The aim of this longitudinal observational study was to evaluate the oral health-related quality of life (OHRQoL) following patient rehabilitation with implant-retained mandibular overdentures (IMO) and to identify the contribution of the different domains to OHRQoL. The Oral Health Impact Profile (OHIP-EDENT), Dental Impact on Daily Living (DIDL), and Geriatric Oral Health Assessment Index (GOHAI) questionnaires were completed twice by 25 patients: after 3 months of rehabilitation with complete dentures (CD) and after 3 months of IMO loading using stud abutments. The evaluation after IMO rehabilitation showed significant improvement in three DIDL domains: appearance (p = 0.011), eating and chewing (p = 0.003), and general performance (p = 0.003). The GOHAI results showed significant differences in two domains: psychosocial (p = 0.005) and pain and discomfort (p = 0.0004). The OHIP-EDENT outcomes showed significant improvements in five domains: functional limitation (p = 0.0001), physical pain (p = 0.0002), physical disability (p = 0.0010), and psychological disability and handicap (p = 0.032). The largest observed effect sizes were close to one standard deviation and were observed in the eating and chewing domain (0.93) of the DIDL; the pain and discomfort domain (0.83) of the GOHAI, and the functional limitation (0.89), physical pain (1.02), physical disability (0.84) domains of the OHIP-EDENT. The percentage of satisfied patients increased in all domains. Self-reported OHRQoL of CD wearers was significantly improved after 3 months of treatment with IMO, especially concerning the functional and pain-related aspects.

Reporting of absolute and relative risk measures in oral health and cardiovascular events studies: A systematic review

Eero Raittio, Javed Ashraf, Julie Farmer, Gustavo G. Nascimento, Musfer Aldossri

Summary: The reporting practices in literature tend to focus more on relative risks rather than absolute risks. Most studies rarely report similarly adjusted underlying absolute risks, which could lead to an overinterpretation of the risk increase of cardiovascular diseases (CVD) related to poor oral health.

Effect of Smoking Exposure on Nonsurgical Periodontal Therapy: 1-Year Follow-up

F. R. M. Leite, R. Lopez, J. B. Pajaniaye, G. G. Nascimento

Summary: This study investigated the influence of different levels of smoking on periodontal healing and found that heavy smokers had worse periodontal parameters compared to light smokers/quitters and moderate smokers after nonsurgical periodontal therapy. Therefore, smoking cessation should be included in periodontal therapy to achieve optimal treatment outcomes.

Summary: This study aimed to describe cytokine profiles in periodontal healing after periodontal therapy in smokers. The results suggest that different smoking trajectories have distinct effects on cytokine expression, which in turn influence periodontal repair.

The Role of Dyslipidemia in Periodontitis

Fernando Valentim Bitencourt, Gustavo G. G. Nascimento, Susilena Arouche Costa, Silvana Regina Orrico Perez, Cecilia Claudia Costa Ribeiro, Fabio Renato Manzolli Leite

Summary: Studies have found that dyslipidemia plays an important role in promoting periodontal breakdown. This study aimed to explore the theoretical pathways between dyslipidemia and periodontitis. Data from 11,917 US adults were analyzed using structural equation modeling (SEM). The results showed that dyslipidemia is associated with periodontitis directly and indirectly through HbA1c and obesity. These findings highlight the need for a multi-professional approach to address the common risk factors of oral and noncommunicable diseases.

Intake and serum levels of micronutrients and chronic oral diseases burden

Susilena Arouche Costa, Gustavo G. Nascimento, Fabio R. M. Leite, Cecilia Claudia Costa Ribeiro, Soraia de Fatima Carvalho Souza

Summary: This study investigated the pathways from micronutrient intake and serum levels to Chronic Oral Diseases Burden using data from NHANES III (n = 7936) and NHANES 2011-2014 (n = 4929). The results showed that higher micronutrient intake and higher vitamin D serum levels were associated with a lower burden of chronic oral diseases. These findings suggest that healthy diet policies can contribute to the prevention and reduction of caries, periodontitis, obesity, and other non-communicable diseases.

Validity of self-reported oral conditions among Brazilian older women: Do socio-economic factors matter?

Gustavo G. Nascimento, Fernanda W. Machado, Andreia M. Cascaes, Alexandre E. Silva, Noeli Boscato, Flavio F. Demarco

Summary: The study aimed to validate the agreement between self-report and clinical examination of oral conditions and evaluate the influence of sociodemographic factors on the accuracy of self-reporting among women aged 60 and older. Results showed high levels of agreement in the self-reporting of edentulism and the use of dental prostheses among older women. However, there were differences observed, particularly among women with lower income and education levels.

Breastfeeding Protects from Overjet in Adolescence by Reducing Pacifier Use: A Birth Cohort Study

Carlos Alberto Feldens, Laura Boianovsky Petracco, Gustavo G. Nascimento, Huihua Li, Marcia Regina Vitolo, Karen Glazer Peres

Summary: Increased dental overjet in adolescence is a clinically relevant outcome associated with the complexity and high cost of treatment. Breastfeeding has a protective effect on dental overjet, with 63.1% of the effect mediated by pacifier use. However, the direct effect of breastfeeding alone on dental overjet was not statistically significant.

Summary: Periodontitis has been increasing in Brazil since 2005, despite a reduction in smoking. High consumption of sugar-sweetened beverages is closely associated with the prevalence of periodontitis. Additionally, overweight/obesity, dyslipidemia, and hyperglycemia are also contributing factors to periodontitis prevalence in Brazilian adults.

Advancing Universal Oral Health Coverage via Person- Centred Outcomes

Gustavo G. Nascimento, Eero Raittio, Vanessa Machado, Fabio R. M. Leite, Joao Botelho

Summary: The World Health Organization member states have proposed a comprehensive Global Strategy on Oral Health in order to achieve universal oral health coverage by 2030. However, challenges and barriers, such as persistent inequalities, pose obstacles to achieving this goal. In low- and middle-income countries, oral health issues have been neglected, leading to increased inequalities. Conversely, in high-income countries, there is excessive dental treatment for some individuals while those with greater needs receive inadequate dental care. It is necessary to analyze the specific needs of individual countries, train oral health professionals with a new philosophy of care, and optimize existing resources. Moving away from a person-centered focus has resulted in individual and societal problems, including under-/overdiagnosis and under-/overtreatment. The person-centered approach takes into account the perceptions, needs, preferences, and circumstances of individuals and populations. The use of patient-reported outcome measures in dentistry to prioritize the individual in treatment lags behind other areas. This paper discusses the challenges and potential solutions of patient-reported outcome measures in dentistry for achieving universal oral health coverage.

Effect of mandibular bone atrophy on maxillary and mandibular bone remodeling and quality of life with an implant-retained mandibular overdenture after 3 years

Alessandra Julie Schuster, Anna Paula da Rosa Possebon, Andre Ribeiro Schinestsck, Otacilio Luiz Chagas-Junior, Fernanda Faot

Summary: This study evaluated the 3-year effects of implant-retained mandibular overdentures on oral-health-related quality of life (OHRQoL) and bone remodeling. Differences between atrophic and nonatrophic patients were confirmed in terms of bone loss, functional limitation, and handicap domains. Nonatrophic patients showed significant bone resorption after 3 years, while atrophic patients showed improvement in bone area. This study suggests that bone atrophy has an impact on OHRQoL and bone remodeling.
