Periprocedural anticoagulation in the uninterrupted edoxaban vs. vitamin K antagonists for ablation of atrial fibrillation (ELIMINATE-AF) trial

Summary: This post hoc analysis of ELIMINATE-AF evaluated the requirements of unfractionated heparin (UFH) and procedure-related bleeding in atrial fibrillation (AF) patients undergoing ablation with uninterrupted edoxaban or vitamin K antagonist (VKA) therapy. The study found that despite higher doses of UFH used with edoxaban vs. VKA to achieve a target ACT during AF ablation, the rate of procedure-related major/clinically relevant non-major bleeding did not differ between the treatment arms.

More trials are needed to address the safety and efficacy of optimal anticoagulant therapy management prior to ablation.

Safety and effectiveness of dabigatran in routine clinical practice: the RE-COVERY DVT/PE study

Summary: RE-COVERY DVT/PE is an international observational study comparing the safety and effectiveness of dabigatran and vitamin K antagonists in patients with deep vein thrombosis and/or pulmonary embolism. The study found that dabigatran had lower rates of bleeding events compared to vitamin K antagonists, but similar rates of symptomatic venous thromboembolism or related mortality. The results of this observational study with 1-year follow-up were consistent with randomized clinical trials.

Summary: This study compared the clinical effects and economic costs of warfarin and novel oral anticoagulants in elderly patients with atrial fibrillation. The results showed that compared with warfarin, dabigatran etexilate and rivaroxaban not only have the ability to inhibit myocardial ischemia indicators and improve left ventricular diastolic function while reducing the incidence of adverse events, but they also offer certain cost-effectiveness advantages for elderly patients with atrial fibrillation.

Summary: This study compared the effectiveness, safety, and mortality between direct oral anticoagulants (DOACs) and conventional pharmacologic venous thromboembolism (VTE) prophylaxis in patients undergoing hip fracture surgery (HFS). The meta-analysis found no significant difference in VTE, deep vein thrombosis, pulmonary embolism, major bleeding, minor bleeding, all bleeding events, and mortality between patients prescribed DOACs and patients prescribed low molecular weight heparin (LMWH). Further studies with larger sample sizes are needed to obtain safer conclusions.

Summary: This study evaluated the prothrombotic response after ventricular tachycardia (VT) ablation in patients with and without oral anticoagulation (OAC). Findings showed that uninterrupted OAC administration suppressed the coagulation response, which may reduce perioperative prothrombotic risk. OAC administration and normal coagulation markers at baseline were independent predictors of stable coagulation status after ablation.

Catheter Ablation Energy Sources and Myocardial Injury and Coagulation Biomarkers During Uninterrupted Periprocedural Edoxaban Use-A Subanalysis of KYU-RABLE

Summary: This study compared the effects of radiofrequency energy and cryoballoon on myocardial injury and coagulation biomarkers during catheter ablation in Japanese non-valvular atrial fibrillation patients. The results showed that both energy sources led to an increase in biomarkers after the procedure, with cryoballoon resulting in a larger increase. However, there was no significant difference in the incidence of thrombotic or bleeding events between the two groups. The findings suggest that uninterrupted edoxaban is equally effective regardless of the energy source used during catheter ablation, but caution should be exercised in selecting periprocedural anticoagulation, especially with cryoballoon.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Coronary Angiography and Chronic Total Occlusion Angioplasty in a Patient With Situs Inversus from the Right Radial Approach

Summary: This article presents a case of a patient with known situs inversus who underwent cardiac catheterization and was found to have chronic total occlusion in the right coronary artery. Situs inversus is a rare congenital abnormality characterized by the inverted position of chest and abdominal organs. Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Evaluation of MADIT-II Risk Stratification Score Among Nationwide Registry of Heart Failure Patients With Primary Prevention Implantable Cardiac Defibrillators or Resynchronization Therapy Devices

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.

Summary: Using latent class analysis (LCA), the study identified clinical, demographic, and social subphenotypes in ASCVD population and assessed the risk of all-cause and cardiovascular mortality across different socioclinical classes. The results showed that the younger, female, non-Hispanic Black or Hispanic individuals with a high burden of co-morbidities and unfavorable social determinants of health had the highest risk of mortality in the identified latent classes.

Summary: This study aimed to examine the relation between reverse septal curvature (RSC) and nonsustained ventricular tachycardia (NSVT) in patients with hypertrophic cardiomyopathy (HCM). The study found that patients with RSC were younger and had a higher incidence of NSVT, and RSC was identified as a strong independent risk factor for NSVT.
