OBJECTIVE To assess the potential association between prenatal use of antidepressants and the risk of attentiondeficit/hyperactivity disorder (ADHD) in offspring. DESIGN Population based cohort study. SETTING Data from the Hong Kong population based electronic medical records on the Clinical Data Analysis and Reporting System. PARTICIPANTS 190 618 children born in Hong Kong public hospitals between January 2001 and December 2009 and followed-up to December 2015. MAIN OUTCOME MEASURE Hazard ratio of maternal antidepressant use during pregnancy and ADHD in children aged 6 to 14 years, with an average follow-up time of 9.3 years (range 7.4-11.0 years). RESULTS Among 190 618 children, 1252 had a mother who used prenatal antidepressants. 5659 children (3.0%) were given a diagnosis of ADHD or received treatment for ADHD. The crude hazard ratio of maternal antidepressant use during pregnancy was 2.26 (P<0.01) compared with non-use. After adjustment for potential confounding factors, including maternal psychiatric disorders and use of other psychiatric drugs, the adjusted hazard ratio was reduced to 1.39 (95% confidence interval 1.07 to 1.82, P=0.01). Likewise, similar results were observed when comparing children of mothers who had used antidepressants before pregnancy with those who were never users (1.76, 1.36 to 2.30, P<0.01). The risk of ADHD in the children of mothers with psychiatric disorders was higher compared with the children of mothers without psychiatric disorders even if the mothers had never used antidepressants (1.84, 1.54 to 2.18, P<0.01). All sensitivity analyses yielded similar results. Sibling matched analysis identified no significant difference in risk of ADHD in siblings exposed to antidepressants during gestation and those not exposed during gestation (0.54, 0.17 to 1.74, P=0.30). CONCLUSIONS The findings suggest that the association between prenatal use of antidepressants and risk of ADHD in offspring can be partially explained by confounding by indication of antidepressants. If there is a causal association, the size of the effect is probably smaller than that reported previously.

Association between the pharmacological treatment of bipolar disorder and risk of traumatic injuries: a self-controlled case series study

Vanessa W. S. Ng, Le Gao, Esther W. Chan, Ho Ming Edwin Lee, Joseph F. Hayes, David P. J. Osborn, Timothy H. Rainer, Kenneth K. C. Man, Ian C. K. Wong

Summary: This study examined the association between pharmacological treatment and the risk of emergency room admissions due to traumatic injuries among patients with bipolar disorder. The analysis found that there was an increased risk of traumatic injuries before treatment, but the risk decreased after treatment initiation and returned to baseline during maintenance treatment. However, the risk increased again after treatment cessation.

Polypharmacy and antidepressant acceptability in comorbid depression and type 2 diabetes: a cohort study using UK primary care data

Summary: This study investigates the association between the number of prescribed medications and early discontinuation of antidepressant treatment in adults with type 2 diabetes. The findings suggest that individuals with higher levels of concurrent polypharmacy are more adherent to treatment, while individuals with lower levels of concurrent polypharmacy may benefit from adherence support.

Summary: Polypharmacy among older people represents a global challenge, with higher prevalence in Asian countries such as Hong Kong, Taiwan, and South Korea. Australia and the United Kingdom have shown decreasing trends in polypharmacy. Monitoring and optimizing medication treatment for older people in Asia is necessary.

Treatment with methylphenidate and the risk of fractures among children and young people: A systematic review and self-controlled case series study

Le Gao, Kenneth K. C. Man, Min Fan, Grace M. Q. Ge, Wallis C. Y. Lau, Ching-Lung Cheung, David Coghill, Patrick Ip, Kirstie H. T. W. Wong, Ian C. K. Wong

Summary: Animal studies show that methylphenidate treatment for around 3 months may lead to less mineralized and weaker appendicular bones. A systematic review and a self-controlled case series study were conducted to assess the risk of fractures before and after treatment initiation. The findings suggest that methylphenidate treatment may lower the risk of all-cause fractures, but more research is needed to investigate the effects of treatment duration and sex.

Summary: This study evaluated the trends and patterns of ADHD medication consumption across multiple countries. The results showed an overall increase in ADHD medication consumption globally, particularly in high-income countries. However, most middle-income countries had lower rates of ADHD medication consumption, indicating the need to address barriers to diagnosis and treatment.

Development and validation of sex-specific hip fracture prediction models using electronic health records: a retrospective, population-based cohort study

Summary: Sex-specific hip fracture prediction models were developed using electronic health records (EHR) without bone mineral density (BMD) data. The logistic regression (LR) model showed better discrimination and classification performance than machine learning (ML) algorithms in both internal and independent validation cohorts for both females and males. The LR models could be integrated into routine clinical workflow to identify high-risk individuals for DXA scan.

Summary: This study systematically evaluated the efficacy and effectiveness of inactivated vaccines against SARS-CoV-2 and found evidence of their efficacy and effectiveness for various outcomes. However, inconsistent reporting of key parameters, high heterogeneity in observational studies, and limitations in study design for certain outcomes undermine the reliability of the findings, highlighting the need for further research to address these limitations.
