Summary: Supragastric belching (SGB) and rumination are behavioral disorders associated with proton pump inhibitor (PPI) non-response. This pilot study compared the diagnostic yield and inter-rater agreement for SGB and rumination using multichannel intraluminal impedance-pH (MII-pH) and post-prandial high-resolution impedance manometry (PPHRIM). Results showed that diagnostic accuracy and inter-rater agreement are higher for MII-pH than PPHRIM, and behavioral disorders are more frequently identified on PPHRIM. Clinical context is essential for accurate interpretation of the studies.

Diagnosis of SGB should be considered in patients with excessive belching and refractory reflux symptoms, and behavioral therapy can alleviate acid exposure and improve quality of life.

Summary: This study evaluated the effect of online cognitive behavior therapy (AF-CBT) on the quality of life (QoL) in patients with symptomatic paroxysmal atrial fibrillation (AF). The results showed that AF-CBT significantly improved AF-specific QoL and reduced healthcare consumption. These findings suggest that online CBT may be an important addition to AF management.

Summary: This retrospective cohort study investigates the symptom refractoriness of patients treated with proton pump inhibitors (PPIs) and suggests that polymorphism in the CYP2C19 gene may explain the differences in patient response to PPI treatment. The study found that poor metabolizers of CYP2C19 had a lower risk of abdominal pain and other related symptoms persistence/recurrence compared to normal metabolizers. Additionally, the study found no significant association between poor metabolizer status and the risk of fractures.

Summary: Belching disorders and rumination syndrome are gut-brain interaction disorders, with excessive gastric belching and supragastric belching being part of the former, and rumination being a behavioral disorder. Recent survey results showed higher prevalence rates for these disorders. Patients with excessive supragastric belching and rumination syndrome are often misdiagnosed as having refractory gastroesophageal reflux disease. Distinguishing between these conditions is crucial for proper treatment as psychological therapy, rather than acid suppressants, is needed. High-resolution impedance manometry and 24-hour impedance-pH monitoring can provide objective diagnoses. Behavioral therapy, specifically diaphragmatic breathing, is the recommended first-line treatment for these disorders. Overlapping with eating disorders and other gut-brain interaction disorders can complicate management.
