Summary: This study investigated the pregnancy status and outcomes of women who underwent bariatric and metabolic surgery (BMS), as well as the impact of pregnancy intervals after BMS. The results showed that the abortion rate was higher in pregnancy intervals less than 2 years compared to those greater than 2 years. It is recommended that postoperative patients avoid pregnancy until their weight is stable to reduce the risk of adverse pregnancy outcomes.

Summary: This study found that bariatric surgery in a predominantly Hispanic and African American population at a community hospital in New York was associated with significant improvements in renal function parameters, including decreased urine albumin-creatinine ratio, improved glomerular filtration rate, reduced prevalence of micro- and macroalbuminuria, and decreased prevalence of hyperfiltration. These results could help inform decisions regarding indications for bariatric surgery.

A retrospective study found that 4.5% of patients who underwent bariatric surgery were diagnosed with cancer, with a higher prevalence in women. Thyroid and breast cancer were the most common types identified, and patients with cancer diagnoses were older and had lower levels of education. Further prospective studies are needed to determine the cancer risk profile of bariatric surgery patients.

Summary: Sleeve gastrectomy is a commonly performed bariatric surgery, with rising prevalence of gastric sleeve stenosis (GSS). Management with pneumatic dilation is successful but underused due to lack of diagnostic criteria. This study aimed to develop quantifiable endoscopic criteria for GSS diagnosis, finding an association between narrowest to widest gastric lumen diameter ratio and presence of bilious fluid with EndoFLIP and gastric lumen morphology diagnosis. Further studies are needed to validate these criteria.

In 2015, the EGCA launched a project to establish a list and definitions of post-operative complications specific for gastrectomy. Summary: Although ERAS programs have been widely adopted for colorectal surgery, their implementation for upper gastrointestinal surgery is challenging in Italy. A survey showed that Italian centers dedicated to gastric cancer do not fully follow evidence-based practices and guidelines of ERAS for gastric surgery. This may be due to surgeons' hesitation to introduce radical changes to traditional postoperative management after gastrectomy.

Summary: This qualitative study investigated the decision-making process of surgeons regarding protective ileostomy creation after rectal cancer surgery. The results showed that 76% of surgeons supported the creation of ileostomy. When making the decision, 88% of surgeons considered risk factors. Emotions influenced the decision-making process in 22% of cases, and personal experiences influenced it in 49% of cases. The most frequently considered risk factors were the distance between the anus and the tumor (96%), neoadjuvant chemoradiotherapy (88%), positive intraoperative leak test (65%), blood loss (37%), and immunosuppression therapy (35%).

Summary: This study compared the outcomes of Self-Expandable Metal Stents (SEMS) and Endoscopic Vacuum Therapy (EVT) in the treatment of anastomotic leaks after esophagectomy/gastrectomy, with a focus on oncologic surgery. The results showed that EVT was more effective and had fewer complications compared to SEMS. However, in the oncologic surgery subgroup, there was no significant difference in the success rate between the two treatments.

Summary: Esophageal adenocarcinoma (EAC) is a severe malignancy with increasing incidence, poorly understood pathogenesis, and low survival rates. TP53 gene alterations, especially missense mutations, negatively affect cancer-specific survival in EAC. HNF1alpha gene disruption is associated with other gene alterations and is identified as a new EAC-mutated gene.
