Platelet concentrates for bone regeneration: Current evidence and future challenges

Activated platelet concentrates are autologous blood preparations containing supraphysiological concentration of platelets. Platelet concentrates are commonly used for bone regeneration purposes based on the fact that growth factors released from activated platelets alpha granules have osteoinductive effects on bone cells. Although most preclinical and clinical studies show that platelet concentrates improve the outcomes of bone regeneration procedures, some studies reported conflicting results and even negative effects on bone healing. Several confounding parameters have been suggested as possible reasons for such inconsistencies (i.e. preparation and activation methods). However, heterogeneity in clinical studies makes drawing evidence-based conclusions difficult. On the other hand, recent findings show that the constituents of platelets dense granules (i.e. serotonin, ATP, Ca2+) have potential inhibitory effects on bone metabolism. Accordingly, we suggest that a partial explanation for the conflicting results could be the potential negative effects that dense granules may have on bone healing.

Salivary LDH in oral cancer and potentially malignant disorders: A systematic review and meta-analysis

Summary: This study evaluated the increase in salivary lactate dehydrogenase (LDH) levels in patients with oral cancer (OC) or oral potentially malignant disorders (OPMD) compared to a healthy control group (CG). Results showed that LDH levels were higher in the OC group and patients with oral leucoplakia and oral submucous fibrosis also had higher levels. Salivary LDH determination may be a useful method for screening and tracking OC and OPMD, but further studies are needed to establish precise cutoff values.

Summary: This study aimed to evaluate the influence of perforating the cortex of the recipient site on cancellous allogeneic block graft integration and revascularization in the maxilla. The results showed that perforating the cortex of the recipient site did not have a significant effect on angiogenesis and bone formation within the block graft.

Efficacy of aerosol reduction measures for dental aerosol generating procedures

Summary: Aerosol particles generated by dental procedures can potentially transmit infectious diseases and contain carcinogens. This study aimed to quantify aerosols produced during drilling and scaling procedures and evaluate measures for aerosol reduction. The results showed that high-volume evacuators (HVEs) and air purifiers effectively reduced aerosol concentration, with the best reduction achieved by using both measures simultaneously.

Aragonite toothpaste for management of dental calculus: A double-blinded randomized controlled clinical trial

Summary: This study evaluates the clinical and radiographic outcomes of the Periosteal Pocket Flap Technique (PPF) combined with the use of xenogeneic and autologous bone and Plasma Rich in Growth Factors (PRGF) compared to conventional Guided Bone Regeneration (GBR) procedures. The results demonstrate that the combination of autograft/xenograft and PRGF in PPF technique is a simpler, cheaper, and faster technique than GBR technique for achieving moderate lateral bone augmentation in implant treatment.

Summary: This study compared the release of PDGF, VEGF, IGF-I, and IL-1β in PRGF and L-PRF, and evaluated their biological implications in osteoblasts. The findings showed that PRGF exhibited a slower controlled release at day 14, contributing to a higher synthesis of type I collagen. L-PRF released significantly higher amounts of IL-1β compared to PRGF.

Chronotherapy in dentistry

Summary: This scoping review systematically maps the evidence supporting chronotherapy in dentistry and identifies knowledge gaps. It found that chrono-radiotherapy and chrono-chemotherapy can reduce treatment side effects and improve therapeutic response in cancer patients, leading to higher survival rates. Animal studies suggest that tooth movement and periodontal tissue response to orthodontic forces follow a diurnal rhythm that may impact bone metabolism. Profound and prolonged local anesthesia can be achieved when injected in the evening. Although the overall quality of the included studies was low, chronotherapy applications in dentistry appear to have favorable outcomes, especially in head and neck cancer treatments.

Electrochemical decontamination of titanium dental implants. An in vitro biofilm model study

Summary: The objective of this study was to investigate the effect of electrochemical treatment on biofilms developed on titanium dental implants. The study found that electrochemical treatment had an impact on the structure and bacterial composition of the biofilms. The reduction in bacterial counts, especially for Fusobacterium nucleatum, demonstrated a bactericidal effect of the treatment.

Drug Delivery Systems in Regenerative Medicine: An Updated Review

Summary: Modern drug discovery methods have led to the development of new drugs with significant potential, but properties like solubility and administration challenges may hinder their benefits. Biotechnology advances have also resulted in the development of molecules with short half-lives, requiring frequent administration. Controlled release systems are necessary to enhance treatment efficacy and improve patient compliance. This review provides an overview of different approaches used for drug delivery.

Patient-reported outcomes and framework fit accuracy of removable partial dentures fabricated using digital techniques: A systematic review and meta-analysis

Summary: The evidence on patient-reported outcomes and clinical performance of digitally fabricated RPDs compared to conventionally fabricated dentures is limited. Some studies suggest that digitally fabricated RPDs are associated with higher patient satisfaction, while others indicate that conventionally fabricated RPDs have better framework fit accuracy. Overall, both digitally and conventionally fabricated metal RPD frameworks showed clinically acceptable fit.

Effect of time-dependent ibuprofen administration on the post operatory after impacted third molar extraction: a cross-over randomized controlled trial

Summary: The purpose of this study was to evaluate the time-dependent administration of ibuprofen in a lower third molar extraction model. The results showed that there were no significant differences in postoperative swelling and pain perception between the control group, which took ibuprofen every 8 hours, and the experimental group, which took ibuprofen only at breakfast and lunch and replaced dinner intake with a placebo. However, trismus was significantly higher in the experimental group at 72 hours postoperatively.
