Screening for oral cancer should be defined as the application of a test to people who are apparently free of disease to identify those who may have oral cancer and to distinguish them from those who may not. The aim of the test is not to be diagnostic but to identify changes that may be the earliest signs of impending disease. Defined in this way, screening is an ongoing public health measure, often funded by governments. A screening program must do no harm and must be cost effective. Governments demand that strict evidence of benefits and cost effectiveness be met before a program may be implemented. Although many studies have investigated the utility of potential screening tests, there have been few evaluations of screening programs and only one randomized controlled trial. Systematic reviews have concluded that there is insufficient evidence to show that oral cancer screening can reduce mortality from oral cancer, and to date, no country has implemented a formal oral cancer screening program. This paper reviews this evidence and tries to identify the barriers to screening and suggests areas of focus for future research.

Summary: This study analyzed a large case series of pediatric patients with salivary gland tumors from Brazil, South Africa, and the United Kingdom. The results showed that these tumors were more common in females with a mean age of 14.1 years. The most common site was the palate, and the most frequently diagnosed tumors were pleomorphic adenoma and mucoepidermoid carcinoma. Surgery was the favored treatment option.

Summary: Primary effusion lymphoma (PEL) is an aggressive neoplasm associated with HHV8 infection and Epstein-Barr virus. Extracavitary PEL is rare and the occurrence in the oral cavity is even rarer. This report highlights the importance of searching for HHV8 infection in oral lymphomas with plasmablastic features.

Cell block preparation as an adjunctive tool after fine-needle aspiration cytology for screening oral and maxillofacial diseases

Summary: This study retrospectively analyzed cell block preparation from fine-needle aspiration cytology of the oral and maxillofacial region. The results showed that cell block preparation had a sensitivity of 70.0%, specificity of 100.0%, positive predictive value of 100.0%, negative predictive value of 62.5%, and accuracy of 80.0% for predicting malignant lesions in the oral and maxillofacial region. Cell block preparation can be a useful adjunctive diagnostic tool for oral and maxillofacial diseases.

The therapeutic effect and mechanism of parthenolide in skeletal disease, cancers, and cytokine storm

Summary: This review summarizes the recent research progress of Parthenolide (PTL) as a medicinal compound, including its therapeutic effects on skeletal diseases, cancers, and inflammation-induced cytokine storm. Mechanistic investigations reveal that PTL predominantly inhibits NF-kappa B activation and other signaling pathways. It is also proposed that PTL could inhibit cytokine storms triggered by COVID-19 via blocking the activation of NF-kappa B signaling. Understanding the pharmacologic properties of PTL will assist in developing its therapeutic application for medical conditions such as arthritis, osteolysis, periodontal disease, cancers, and COVID-19-related disease.

Salivary-Based Cell-Free Mitochondrial DNA Level Is an Independent Prognostic Biomarker for Patients with Head and Neck Squamous Cell Carcinoma

Summary: Changes in the copy numbers of cell-free nuclear DNA (cf-nDNA) and cell-free mitochondrial DNA (cf-mtDNA) have diagnostic potential in head and neck squamous cell carcinoma (HNSCC). This study examined the use of saliva-based cf-nDNA and cf-mtDNA to predict overall survival in HNSCC patients. Higher copy numbers of cf-nDNA and cf-mtDNA were associated with deceased patients and poorer overall survival. Multivariate analysis showed that the copy numbers of cf-nDNA and cf-mtDNA, along with HNSCC stage, were predictors of overall survival. Saliva can be a reliable and non-invasive source to predict HNSCC patient survival, with cf-mtDNA levels acting as the sole predictor.

The impact of radiation caries on morbidity and mortality outcomes of head and neck squamous cell carcinoma patients

Summary: This study aimed to evaluate the impact of radiation caries (RC) on the morbidity and mortality outcomes of head and neck squamous cell carcinoma (HNSCC) patients. The results showed that RC patients required more dental procedures and medication prescriptions compared to the control and edentulous groups, and had a higher risk of osteoradionecrosis (ORN). Furthermore, RC patients had lower disease-free survival rates. In conclusion, RC has a negative impact on the morbidity and mortality outcomes of cancer survivors.

Summary: This study compared the outcomes of using Nd:YAG laser and ceramic bur for gingival depigmentation, and found that both methods had similar pain experience and pigmentation recurrence rates. Ceramic bur had more bleeding during surgery but faster clinical recovery. Smokers were more likely to have low depigmentation treatment stability.

Summary: This study assessed the measurement invariance of the BAS-2 across 65 nations, 40 languages, gender identities, and age groups using the BINS dataset. The results showed that the BAS-2 model has widespread applicability and there are differences in latent body appreciation across nations and languages, while differences across gender identities and age groups were small.

Cystic Lesions of the Jaws: The Top 10 Differential Diagnoses to Ponder

Summary: This article discusses the challenges in differential diagnosis of cystic lesions of the gnathic bones. Two clinical cases are presented, followed by a discussion of the ten most likely differential diagnoses. Emphasis is placed on the histologic differences between these entities, empowering readers to diagnose them confidently. Common diagnostic pitfalls and limitations in gnathic pathology are also discussed.

Summary: This study aimed to identify the barriers faced by Brazilian dentists when delivering bad news about oral and oropharyngeal cancer diagnoses. A questionnaire based on the SPIKES protocol was used, and the results showed that 44.6% of respondents expressed concern about the patient's emotional reactions, and 46.24% had never received any specific training in communicating bad news.

Active search screening for oral potentially malignant disorders and oral cancer in the city of Piracicaba

Summary: Oral cancer is a significant global public health problem, with a high number of new cases and deaths reported annually. Squamous cell carcinoma (SCC) is the most common type of oral malignancy and is often preceded by oral potentially malignant disorders (OPMDs). This research aimed to conduct an active search in a screening program in Piracicaba, Brazil. A total of 756 patients were examined, and high-risk patients were identified and referred for dental consultation. The study found that screening high-risk patients was effective in diagnosing OPMD and SCC. The authors suggest implementing this program on a larger scale to reduce the prevalence of the disease.

Use of oral nutritional supplements in irradiated patients with head and neck cancer

Summary: cfDNA and cf-mtDNA in saliva show potential as biomarkers for detecting HNSCC, with higher levels in HNSCC patients compared to healthy volunteers. The diagnostic indices based on these molecules have good accuracy in discriminating HNSCC, suggesting their utility in precision medicine.
