Comparative Study of C-arms for Intraoperative 3-dimensional Imaging and Navigation in Minimally Invasive Spine Surgery Part I Applicability and Image Quality

Study Design: This was a retrospective analysis. Objective: This study compares 2 different 3-dimensional (3D) C-arm devices for intraoperative imaging and navigation with regard to clinical applicability and image quality. Summary of Background Data: Minimally invasive spine surgery requires intraoperative imaging techniques to adequately visualize the unexposed spine. For this purpose, mobile 3D C-arms became available along with the evolution of intraoperative navigation techniques. Methods: The C-arm devices Siremobil Iso-C 3D (Siemens) and Vision FD Vario 3D (Ziehm) perform an automated orbital rotation around the patient acquiring a 3D image set out of multiple successive fluoroscopic images. We report on technical specifications of the C-arms and our daily experience regarding clinical applicability. Furthermore, 5 spine surgeons evaluated blinded triplanar planes of 40 cervical, thoracic, and lumbar 3D scans that were obtained during routine surgery regarding usability for navigation. We assessed the delineation of cortical bone, artifacts, and overall image quality using a 0-10 numeric rating scale. Results: The Siremobil Iso-C 3D requires 128 seconds for its 190-degree scanning arc with equidistant isocenter. The Vision FD Vario 3D performs an elliptical scanning arc and completes its 135-degree scan in 64 seconds; furthermore, it features a flat panel detector and fully digital imaging. The smaller dimensions of the Vision FD Vario 3D made it easier to maneuver in the operating room compared with the more bulky Siremobil Iso-C 3D. With respect to image quality in cervical 3D scans, the Siremobil Iso-C 3D reached significantly higher scores in all categories. The Vision FD Vario 3D revealed less artifacts in lumbar 3D scans. Conclusions: The Siremobil Iso-C 3D provides high-quality 3D scans in slender spine regions (eg, cervical spine), whereas the Vision FD Vario 3D appears to have advantages in the lumbar spine. Further evolution and novel devices are needed to optimize image quality and handling.

Summary: Hippocampal sclerosis with gray-white matter blurring in the anterior temporal lobe is associated with prolonged T1 relaxation and axonal loss. Patients without GMB in the temporal poles showed less reduction in axonal fraction on imaging compared to normal controls. Contralateral values did not differ significantly between patients and normal controls, but reduced axonal density and diameter were confirmed in the GMB-affected temporopolar white matter compared to temporal poles without GMB.

Summary: This prospective study evaluated a protocol for radiation-sparing kyphoplasty by assessing dosemetrically recorded radiation exposures to both patient and surgeon. The results showed that the implementation of a strict intraoperative radiation protection protocol allows for safely performed kyphoplasty with ultra-low radiation exposure for the patient and surgeon without exceeding the annual occupational dose limits.

Mastication after craniotomy: pilot assessment of postoperative oral health-related quality of life

Summary: Patients undergoing neurosurgical cranial approaches often experience postoperative pain, reduced mouth opening, and masticatory problems due to manipulation of the temporal muscle. A study using the Oral Health Impact Profile (OHIP14) questionnaire found that postoperative oral health-related quality of life scores increase significantly after 3 months, decrease after 15 months, and do not return to preoperative levels. Postoperative impairment in mouth opening and masticatory pain can cause feedback from patients and their dentists.

Summary: The manual measurement technique of spinal sagittal alignment on X-rays is susceptible to rater-dependent variability. This study found that different raters had different significance levels when evaluating the pre- to postoperative change of lumbar lordosis in patients receiving lumbar spondylodesis. Therefore, it is important to describe the evaluation process and validate the consistency of results through multiple raters in every publication.

A Retrospective Evaluation of the Retrosigmoidal Approach for Petroclival Meningioma Surgery and Prognostic Factors Affecting Clinical Outcome

Summary: This study evaluated the prognostic factors influencing the progression-free survival rates of patients with petroclival meningioma. The extent of surgical resection and postoperative radiotherapy were found to be significantly associated with improved PFS. In cases of extended petroclival meningioma with infiltration of the cavernous sinus, near total resection and additional radiotherapy also showed positive effects. Surgical resection remains the most important prognostic factor, but neurological morbidity should be considered.

Summary: This study aimed to quantitatively analyze the T2 signal intensity of the spinal cord in degenerative cervical myelopathy (DCM) patients using high-resolution MRI segmentation. The results showed significantly increased signal variability in DCM patients compared to healthy volunteers, and the new analysis method demonstrated sufficient diagnostic accuracy.
