Summary: Surgery is the main potential cure for pancreatic ductal adenocarcinoma, with advancements in surgical techniques and multimodal approaches. Despite low survival rates, combining surgery with adjuvant therapy can increase five-year survival to around 20%. However, pancreatic resection is complex and high-risk, with ongoing developments in minimally-invasive and robotic approaches.

Summary: Pancreatic ductal adenocarcinoma (PDAC) is a deadly tumor characterized by a tumor microenvironment consisting of fibroblasts and immunosuppressive cells. Traditional therapies are limited by poor drug delivery efficiency, immunosuppressive tumor microenvironment, and adverse effects. Therefore, there is an urgent need for effective and safe treatments. In recent years, hydrogel-based therapies have been developed as potential therapeutic options for PDAC, offering controlled drug delivery with excellent biocompatibility and high drug load capacity.

Summary: This study compared the trends and outcomes of adjuvant therapy in patients with resectable PDAC who underwent upfront surgery. The study found that non-white race and residence in lower education attainment areas were associated with lower rates of adjuvant therapy, and increasing distance from the treating facility was also associated with lower odds of initiating adjuvant therapy.

Summary: Pancreatic ductal adenocarcinoma has poor clinical outcome and limited response to immunotherapy. Stromal fibroblasts play a dominant role in the tumor microenvironment and exhibit high transcriptional heterogeneity influenced by tumor proximity. Targeting tumor-proximal fibroblast subsets, along with HIF-1 alpha inhibition and immune stimulation, may offer a multimodal therapeutic approach for this disease.
