Esophageal Dysmotility in Patients following Total Laryngectomy

Objectives. Dysphagia is common in total laryngectomees, with some symptoms suggesting esophageal dysmotility. Tracheoesophageal (TE) phonation requires effective esophago-pharyngeal air passage. Hence, esophageal dysmotility may affect deglutition or TE phonation. This study aimed to determine (1) the characteristics of esophageal dysmotility in laryngectomees, (2) whether clinical history is sensitive in detecting esophageal dysmotility, and (3) the relationship between esophageal dysmotility and TE prosthesis dysfunction. Study Design. Multidisciplinary cross-sectional study. Setting. Tertiary academic hospital. Subjects and Methods. For 31 participants undergone total laryngectomy 1 to 12 years prior, clinical histories were taken by a gastroenterologist and a speech pathologist experienced in managing dysphagia. Esophageal high-resolution manometry was performed and analyzed using Chicago Classification v3.0. Results. Interpretable manometric studies were obtained in 23 (1 normal manometry). Esophageal dysmotility patterns included achalasia, esophagogastric junction outflow obstruction, diffuse esophageal spasm, and other major (30%) and minor (50%) peristaltic disorders. The sensitivity of predicting any esophageal dysmotility was 28%, but it is noteworthy that patients with achalasia and diffuse esophageal spasm (DES) were predicted. Two of 4 participants with TE puncture leakage had poor esophageal clearance. Of 20 TE speakers, 12 had voice problems, no correlation between poor voice, and any dysmotility pattern. Conclusions. Peristaltic and lower esophageal sphincter dysfunction are common in laryngectomees. Clinical history, while not predictive of minor motor abnormalities, predicted correctly cases with treatable spastic motor disorders. Dysmotility was not associated with poor phonation, although TE puncture leakage might be linked to poor esophageal clearance. Esophageal dysmotility should be considered in the laryngectomees with persisting dysphagia or leaking TE puncture.

Benchmarking network-based gene prioritization methods for cerebral small vessel disease

Summary: Different algorithms for prioritizing disease-associated genes were benchmarked for application in cerebral small vessel disease, with random walk with restart on the heterogeneous network (RWRH) showing the best performance. Although RWRH had bias caused by degree centrality, it had overall better performance for cSVD. Current pure network-based gene prioritization algorithms may not discover novel disease-associated genes not linked to known ones.

Evaluation of oropharyngeal deglutitive pressure dynamics in patients with Parkinson's disease

Summary: Oropharyngeal dysphagia is common in Parkinson's disease (PD) and its neurophysiology is complex. This study aimed to investigate the changes in oropharyngeal pressure dynamics in PD and their correlation with clinical characteristics. The results showed that PD patients had impaired upper esophageal sphincter relaxation, increased flow resistance, and higher pharyngeal contractility compared to healthy controls. PD severity and dysphagia severity were associated with certain parameters of oropharyngeal function. These findings suggest that UES dysfunction and increased flow resistance may contribute to dysphagia in PD.

Dual Encoder-Based Dynamic-Channel Graph Convolutional Network With Edge Enhancement for Retinal Vessel Segmentation

Summary: Retinal vessel segmentation is crucial for diagnosing fundus diseases, and this paper proposes a novel method called DE-DCGCN-EE. It uses a dual encoder to preserve edge information during down-sampling, a dynamic-channel graph convolutional network to address the issue of insufficient channel information utilization, and an edge enhancement block to improve the accuracy of fine blood vessel segmentation. Experimental results show that DE-DCGCN-EE outperforms other state-of-the-art methods on five retinal image datasets, indicating its potential clinical application.

Transient hypopharyngeal intrabolus pressurization patterns: Clinically relevant or normal variant?

Summary: Translating: In oropharyngeal dysphagia, impaired pharyngoesophageal junction (PEJ) opening is reflected by an elevated hypopharyngeal intrabolus pressure (IBP), quantifiable using pharyngeal high-resolution manometry with impedance (P-HRM-I). Transient intrabolus pressurization (TP) phenomena are not sustained and last for only a brief period. We hypothesized that TP patterns reflect impaired coordination between timing of hypopharyngeal bolus arrival and PEJ relaxation. Chinese Summary: Transient intrabolus pressurization (TP) in oropharyngeal dysphagia reflects mistimed coordination between bolus arrival and PEJ relaxation, with higher IBP and shorter PEJ relaxation time during TP swallows. In Patients, RT-DCL time difference correlated with IBP, suggesting pathological TP swallows impede bolus flow due to mistimed coordination. English Summary: In oropharyngeal dysphagia, transient intrabolus pressurization (TP) patterns may indicate impaired coordination between bolus arrival and PEJ relaxation, leading to an increased IBP and shorter PEJ relaxation time during TP swallows. The mistimed coordination in TP swallows may impede bolus flow and result in a brief period of pharyngeal chamber pressurization by muscular forces, with IBP correlating with RT-DCL time difference in Patients.

The Role of Hounsfield Unit in Intraoperative Endplate Violation and Delayed Cage Subsidence with Oblique Lateral Interbody Fusion

Summary: This retrospective study aimed to investigate the risk factors for intraoperative endplate violations and delayed cage subsidence in oblique lateral interbody fusion (OLIF) surgery, as well as the association between low Hounsfield unit (HU) values and these complications. The results showed that intraoperative endplate violations and delayed cage subsidence are not uncommon in OLIF surgery, and HU values can serve as good predictors for these complications.

Gastric organoids: Advancing the study of H. pylori pathogenesis and inflammation

Using high resolution manometry impedance to diagnose upper esophageal sphincter and pharyngeal motor disorders

Summary: This study aimed to establish a methodology for diagnosing oropharyngeal disorders using P-HRM-I. The results showed that UES relaxation pressure was the best indicator for differentiating patients from controls and diagnosing UES disorders. A diagnostic scheme was also devised to identify patients with UES disorder or propulsive disorder. Additionally, the study found that patients with pharyngeal pressurizations and evidence of reduced UES relaxation and/or distensibility had higher aspiration scores.

5-Year progression prediction of endplate defects: Utilizing the EDPP-Flow convolutional neural network based on unbalanced data

SpineGEM: A Hybrid-Supervised Model Generation Strategy Enabling Accurate Spine Disease Classification with a Small Training Dataset
