Summary: This study examined the effect of neck-specific exercises delivered via the internet (NSEIT) compared to exercises supervised by a physiotherapist (NSE) in chronic whiplash-associated disorders (WADs). The results showed that NSEIT was noninferior to NSE and required less physiotherapist time.

Summary: The study found that patients with nonspecific chronic neck pain (NSNP) have increased pronociceptive and impaired antinociceptive mechanisms, contributing to long-lasting pain and treatment failures. However, the observational nature of the study prevents establishing a clear cause-effect relationship. Normal PPT values should not be interpreted as absence of altered nociceptive processing in the clinic environment.

Summary: The association between Modic changes and lower back pain has been uncertain in previous studies and reviews, but clinical trials have shown that nerve ablation can significantly reduce pain and disability in patients with chronic lower back pain and Modic changes. This review explores the conflicting literature.

Summary: The study compared the effects of NSE, NSEB, and PPA on neck muscle endurance, active cervical range of motion, grip strength, and pain intensity in individuals with persistent WAD. NSE and NSEB showed greater improvements than PPA, with NSE and NSEB resulting in decreased disability at the 12-month follow-up in both high and low neck-related function individuals, while PPA led to increased disability for those with low neck-related function.

Summary: This study revealed altered neural responses in individuals with whiplash-associated disorders when imagining neck-specific movements. These patients may overpredict and amplify the sensory and affective consequences of the movements, leading to dysfunctional affective and/or behavioral responses. The findings provide new insights into the neural mechanisms underlying chronic pain in people with whiplash-associated disorders.

Summary: Tension-type headache and non-specific low back pain share similar clinical features, including tender points and areas, abnormal activation patterns observed in electromyography, and discoordinated motor control of affected muscles and adjacent muscle groups. These common features suggest a potential analogous pathophysiology involving the neuromotor control in the cervical and lumbar regions.

Summary: This study evaluated the effectiveness of a risk-stratified clinical pathway of care for acute whiplash injury. The results showed that risk-stratified care did not improve patient outcomes, and the current pathway is not recommended.

Summary: This study conducted in a Norwegian hospital observed 270 outpatient surgery patients and found that only 35% of them took pain relief medication at home 1-2 days after discharge. The use of pain relief medication was associated with factors such as chronic pain, pre- and post-surgical pain severity, and the ability to cope with pain. However, the low levels of postoperative pain did not lead to widespread chronic opioid use.

Summary: This study synthesized the evidence on the predictive ability of measures of physical function of the neck region and perceived function on prognosis after a whiplash injury. Higher pain-related disability and WAD grade were associated with poor outcome, while the predictive ability of other measures of neck physical function was inconclusive. The study found a lack of assessment of the predictive ability of more contemporary measures of neck function.

Summary: This study evaluated the use of positron emission tomography and computed tomography to detect peripheral musculoskeletal injuries and inflammation in acute whiplash injury and their correlation with pain and disability. The results showed possible tissue injuries after acute whiplash and a correlation between the injuries and subjective pain and disability. Some patients recovered during follow-up, while others experienced persistent symptoms and correlated reductions in pain levels with scan results.

Summary: Back pain is highly associated with mood and anxiety symptoms and syndromes, but the underlying mechanisms are still unclear. This study found a familial aggregation of back/neck pain with Major Depressive Disorder (MDD), but not with bipolar disorder. Onset of back/neck pain occurred earlier in individuals with bipolar disorder compared to controls.

Summary: This study investigated whether baseline pain extent is an independent predictor of pain and disability in people with chronic WAD one to two years later. The findings showed a significant association between pain extent and disability at the 1-year and 2-year follow-ups, but this association was no longer significant after controlling for other factors.

Summary: The pharmacological management of nonspecific chronic low back pain aims to improve daily activities and enhance quality of life, but the long-term results are unpredictable. Education needs to be improved to prevent non-evidence-based solutions from being promoted. The management of NCLBP is diverse and often leads to residual symptoms. Depending on the severity, pharmacological management can range from nonopioid to opioid analgesics.

Summary: The aim of this study was to identify 5-week pain intensity trajectories in children/young people with cerebral palsy (CP) and examine their association with physical and psychological well-being. The results showed that membership in higher pain intensity trajectories was associated with lower levels of physical and psychological well-being.

Summary: This study aimed to describe the clinical course of pain intensity in individuals with cerebral palsy (CP) receiving usual care or specific interventions. The findings showed that children and adults receiving usual care had stable pain intensity over time, while interventions (such as continuous intrathecal baclofen and botulinum neurotoxin A injections) led to a significant reduction in pain intensity.

Summary: This study aimed to assess cultural competency among Canadian chiropractors and understand their perspectives on providing services to equity-seeking communities. A cross-sectional survey of Canadian Chiropractic Association members was conducted, revealing that most chiropractors observed cultural health disparities and identified cost and language as barriers to care. The study highlights a gap between knowledge and behavior and identifies barriers and challenges that may inform profession-specific training in cultural competence.

Summary: The study aims to develop a rehabilitation program for degenerative cervical radiculopathy (DCR) based on the perspectives of those suffering from the condition. Through qualitative interviews, the study found that patients desire a patient-centered approach, validating and reassuring healthcare providers, easier access to healthcare services, a supportive team environment, and peer support. The ideal rehabilitation program can be conceptualized using the enactive-biopsychosocial model.

Summary: This study aimed to develop clinical prediction models to predict the recovery and insurance claim closure of patients with neck pain and associated disorders caused or aggravated by a traffic collision. The results showed that prior neck injury claim, expectation of recovery, age, percentage of body in pain, disability, neck pain intensity, and headache intensity were important predictors for recovery and claim closure.

Summary: This study estimated the minimal important difference (MID) of WHODAS-12 in individuals with chronic low back pain. The MID was found to be -3.22 (95% CI -4.79 to -1.64) or -5.99 (95% CI -7.20 to -4.79). These findings contribute to the assessment of whether rehabilitation or other health services achieve meaningful functional improvements for patients with chronic low back pain.
