Alignment options for total knee arthroplasty: A systematic review

In spite of improvements in implant designs and surgical precision, functional outcomes of mechanically aligned total knee arthroplasty (MA TKA) have plateaued. This suggests probable technical intrinsic limitations that few alternate more anatomical recently promoted surgical techniques are trying to solve. This review aims at (1) classifying the different options to frontally align TKA implants, (2) at comparing their safety and efficacy with the one from MA TKAs, therefore answering the following questions: does alternative techniques to position TKA improve functional outcomes of TKA (question 1)? Is there any pathoanatomy not suitable for kinematic implantation of a TKA (question 2)? A systematic review of the existing literature utilizing PubMed and Google Scholar search engines was performed in February 2017. Only studies published in peer-reviewed journals over the last ten years in either English or French were reviewed. We identified 569 reports, of which 13 met our eligibility criteria. Four alternative techniques to position a TKA are challenging the traditional MA technique: anatomic (AA), adjusted mechanical (aMA), kinematic (KA), and restricted kinematic (rKA) alignment techniques. Regarding osteoarthritic patients with slight to mid constitutional knee frontal deformity, the KA technique enables a faster recovery and generally generates higher functional TKA outcomes than the MA technique. Kinematic alignment for TKA is a new attractive technique for TKA at early to mid-term, but need longer follow-up in order to assess its true value. It is probable that some forms of pathoanatomy might affect longer-term clinical outcomes of KA TKA and make the rKA technique or additional surgical corrections (realignment osteotomy, retinacular ligament reconstruction etc.) relevant for this sub-group of patients. Longer follow-up is needed to define the best indication of each alternative surgical technique for TKA. Level I for question 1 (systematic review of Level I studies), level 4 for question 2. Summary: This study reports on the revision rate, clinical outcomes, and radiological signs in patients who underwent uncemented TKA using restricted KA principles. Results showed that in 99% of cases with an average follow-up of 49 months, there was no implant revision for aseptic loosening, indicating good short-term functional outcomes and anticipated favorable mid- to long-term implant survivorship.

Variation in the patellar tendon moment arm identified with an improved measurement framework

Summary: The mechanical advantage of the knee extensor mechanism is heavily influenced by the patellar tendon moment arm (PTMA), which varies due to factors such as gender and knee size. This study optimized PTMA measurement and found that males have a consistently larger PTMA compared to females, with knee size also playing a significant role. The peak PTMA occurs closer to terminal extension in females, despite scaling eliminating sex differences in PTMA magnitude.

Liner dissociation in a large-diameter ceramic-bearing acetabular component: a report of five cases

Summary: This report presents cases of ceramic liner dissociation from a monoblock cup in 5 patients. The patients experienced sudden pain and audible noise, and the displacement of the liner was confirmed by X-ray and CT scans. Further research should consider avoiding cup diameters over 40mm, improving the locking mechanism, or providing a true monoblock acetabular implant.

This highlights the importance of following ERAS principles when implementing an outpatient THA/TKA program.

Total and partial knee arthroplasty implants that maintain native load transfer in the tibia

The risk of tibial eminence avulsion fracture with bi-unicondylar knee arthroplasty A FINITE ELEMENT ANALYSIS

Summary: The study aimed to compare the risk of tibial eminence avulsion intraoperatively for bi-unicondylar knee arthroplasty (Bi-UKA), isolated medial unicondylar knee arthroplasty (UKA-M), and bicruciate-retaining total knee arthroplasty (BCR-TKA) and consider the impact of implant positioning, overstuffing, and sex. The results showed that lateral translation of the medial implant in Bi-UKA and joint overstuffing had the greatest impact on risk increase. Smaller and less dense tibiae had a higher risk, and removing anterior bone doubled the risk.

Evolution of Concepts: Can Personalized Hip Arthroplasty Improve Joint Stability?

Summary: Hip arthroplasty procedures have high success and reproducibility. However, hip dislocations are the most common cause for revisions within the first two years post-surgery, despite the majority of the dislocations occurring within the 'safe zone' of the acetabular component. The limitations of this boundary neglect the variability of individual hip anatomy and functional pelvic orientation. To address hip instability and enhance outcomes, the alternative concept of functional acetabular orientation is discussed, including the kinematic alignment technique and the use of large-diameter heads in preventing total hip arthroplasty dislocations.

Three-Dimensional Preoperative Planning Software for Hip Resurfacing Arthroplasty
