Clinicopathologic implications of immune classification by PD-L1 expression and CD8-positive tumor-infiltrating lymphocytes in stage II and III gastric cancer patients

We co-assessed PD-L1 expression and CD8(+) tumor-infiltrating lymphocytes in gastric cancer (GC), and categorized into 4 microenvironment immune types. Immunohistochemistry (PD-L1, CD8, Foxp3, E-cadherin, and p53), PD-L1 mRNA in situ hybridization (ISH), microsatellite instability (MSI), and EBV ISH were performed in 392 stage II/III GCs treated with curative surgery and fluoropyrimidine-based adjuvant chemotherapy, and two public genome databases were analyzed for validation. PD-L1(+) was found in 98/392 GCs (25.0%). The proportions of immune types are as follows: PD-L1(+)/CD8(High), 22.7%; PD-L1(-)/CD8(Low), 22.7%; PD-L1(+)/CD8(Low), 2.3%; PD-L1(-)/CD8(High), 52.3%. PD-L1(+)/CD8(High) type accounted for majority of EBV+ and MSI-high (MSI-H) GCs (92.0% and 66.7%, respectively), and genome analysis from public datasets demonstrated similar pattern. PD-L1-/CD8High showed the best overall survival (OS) and PD-L1(-)/CD8(Low) the worst (P < 0.001). PD-L1 expression alone was not associated with OS, however, PD-L1(-)/CD8(High) type compared to PD-L1+/CD8High was independent favorable prognostic factor of OS by multivariate analysis (P = 0.042). Adaptation of recent molecular classification based on EBV, MSI, E-cadherin, and p53 showed no significant survival differences. These findings support the close relationship between PD-L1/CD8 status based immune types and EBV+, MSI-H GCs, and their prognostic significance in stage II/III GCs.

Risk Stratification for Sarcopenic Obesity in Subjects With Nonalcoholic Fatty Liver Disease

Summary: This study aimed to determine the relationship between sarcopenic obesity (SO) and the risk of significant liver fibrosis or cardiovascular disease (CVD) in nonalcoholic fatty liver disease (NAFLD). Through a multicenter retrospective study, it was found that high-risk subjects with SO had a higher risk of liver fibrosis and CVD development. The results demonstrated that high-risk subjects with SO had significantly increased risks of liver fibrosis and CVD development.

Post-operative recurrence of liver cancer according to antiviral therapy for detectable hepatitis B viremia: A nationwide study

Summary: This study aimed to compare the postoperative recurrence of hepatocellular carcinoma (HCC) in patients with hepatitis B virus (HBV)-infected HCC who received antiviral therapy (AVT) compared to those who did not. The results showed that the recurrence rates of HCC were similar between AVT users and non-users with detectable serum HBV-DNA, regardless of the presence of liver cirrhosis. Therefore, initiating AVT based on detectable serum HBV-DNA can provide a similar risk of postoperative HCC recurrence.

Comprehensive Study of Microsatellite Instability Testing and Its Comparison With Immunohistochemistry in Gastric Cancers

Summary: In this study, PCR-based MSI testing and IHC for MMR protein expression were comprehensively analyzed and compared in gastric cancer patients. Among the 5,676 cases, the majority were microsatellite stable/MSI-low, while a small percentage showed MSI-high and loss of MLH1 expression. Re-evaluation of discordant/indeterminate cases revealed the need for complementary testing in order to improve accuracy.

Comparison of kidney function decline between chronic hepatitis B patients with or without antiviral therapy

Next-generation sequencing analysis of hepatitis C virus resistance-associated substitutions in direct-acting antiviral failure in South Korea

Summary: In this study, next-generation sequencing was used to analyze resistance-associated substitutions (RASs) and retreatment outcomes in South Korean patients with chronic hepatitis C virus (HCV) infection who failed direct-acting antiviral agent (DAA) treatment. The results showed that baseline NS3 and NS5A RASs were commonly present, and there was an increasing trend of NS5A RASs after failed DAA treatment in genotype 1b patients. However, RASs were rarely present in genotype 2 patients treated with sofosbuvir+ribavirin. Overall, retreatment with pan-genotypic DAA was highly successful in Korea, and active retreatment after unsuccessful DAA treatment is encouraged.

Single-port intragastric wedge resection using the tunnel method: a novel surgical approach for treating endophytic gastric subepithelial tumors
