Drug Utilization on Neonatal Wards: A Systematic Review of Observational Studies

Despite limited evidence on safety and efficacy of drug use in neonates, drugs are extensively used in this age group. However, the availability of information on drug consumption in neonates, especially inpatient neonates, is limited. This paper systematically reviews published studies on drug utilization in hospitalized neonates. A systematic literature review was carried out to identify observational studies published from inception of databases used till August 2016. Four search engines, namely Medline, CINAHL, Embase, and PubMed, were used. Publications written in English that described drug utilization in neonatal wards were selected. Assessment of the data was based on the category of the study design, the objective of study and the method used in reporting drug consumption. A total of 20 drug utilization studies were identified, 12 of which focused on all drug classes, while the other eight evaluated antimicrobials. Studies were reported in Europe (n = 7), the United States (n = 6), India (n = 5), Brazil (n = 1), and Iran (n = 1). Substantial variance with regard to study types (study design and methods), data source, and sample size were found among the selected studies. Of the studies included, 45% were cross-sectional or retrospective, 40% were prospective studies, and the remaining 15% were point prevalence surveys. More than 70% of the studies were descriptive studies, describing drug consumption patterns. Fifteen per cent of the descriptive studies evaluated changes in drug utilization patterns in neonates. Volume of units was the most prevalent method used for reporting all drug categories. The ATC/DDD system for reporting drug use was only seen in studies evaluating antimicrobials. The most commonly reported drugs across all studies are anti-infectives for systemic use, followed by drugs for the cardiovascular system, the nervous system and the respiratory system. Ampicillin and gentamicin were the most prescribed antimicrobials in hospitalized neonates. The present review reveals that neonates are exposed to a high number of drugs and various methods are used to report drug consumption in this age group. The best measure of drug consumption to quantify prevalence of drug use in neonates remains to be identified and additional research in this area is warranted.

Meta-Analysis and Systematic Review of HLA DQ2/DQ8 in Adults with Celiac Disease

Summary: Although HLA DQ2 and/or DQ8 genes increase the risk of developing celiac disease (CD), other factors such as genetics, the environment, and the immune system also play a role in the genesis of the disease. A comprehensive and meta-analytical study of adult CD patients was conducted to gain insight into the assessment and distribution of HLA class II (HLA-DQ2/DQ8) and explore screening techniques. DQ2/DQ2 homozygotes have the highest risk of developing CD, and DQ2/DQ8 typing is an effective test for differential diagnosis. The inclusion of non-HLA genes in diagnosis is limited due to their low contribution to the heritability of CD.

Summary: Antimicrobial drug resistance poses a serious threat to global public health, and the discovery of new antibacterial agents is urgently needed. Quinoline derivatives have shown a range of pharmacological properties, including potential antibacterial effects. This study evaluated the binding affinity of three newly designed hydroxyquinoline compounds to multiple bacterial protein targets, and identified two compounds that may serve as new antibiotic scaffolds.

Summary: The aim of this study was to synthesize carbohydrazide derivatives as DPP-IV inhibitors and evaluate their antidiabetic activity. Molecular docking, ADMET analysis, and drug-likeness guidelines were used for simulations. The synthesized compounds showed excellent hypoglycemic potential both in vitro and in vivo, and could be used as leads in developing novel antidiabetic agents.

Summary: Thiazole and oxazole analogues of diaminopimelic acid (DAP) showed significant antibacterial efficacy, with MICs of 70-80 μg/mL against all tested bacteria. The docking scores revealed that the synthesized compounds interact more precisely with dapF than LL-DAP, suggesting their potential as inhibitors of bacterial cell wall synthesis. The structure-based drug design approach was used to synthesize these compounds.

Diabetes Mellitus and Its Therapy in Brunei Darussalam-A Critical Revisit

Summary: This study aimed to identify and review research related to the practice of patients with DM in Brunei Darussalam in order to identify associated factors influencing diabetic medication knowledge and adherence. The results revealed relatively poor medication knowledge and inappropriate diabetes-related practices in studies conducted in Brunei Darussalam. However, no study has directly measured both medication awareness and compliance among Brunei Darussalam T2DM patients. Future studies can focus on the relation between these two factors in different geographical locations characterized by different diabetes-related practices and culture.

Drug-related problems among older people with dementia: A systematic review

Summary: This systematic review examined drug-related problems (DRPs) among patients with dementia or cognitive impairments, specifically focusing on medication misadventures such as adverse drug reactions (ADRs) and inappropriate drug use. The findings indicate a high prevalence of DRPs among dementia patients, especially older individuals. Medication misadventures, ADRs, and potentially inappropriate medication use were identified as the most common types of DRPs among older people with dementia.

Knowledge, attitude and perceptions of pharmacists regarding renal dose adjustment among chronic kidney disease patients in Pakistan

The study highlights the importance of targeted training programs for pharmacists, access to reliable resources, and promoting research and dissemination of results in renal pharmacotherapy to improve public health outcomes.
