Summary: Glaucoma, a chronic disease that leads to irreversible vision loss, is the second leading cause of such loss worldwide. Non-adherence to glaucoma medications is a major problem, with up to 50% of patients failing to receive the intended benefits of treatment. Various factors, such as medication type, physician-patient communication, and patient behavior and lifestyle, contribute to non-adherence. Failing to take medication as prescribed increases the risk of poor treatment outcomes and disease progression. Therefore, adopting a patient-centered approach, educating patients about their chronic disease, and assessing their views on treatment are crucial in addressing adherence.

Summary: Poor adherence to topical glaucoma medications is detrimental to the visual field outcomes in glaucoma patients. Various barriers to adherence, such as forgetfulness and medication side effects, need to be overcome to slow down disease progression. Preservative-free medications and sustained drug delivery systems show promise in reducing ocular surface disease symptoms and improving adherence. Smart drug delivery-connected devices and minimally invasive glaucoma procedures are potential solutions to aid in adherence and improve monitoring and treatment.

Summary: This review discusses common reasons for nonadherence to ocular medical therapy and reviews existing and emerging options to reduce nonadherence and optimize therapeutic outcomes. Nonadherence can result from patient-related issues and healthcare-related issues, and several strategies have been developed to overcome these barriers. Identifying nonadherence and its causes helps in developing strategies to address it, and many common causes can be mitigated through various approaches.

Summary: Glaucoma is a progressive optic neuropathy characterized by retinal ganglion cell loss and visual field deterioration. While the exact cause of glaucoma is still unknown, elevated intraocular pressure has been identified as a modifiable risk factor. Treatment with eye drops to lower intraocular pressure has been proven effective in reducing the risk of disease progression. However, many patients with glaucoma struggle with adherence to medication, which is associated with poor treatment outcomes and increased healthcare costs.

The results showed that only 13.0% of participants complied with ART treatment, with Greek women and those with higher education levels being more likely to comply. Participants who complied with ART had significantly higher knowledge and attitude scores.

Summary: Effective eye drop delivery systems for treating diseases of the posterior segment have yet to be clinically validated. In this study, a combination of a hypotonic gel-forming eye drop delivery system and an engineered melanin binding and cell-penetrating peptide was used to achieve effective and sustained therapeutic drug concentration, enhancing neuroprotection in a rat model of optic nerve injury.
