Non-Hodgkin lymphoma (NHL) comprises numerous biologically and clinically heterogeneous subtypes, with limited data examining the risk factors for these distinct disease entities. Many limitations exist when studying lymphoma epidemiology; therefore, until recently, little was known regarding the etiology of NHL subtypes. This review highlights the results of recent pooled analyses examining the risk factors for NHL subtypes. We outline the heterogeneity and commonality among the risk factors for NHL subtypes, with proposed subtype-specific as well as shared etiologic mechanisms. In addition, we describe how the study of lymphoma epidemiology may translate into prevention or therapeutic targeting as we continue to explore the complexities of lifestyle and genetic factors that impact lymphomagenesis.

Summary: The distribution of baseline disease can impact the prognosis of patients with secondary CNS DLBCL, with those having localized disease potentially benefiting from stem cell transplantation post-Radiation Therapy. Receipt of SCT, response after RT, and presence of localized disease were associated with improved overall survival and CNS-disease-free survival.

Summary: This study investigates the mechanisms driving resistance to treatment in mantle cell lymphoma (MCL) by analyzing the cellular and molecular heterogeneity within patients using single-cell sequencing. The researchers found temporal activation of cancer hallmark pathways and acquisition of 17q in refractory MCL, leading to upregulation of BIRC5/survivin expression. They demonstrate that targeting BIRC5 results in effective tumor inhibition in preclinical models and also identify changes in the tumor microenvironment in refractory MCL, including decreased CD8+ T cells and aberrant cell-to-cell communication networks.

Summary: The impact of obesity on lymphoma-specific survival varies among different subtypes of lymphoma, with obesity associated with shorter survival in follicular lymphoma (FL) patients but not in diffuse large B-cell lymphoma (DLBCL) and chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) patients. Further research is needed to explore the effects of weight control in FL patients post-diagnosis.

Summary: In the past two decades, the development of cancer drugs has shifted from cytotoxic chemotherapy to targeted treatment. This study analyzed patient-level data from phase I trials sponsored by the National Cancer Institute Cancer Therapy Evaluation Program to assess the trend of toxicity and response rates for patients with hematologic malignancies over time. The study found that the overall response and complete response rates improved significantly, while the rate of toxicity-related death remained stable.

Outcomes of breakthrough COVID-19 infections in patients with hematologic malignancies

Summary: Patients with hematologic malignancies who have been vaccinated against COVID-19 have lower hospitalization rates compared to those who have not been vaccinated, but their rates of COVID-19-associated death are similar. Female, older patients, and individuals with higher comorbidity scores are at higher risk of death from COVID-19 infections. Vaccination is associated with a decreased risk of hospitalization but not with a decreased risk of death from COVID-19 infections. Protective strategies are needed in addition to vaccination for this vulnerable patient population.

Real-world experience of CAR T-cell therapy in older patients with relapsed/refractory diffuse large B-cell lymphoma

Summary: This study analyzed the outcomes and costs of CAR T-cell therapy in 551 older patients (aged >= 65 years) with DLBCL. The results showed that CAR T-cell therapy was less used in older patients, especially those aged >= 75 years, and this age group had a lower rate of event-free survival. This highlights the need for more accessible, effective, and tolerable therapy in older patients.
