Identification of Atrial Fibrillation by Quantitative Analyses of Fingertip Photoplethysmogram

Atrial fibrillation (AF) detection is crucial for stroke prevention. We investigated the potential of quantitative analyses of photoplethysmogram (PPG) waveforms to identify AF. Continuous electrocardiogram (EKG) and fingertip PPG were recorded simultaneously in acute stroke patients (n = 666) admitted to an intensive care unit. Each EKG was visually labeled as AF (n = 150, 22.5%) or non-AF. Linear and nonlinear features from the pulse interval (PIN) and peak amplitude (AMP) of PPG waveforms were extracted from the first 1, 2, and 10 min of data. Logistic regression analysis revealed six independent PPG features feasibly identifying AF rhythm, including three PIN-related (mean, mean of standard deviation, and sample entropy), and three AMP-related features (mean of the root mean square of the successive differences, sample entropy, and turning point ratio) (all p < 0.01). The performance of the PPG analytic program comprising all 6 features that were extracted from the 2-min data was better than that from the 1-min data (area under the receiver operating characteristic curve was 0.972 (95% confidence interval 0.951-0.989) vs. 0.949 (0.929-0.970), p < 0.001 and was comparable to that from the 10-min data [0.973 (0.953-0.993)] for AF identification. In summary, our study established the optimal PPG analytic program in reliably identifying AF rhythm.

Summary: This study aimed to investigate the relationship between plasma ceramides and sphingosine-1-phosphate levels in acute ischemic stroke patients undergoing endovascular thrombectomy (EVT) and their clinical outcomes. The results showed that long-chain ceramides were significantly elevated in AIS patients compared to controls, while the levels of sphingosine-1-phosphate were significantly lower. Additionally, specific types of ceramides could serve as early prognostic indicators for AIS patients undergoing EVT.

Impact of Direct Oral Anticoagulant Concentration on Clinical Outcomes in Asian Patients with Atrial Fibrillation

Summary: This study reports on the association between direct oral anticoagulant (DOAC) concentration and clinical outcomes in Asian patients with atrial fibrillation (AF). The proportion of DOAC concentrations falling outside the expected range was higher than reported in clinical trials. Low trough concentration was associated with increased risk of systemic thromboembolism (SSE), while high trough concentration was associated with major bleeding. Measurement of trough DOAC concentration should be considered for patients at risk of concentration deviations.

Summary: This study found that plasma ferroptosis biomarkers at the hyperacute stage are associated with long-term outcomes in patients with acute ischemic stroke undergoing EVT. The severity of stroke at admission is correlated with levels of 4-hydroxynonenal (4-HNE) and soluble transferrin receptor (sTfR). Moreover, plasma homocysteine levels are associated with functional outcome and mortality at 3 months post-EVT.

Summary: Hsu et al. conducted a genome-wide association study on headaches in East Asians, which revealed that a single-nucleotide polymorphism (rs8072917) of the RNF213 gene is the causal variant for broadly defined headaches. This study also found associations between the causal variant and other medical conditions such as cellulitis, neck and face abscesses, and muscular symptoms. This research sheds light on the genetic architecture of headaches in individuals of East Asian ancestry and has potential implications for the development of genetic tests and treatments.

A comparative study of plasma and dried blood spot metabolomics and its application to diabetes mellitus

Summary: Metabolomics has emerged as a promising approach for understanding pathological mechanisms. This study compared plasma (PLS) and dried blood spot (DBS) samples to investigate the correlation between their metabolites. It was found that approximately 47% of the samples showed a strong correlation, 19% showed a moderate correlation, and 34% showed a low or negligible correlation. Additionally, two significant metabolites were identified in both PLS and DBS samples from diabetes mellitus (DM) patients.
