Summary: This study aimed to report and compare the outcomes of different revisional techniques for failed anti-reflux surgery. By retrospectively analyzing patients who underwent revisional surgery between 2016 and 2021, it was found that conversion to RYGB may offer better resolution of reflux than RF, especially for obese patients.

Summary: Elevated plasma bile acids and FGF19 levels were observed in subjects post-RYGB, with postprandial hypoglycemia occurring in a subset of patients. Those who experienced hypoglycemia showed higher postprandial bile acid levels coinciding with increased GLP-1 and insulin responses during meal tests. Additionally, computer simulations suggested that altered intestinal transit may underlie exaggerated postprandial bile acid responses in hypoglycemic subjects.

Summary: Bariatric surgery, especially Roux-en-Y gastric bypass (RYGB), has a significant impact on the metabolism and gut microbiota of patients, leading to changes in metabolites and bacterial composition that are associated with successful weight loss and metabolic improvements. Further research is needed to understand the functional clinical outcomes of these metabolic changes.

Summary: Laparoscopic Roux-en-Y gastric bypass (RYGB) is the preferred surgical procedure for treating severe obesity and gastroesophageal reflux disease (GERD). It provides satisfactory weight loss and resolution of GERD symptoms in most patients, but about 24.4% of patients may experience GERD symptoms postoperatively.

Summary: The study found that redo fundoplication and conversion to Roux-en-Y anatomy with a gastric diversion are equally effective in resolving reflux symptoms in patients with failed fundoplications. Despite higher BMI, longer operative times, and greater blood loss in RYGD patients, they showed better resolution of other comorbidities and greater reduction in excess body weight.

Summary: Postoperative dumping syndrome is a common complication after Roux-en-y gastric bypass for morbid obesity, typically managed with dietary or pharmacologic measures. In cases where conservative treatment is ineffective, surgical intervention may be considered. The use of a modified Nissen fundoplication for late dumping syndrome after RYGB shows promise as a surgical alternative.

Summary: This study compared the short-term and long-term outcomes as well as the quality of life of patients undergoing Roux-en-Y (R-Y) or Billroth II with Braun (BII + B) reconstruction after distal gastrectomy. The results showed that R-Y reconstruction had a longer operative time but similar perioperative variables, complication rates, nutritional outcomes, and quality of life scores compared to BII + B reconstruction. However, R-Y reconstruction reduced the incidence of residual gastritis, bile reflux, and reflux esophagitis, and provided better postoperative quality of life for patients.

Summary: The study found high rates of occult signet ring cell gastric cancer among carriers of CDH1 P/LP variants with no family history of gastric cancer, suggesting that the germline CDH1 P/LP variants have a highly penetrant gastric phenotype regardless of family history. This data may be useful for counseling families with CDH1 variants presumed to have hereditary lobular breast cancer.

Summary: Wild-type KIT and PDGFRA gastrointestinal stromal tumors are rare and have limited treatment options. This study analyzed data from a large national database to identify factors influencing overall survival, finding that tumors in the small intestine and with high mitotic count were associated with decreased OS. Summary: This study aimed to investigate the potential survival benefit of adjuvant chemotherapy (AC) in patients with stage II colon cancer. The results showed that patients who received AC had significantly increased overall survival (OS) across different numbers of high-risk features. Subgroup analysis revealed a reduced risk of death in patients receiving AC and an increased OS in patients with T4 tumors.

Summary: Pathogenic and likely pathogenic variants in the CDH1 gene are associated with increased risk of gastric and breast cancers, as well as hereditary cleft lip and palate (CLP). This study aimed to determine the prevalence of CLP in families with these variants. The results showed a high prevalence of CLP in families carrying CDH1 variants, with no clear genotype-phenotype pattern. Genetic testing for CDH1 should be considered in families with CLP and a history of gastric or lobular breast cancer.

Summary: Hereditary diffuse gastric cancer (HDGC) is an autosomal dominant cancer syndrome associated with a high risk of diffuse-type gastric cancer. Loss-of-function mutations in the CDH1 gene have been identified as the primary cause of HDGC. Summary: DCD liver transplantation is an underutilized method that can increase access to liver transplantation. With proper donor and recipient selection, matching, surgical technique, and perioperative management, these transplants can achieve similar patient and graft survival rates as DBD liver transplantation. The main limitations in the further growth of DCD liver transplantation are driven by risk aversion, and the current experience is mainly concentrated in large centers. With the advancement of regional and machine perfusion techniques, the definition of a viable DCD liver allograft may be redefined, encouraging wider usage and acceptance.

Summary: hCCA is a biologically aggressive disease and surgical resection is the only curative treatment. Factors such as patient's physical condition, tumor involvement, residual liver volume, and assessment of metastatic disease heavily influence the feasibility of surgical resection. A multidisciplinary, holistic, and individualized approach is critical for accurately determining resectability and optimizing clinical outcomes for patients with hCCA.
