Summary: Insulinomas are rare hormone-producing pancreatic neoplasms. Most cases are diagnosed based on the Whipple triad. Nonmetastatic insulinomas are considered indolent and metastatic cases are considered aggressive. Surgical excision is the treatment of choice for solitary insulinomas, while debulking procedures and various medications can be used for aggressive cases.

Summary: This study aimed to investigate the relationship between the size and echogenicity of insulinoma and the results of a 72-hour fast and hormone levels. The study found no correlation between the size of the insulinoma and the duration of fasting or hormone levels. However, homogeneous tumors were significantly smaller and had a lower Ki-67 index, suggesting a higher differentiation level and a potential for a shorter fasting period.

Summary: This study explored the association between insulin resistance (IR) and ovarian sensitivity to exogenous gonadotropin (Gn) in women with polycystic ovary syndrome (PCOS). It was found that IR was negatively correlated with ovarian sensitivity. This provides new insights for individualized treatment in PCOS patients with IR undergoing assisted reproductive technology (ART).

Summary: Insulinomas are rare functioning neuroendocrine tumors, and some are associated with multiple endocrine neoplasia 1 (MEN1). Most tumors cause symptomatic hypoglycemia. Various techniques are used to localize the lesion. We present a case of insulinoma, initially negative on non-invasive imaging, diagnosed and localized with endoscopic ultrasound. A fine needle biopsy confirmed the diagnosis, and the patient underwent surgery.

Summary: Insulinomas are common functional neuroendocrine tumors, typically with a diameter under 3 cm. However, there have been rare cases of giant insulinomas exceeding 9 cm in diameter. This article reports a case of a 38-year-old woman with a large insulinoma, treated successfully with surgical excision. Follow-up showed no recurrence or metastasis. The etiology of giant insulinomas remains unclear, but further genetic analysis may shed light on this rare subtype of neuroendocrine pancreatic tumors.

Summary: Reactive postprandial hypoglycemia (RPH) is an understudied condition that lacks clinical definition and knowledge of future health implications. This study aimed to assess the glycemic response after glucose ingestion in individuals with RPH and compare it with healthy volunteers. The results showed that RPH subjects did not exhibit deteriorated insulin sensitivity or insulin resistance, and there were no significant differences in glucose-insulin curves during oral glucose tolerance tests compared to healthy controls.
