Summary: Despite limitations in certain tumor types and patients, immunotherapies have achieved revolutionary success in cancer treatment. However, the efficacy of immunotherapies is dependent on the viability and functionality of tumor antigen-specific CD8 T cells within the immunosuppressive tumor microenvironment, where oxygen levels are often low. This study reveals that the antidiabetic drug metformin can improve CD8 T cell fitness in hypoxia, increasing their proliferation and cytokine production, and enhancing their infiltration and survival in hypoxic tumor areas. This presents a promising strategy to overcome resistance to immunotherapy in hypoxic and immunosuppressive tumors.

Summary: PD-1-based immunotherapy has been a game-changer in cancer treatment, but not all patients benefit from it and some may even experience hyperprogressive disease. In addition to CD8 T cells and conventional T cells, other cell types like B cells, regulatory T cells, natural killer cells, dendritic cells, tumor-associated macrophages, and cancer cells also express PD-1, contributing to the response to PD-1 blockade therapy. Further research is exploring the role of non-canonical PD-1 signaling in different cell types and how this knowledge can enhance the effectiveness of PD-1 blockade immunotherapy.

Summary: This study aimed to determine the relationship between sarcopenic obesity (SO) and the risk of significant liver fibrosis or cardiovascular disease (CVD) in nonalcoholic fatty liver disease (NAFLD). Through a multicenter retrospective study, it was found that high-risk subjects with SO had a higher risk of liver fibrosis and CVD development. The results demonstrated that high-risk subjects with SO had significantly increased risks of liver fibrosis and CVD development.

Summary: This study aimed to compare the postoperative recurrence of hepatocellular carcinoma (HCC) in patients with hepatitis B virus (HBV)-infected HCC who received antiviral therapy (AVT) compared to those who did not. The results showed that the recurrence rates of HCC were similar between AVT users and non-users with detectable serum HBV-DNA, regardless of the presence of liver cirrhosis. Therefore, initiating AVT based on detectable serum HBV-DNA can provide a similar risk of postoperative HCC recurrence.

Re-evaluation of discordant/indeterminate cases revealed the need for complementary testing in order to improve accuracy.

Summary: In this study, next-generation sequencing was used to analyze resistance-associated substitutions (RASs) and retreatment outcomes in South Korean patients with chronic hepatitis C virus (HCV) infection who failed direct-acting antiviral agent (DAA) treatment. The results showed that baseline NS3 and NS5A RASs were commonly present, and there was an increasing trend of NS5A RASs after failed DAA treatment in genotype 1b patients. However, RASs were rarely present in genotype 2 patients treated with sofosbuvir+ribavirin. Overall, retreatment with pan-genotypic DAA was highly successful in Korea, and active retreatment after unsuccessful DAA treatment is encouraged.
