, and other agents, highlighting the importance of comprehensive cardiac evaluation and surveillance strategies for early detection and management.

Summary: Recent studies have shown that microtransplant (MST) could improve outcome of patients with elderly acute myeloid leukemia (EAML). To further standardize the MST therapy and improve outcomes in EAML patients, the International Microtransplant Interest Group provides recommendations and considerations for MST in the treatment of EAML, based on analysis of the literature on MST, especially MST with EAML from January 1st, 2011 to November 30th, 2022. The recommendations address key issues including therapeutic principle, candidate selection, chemotherapy regimens, post-remission therapy, and other aspects such as donor screening and complications.

Summary: When comparing the outcomes of using a haploidentical donor and a matched unrelated donor (MUD) for lymphoma patients receiving post-transplantation cyclophosphamide (PTCy) graft-versus-host disease (GVHD) prophylaxis, the study found that MUD had better results in terms of overall mortality, progression-free survival, nonrelapse mortality, platelet engraftment, acute grade 2-4 GVHD incidence, and chronic GVHD. However, there were no significant differences in terms of relapse and neutrophil engraftment. Therefore, if a MUD is available in a timely manner, it should be preferred over a haploidentical donor for lymphoma patients using PTCy-based GVHD prophylaxis.
