The purpose of this study was to assess the efficacy and safety of cetuximab plus platinum-based chemotherapy for patients specifically diagnosed with recurrent or metastatic oral squamous cell carcinoma (OSCC). We conducted a multicenter retrospective observational study of patients who underwent first-line cetuximab plus platinum-based chemotherapy between December 2012 and June 2015. 65 patients received weekly cetuximab (week 1, 400 mg/m(2); subsequent weeks, 250 mg/m(2)) plus a maximum of six 3-weekly cycles of cisplatin (80 or 100 mg/m(2), day 1) or carboplatin (at an area under the curve of 5 mg/mL/min as a 1-h intravenous infusion on day 1) and 5-fluorouracil (800 or 1000 mg/m(2)/day, days 1-4). Patients with stable disease who received cetuximab plus platinum-based chemotherapy continued to receive cetuximab until disease progression or unacceptable toxicities, whichever occurred first. The median follow-up was 10.5 (range 1.2-34.2) months. The best overall response and the disease control rates were 46.2 and 67.7%, respectively. The median overall survival and progression-free survival rates were 12.1 and 7.8 months, respectively. The most common grades 3-4 adverse events were skin rash (9.2%) followed by leukopenia (6.2%). None of the adverse events were fatal. The results of our multicenter retrospective study, which was the largest of its kind to date, suggest that first-line cetuximab plus platinum-based chemotherapy is suitable and well-tolerated for the systemic therapy of recurrent or metastatic OSCC.

Surgical treatment and prognosis of posteriorly invading oral cancer: Potential clinical significance of pterygomandibular raphe

Mitsunobu Otsuru, Souichi Yanamoto, Tomofumi Naruse, Keisuke Omori, Kota Morishita, Misa Sumi, Masahiro Umeda

Summary: This study investigated the usefulness of the pterygomandibular raphe (PMR) for diagnosing invasion and determining surgical methods in oral squamous cell carcinoma (OSCC) with posterior invasion. The results showed that OSCCs with invasion beyond the PMR had significantly higher recurrence rates and poorer survival rates. The initial surgery is crucial for local control.

Summary: This multicenter, retrospective study aimed to determine the appropriate resection margin for early tongue cancer and investigate the need for additional treatment for close margins and stump dysplasia. The study found that a horizontal margin of 3.3 mm was defined as a close margin. Close margins were significantly correlated with local recurrence but did not affect survival. Additional treatment was also found to be ineffective in patients with dysplasia at the surgical margin.

Oral Bacterial Contributions to Gingival Carcinogenesis and Progression

Kohei Okuyama, Souichi Yanamoto

Summary: Several periodontitis-associated species induce genes related to various biological processes, including cell proliferation, apoptosis, immune response, and carcinogenesis. A review of studies showed strong associations between gingival squamous cell carcinoma (GSCC) and three periodontitis-related oral bacteria (Porphyromonas gingivalis, Fusobacterium nucleatum, and Prevotella intermedia). These bacteria produce lipopolysaccharide (LPS) and secrete cytokines and molecules that are involved in carcinogenesis, tumor progression, and metastasis. The elevated levels of LPS in GSCC can be assessed through functional analysis. Polymicrobial infections can synergistically induce inflammation and upregulate proinflammatory molecules, promoting carcinogenesis. This review aids in identifying novel targets for the treatment and prevention of GSCC, which may be associated with periodontal pathogens.

Reconstruction using a facial artery musculomucosal flap technique after removal of necrotic bone in the anterior areas in medication-related osteonecrosis of the jaw: A technical note

Yoshinari Myoken, Takeshi Kawamoto, Yoshinori Fujita, Souichi Yanamoto

Summary: The study evaluated the use of facial artery musculomucosal (FAMM) flap for treating medication-related osteonecrosis of the jaw (MRONJ) in the anterior area. Successful wound healing was achieved in all patients with stage 2 or 3 MRONJ after 6 months of follow-up. The study suggests that FAMM flap is an effective surgical option for well-vascularized wound healing in these patients.

Summary: Salivary gland tumors are histologically diverse, and this report presents a rare case of primary secretory carcinoma of the minor salivary gland in the lip, with the ETV6-NTRK3 fusion gene. The patient underwent surgical treatment, and no recurrence or metastasis has been observed 13 months postoperatively.

Summary: This study aimed to explore the efficacy and safety of zinc acetate hydrate (ZAH) in patients with hypozincemia-induced dysgeusia. The study found that ZAH treatment significantly increased serum zinc levels, improved gustatory sensitivity, and did not affect serum copper levels, making it more effective than polaprezinc (PPZ).

Relationship between Tumor Budding and Partial Epithelial-Mesenchymal Transition in Head and Neck Cancer

Kohei Okuyama, Keiji Suzuki, Souichi Yanamoto

Summary: This article summarizes the latest research on the relevance of tumor budding (TB) and partial epithelial-mesenchymal transition (p-EMT) as prognostic markers in the tumor microenvironment (TME) of head and neck squamous cell carcinoma (HNSCC). Understanding the mechanism of TB onset and the relationship between p-EMTs may facilitate the development of novel diagnostic and prognostic methods, and targeted therapies for the prevention of invasion and metastasis.

Tumor-Infiltrating CD45RO+ Memory Cells Are Associated with Favorable Prognosis in Oral Squamous Cell Carcinoma Patients

Nanako Ito, Sachiko Yamasaki, Tomoaki Shintani, Kensaku Matsui, Fumitaka Obayashi, Koichi Koizumi, Ryouji Tani, Souichi Yanamoto, Tetsuji Okamoto

Summary: This study investigated the association of tumor-infiltrating lymphocytes (TILs) and MICA with oral squamous cell carcinoma (OSCC) and their potential as biomarkers. The results showed that a high ratio of CD45RO-expressing TILs was associated with improved disease-free/overall survival in OSCC patients, while a low ratio of CD45RO-expressing TILs was associated with poorer survival. Furthermore, the expression of MICA in tumors was related to the proportion of CD45RO-expressing TILs. Therefore, CD45RO-expressing TILs can serve as biomarkers for predicting the prognosis of OSCC.
