Analysis of the mutational landscape of classic Hodgkin lymphoma identifies disease heterogeneity and potential therapeutic targets

Defining the mutational landscape of classic Hodgkin lymphoma is still a major research goal. New targeted next-generation sequencing (NGS) techniques may identify pathogenic mechanisms and new therapeutic opportunities related to this disease. We describe the mutational profile of a series of 57 cHL cases, enriched in Hodgkin and Reed-Sternberg (HRS) cells. Overall, the results confirm the presence of strong genomic heterogeneity. However, several variants were consistently detected in genes related to relevant signaling pathways, such as GM-CSF/IL-3, CBP/EP300, JAK/STAT, NF-kappaB, and numerous variants of genes affecting the B-cell receptor (BCR) pathway, such as BTK, CARD11, BCL10, among others. This unexpectedly high prevalence of mutations affecting the BCR pathway suggests some requirement for active BCR signaling for cHL cell viability. Additionally, incubation of a panel of cHL cellular models with selective BTK inhibitors in vitro constrains cell proliferation and causes cell death. Our results indicate new pathogenic mechanisms and therapeutic opportunities in this disease.

Summary: Lung adenocarcinoma patients' survival and disease recurrence can be predicted by the levels of mitochondrial proteins related to fatty acid oxidation, oxidative phosphorylation, and antioxidant response. Overexpression of ATPase Inhibitory Factor 1 prevents metastatic disease in lung adenocarcinoma. Targeting fatty acid assimilation or oxidation in combination with an oxidative phosphorylation inhibitor shows therapeutic potential in mice with lung adenocarcinomas.

Cancer as an infective disease: the role of EVs in tumorigenesis

Summary: Cancer is considered an evolutionary disease where tumor cells adapt to the environment and eventually form metastasis. Communication between tumor cells and the surrounding microenvironment via extracellular vesicles (EVs) is crucial for the success of the metastatic process. Recent studies suggest that tumor-derived EVs not only modify the microenvironment to facilitate tumor progression but also induce malignant transformation in cells outside the primary tumor. Further research is needed to understand the role of EVs in cancer progression and metastasis, which may have implications for novel cancer treatments.

Summary: This article reports a case of a 77-year-old woman diagnosed with Waldenstrom macroglobulinemia (WM) following severe and sudden pancytopenia associated with cold agglutinin syndrome. Treatment with rituximab, corticosteroids, and cyclophosphamide was initiated to manage WM and underlying hemolysis. Despite improvements in hemolysis parameters, pancytopenia persisted and ibrutinib was started as a second-line treatment. The patient developed an uncommon invasive fungal infection (IFI) with bone marrow granulomatosis and myelofibrosis during treatment, highlighting a poor hematopoietic response and numerous complications.

Summary: The new lymphoma classifications now include genetics, which allows for better subclassification, risk stratification, and treatment prediction. Lymphomas have few recurrent and disease-specific mutations, and most have a diverse genetic landscape with rare mutations contributing to the clinical heterogeneity of the disease. Multiple studies have identified genetic markers for diagnostics and prognosis, and next-generation sequencing is becoming a crucial tool in clinical laboratories. This review provides a guide for using next-generation sequencing in lymphoma research, discussing the genetic alterations in commonly encountered lymphoma types and proposing targeted sequencing panels for detecting mutations and copy-number alterations in B- and NK/T-cell lymphomas.

Summary: This article presents a rare case of a 25-year-old patient diagnosed with human immunodeficiency virus (HIV) and the development of primary effusion lymphoma (PEL), Kaposi's sarcoma (KS), and multicentric Castleman's disease (MCD). Despite intensive treatment, the patient's condition did not improve. This case highlights the need for new therapies and research in this field.
